Dupuytren's contracture: an analysis of outcomes of percutaneous needle fasciotomy versus open fasciectomy.

BACKGROUND: Percutaneous needle fasciotomy (PNF) is a minimally invasive technique used to manage Dupuytren's contracture. We compared outcomes of PNF versus open fasciectomy (OF) to examine the suitability of PNF in Australia. METHOD: A retrospective cohort study using two questionnaires regarding Dupuytren's treatment was used to assess patients with uncomplicated primary disease. The primary outcomes were immediate and medium-term correction of contracture (2-year mean follow-up to time of survey). Secondary outcomes were patient satisfaction and complications including tendon/nerve injury

Foot pain after a plantar fasciotomy: an MR analysis to determine potential causes.

PURPOSE: The purpose of this work was to determine potential causes of foot pain in patients who have had a surgical release of the plantar fascia for treatment of fasciitis. METHOD: We studied 17 patients (15 women, 2 men; age range 22-59 years, mean 40 years) with foot pain after undergoing a fasciotomy. Fourteen unilateral and three bilateral procedures accounted for the 20 ankles evaluated. Mean duration after surgery was 22 months (range 3-53 months). Each patient was instructed to localize the pain to a region of the foot; classify the pain as new onset, persistent, or recurrent; and ch

[Computerized tomography and magnetic resonance tomography studies before and after para-tibial fasciotomy].

Paratibial fasciotomy has been employed since 1981 to treat patients with chronic venous insufficiency (CVI) and therapy-resistant leg ulcers with severe lipodermatosclerosis. The characteristic morphological changes in CVI were evaluated shown before and after paratibial fasciotomy using computerized tomography (CT) and magnet resonance tomography (MRT). 10 patients (6 female, 4 male) were examined by CT and MRT pre- and postoperatively. Preoperatively there is a clear thickening of the dermis and subcutaneous field. In addition, the area around the achilles tendon is thickened, the fasciae a

THE PROTECTIVE EFFECT OF FASCIOTOMY COMBINED WITH HYPERTONIC SALINE FLUSHING FOR CRUSH SYNDROME IN RATS.

In natural disasters such as earthquakes and landslides, the main problem that wounded survivors are confronted with is crush syndrome (CS). The aim of this study was to explore more convenient and effective early treatment measures for it. In the present study, we investigated the protective effect of fasciotomy combined with different concentration of hypertonic saline flushing with CS rats. CS model was prepared by compressing the buttocks and both lowering limbs of rats with 7.5 kg dumbbell for 4 h. The rats were divided into 10 groups, which were normal control group, model group, incisio

Acute compartment syndrome and fasciotomy after a viper bite in Italy: a case report.

BACKGROUND: Bites caused by European vipers are rare medical emergencies but can occasionally cause life-threatening complications. Viper venom causes local symptoms, which can be accompanied by systemic manifestations in severe cases. The local effects of snakebites include edema and, more rarely, necrosis and compartment syndrome. The consequences of envenomation are often more pronounced in children due to their smaller body size. CASE PRESENTATION: We present the case of a 6-year-old girl who experienced multiple viper bites in the lower limb in northwest Italy. The girl received supporti

RANDOMIZED CONTROLLED TRIAL OF LIMITED FASCIOTOMY WITH INJECTION OF ADIPOSE GRAFT FOR DUPUYTREN'S DISEASE.

OBJECTIVE: Dupuytren's disease is a genetic disorder related to the proliferation of myofibroblasts. The pluripotent property of stem cells present in adipose tissue inhibits myofibroblast proliferation. Our study sought to evaluate the effect of stem cell-rich fat grafts in patients that underwent limited fasciotomy. METHODS: We studied 45 patients, in a single-blind, prospective, randomized clinical trial. All patients underwent limited fasciotomy. In one group, fat graft was injected. RESULTS: The total passive extension deficit results did not exhibit a significant difference. Fat group

Small incision fasciotomy in a patient with compartment syndrome and peripheral arterial occlusive disease.

An 82-year-old-man with a previous history of atrial fibrillation was admitted with acute limb ischemia. Emergent embolectomy was performed, but after the operation, the patient suffered from recurrent ischemic pain. Peripheral angiography revealed thrombosis of the distal popliteal artery due to pre-existing peripheral arterial occlusive disease. Bypass surgery of the popliteal artery and posterior tibial artery was then performed. Although peripheral blood flow was restored after the operation, he suffered from compartment syndrome the next day. The patient was treated with an emergent bed-s

Comparing Treatment Approaches for Dupuytren Contracture: Percutaneous Needle Fasciotomy With and Without Corticosteroids.

INTRODUCTION: Percutaneous needle fasciotomy (PNF) is a standard treatment for Dupuytren contracture. In some instances, clinicians administer intraoperative corticosteroids (ICS) and/or postoperative corticosteroids (PCS) alongside PNF to improve outcomes. However, few studies have systematically investigated the efficacy of ICS and/or PCS as adjuvant therapies to PNF. The purpose of this study was to review clinical outcomes between PNF alone and PNF with corticosteroid use in the treatment of Dupuytren contracture. METHODS: A systematic review was done in February 2024 using SCOPUS and OVI

Surgery, Needle Fasciotomy, or Collagenase Injection for Dupuytren Contracture : A Randomized Controlled Trial.

BACKGROUND: Surgery, needle fasciotomy, and collagenase injection are used to treat Dupuytren contracture. The treatment decision requires balancing initial morbidity and costs of surgery against its potential long-term benefits over needle fasciotomy and collagenase. OBJECTIVE: To compare the effectiveness of surgery, needle fasciotomy, and collagenase injection at 3 months and 2 years (secondary time points of the trial). DESIGN: A multicenter, randomized, outcome assessor-blinded, superiority trial. (ClinicalTrials.gov: NCT03192020). SETTING: 6 public hospitals in Finland. PARTICIPANTS:

Single-Incision 4-Compartment Fasciotomy of the Lower Leg: Safe, Effective, and Advantageous.

The optimal surgical approach for acute compartment syndrome (ACS) of the lower leg remains debatable. Although a majority of surgeons tend to use a 2-incision approach to 4-compartment fasciotomies, the authors have used a single-incision technique followed by protocolized, staged skin closure. The purpose of this study was to determine the safety, efficacy, and complication rate of this strategy. This retrospective study included all patients treated for ACS by a single surgeon during a 3-year period. A protocol was used including a single-incision technique followed by vacuum-assisted wound

Clinical Outcomes Following an Open Gastrocnemius Recession Combined With an Endoscopic Plantar Fasciotomy.

UNLABELLED: At present, there is a paucity of literature describing the clinical outcomes following a combined gastrocnemius recession and endoscopic plantar fasciotomy. The purpose of the present report is to describe our preliminary findings following a combined gastrocnemius recession and endoscopic plantar fasciotomy for the treatment of plantar fasciitis and gastrocnemius equinus recalcitrant to conservative therapy. Twenty-five consecutive ankles in 23 patients (age 51.2 ± 12.5 years, 4 men) met the inclusion and exclusion criteria. The average follow-up was 3.7 months. Compared across t

U.S. Military Experience From 2001 to 2010 With Extremity Fasciotomy in War Surgery.

INTRODUCTION: After trauma, compartment syndrome of the extremities is a common, disabling, and-if managed suboptimally-lethal problem. Its treatment by surgical fasciotomy continues to be useful but controversial. The purpose of this survey is to measure survival and fasciotomy in a large trauma system to characterize trends and to determine if fasciotomy is associated with improved survival. METHODS: We retrospectively surveyed data from a military trauma registry for U.S. casualties from 2001 to 2010. Casualties had extremity injury or extremity fasciotomy. We associated survival and fasci

Management of complex extremity injuries: tourniquets, compartment syndrome detection, fasciotomy, and amputation care.

Historically, complex extremity injuries, otherwise known as mangled extremities, have been difficult management problems. This is especially true in multiply-injured patients where many priorities exist and where amputation is considered a failure of limb salvage. Over the past decade, advances in the total management of complex extremity injuries, from the placement of life-saving and limb-saving tourniquets in the prehospital setting to the advancement of prosthetics and rehabilitation months to years later, have resulted in superb functional results regardless of whether limb salvage or am

[Does digital percutaneous fasciotomy in Dupuytren's contracture provokes collateral nerves injuries? 25 cases].

It is well known that percutaneous fasciotomy must not be realized at the digital level because of the risks of iatrogenic nervous injury. The purpose of this study is to verify if the percutaneous fasciotomy in the digital level is responsible or not for sensory complication by injury of the collateral nerves, thanks to the precise sensory analysis of a series of twenty five palmodigital or digital fasciotomies. Our series included 20 patients operated on between September 2006 and June 2008, as a total of 25 fingers. Only one patient presented preoperative sensory disorders. It was a multi-o

Overdiagnosis of necrotizing fasciitis: clinical-radiological suspicion versus intraoperative findings in 62 fasciotomy patients.

INTRODUCTION: Necrotising fasciitis (NF) is a rare but life-threatening soft tissue infection, whose diagnosis remains challenging due to non-specific early clinical signs. This retrospective single-center study aimed to compare the clinical, radiological, and microbiological characteristics, as well as outcomes, of patients undergoing fasciotomy for suspected NF, distinguishing between those with surgically confirmed necrosis and those without. METHODS: Data from 62 adults who underwent fasciotomy for suspected NF between 2013 and 2023 were retrospectively analyzed. Patients were stratified

Trans-Ulnar Single Incision Fasciotomy for Decompression of Forearm Compartment Syndrome: A Cadaveric Study.

OBJECTIVES: Acute compartment syndrome (ACS) is a medical emergency that requires timely intervention, and delays in treatment can lead to severe complications such as nerve injury, muscle necrosis, amputation, and even death. Definitive treatment of ACS requires a fasciotomy. Currently, there is no consensus on the best approach for a forearm fasciotomy; compartment release is most commonly done through volar or combined volar and dorsal incisions. A trans-ulnar single incision approach has been demonstrated to be effective in a case report. This study investigates if a trans-ulnar single inc
