Treatment of fecal incontinence-is there a light in the end of the tunnel?

Fecal incontinence (FI) is a common disease with higher incidence rates in the elderly population. Treatment of affected patients remains challenging and ranges from conservative management to surgical techniques. Despite all efforts patients often undergo several therapeutic measurements to achieve reasonable functional improvements.Although sacral neuromodulation still remains a key therapy with success rates up to 80%, a significant number of patients do not respond sufficiently and require further treatment.Several artificial bowel sphincter devices exist, which can lead to better function

Diagnosis and management of fecal incontinence in children and adolescents.

Fecal incontinence (FI) is a commonly occurring disease of high concern. It is characterized by voluntary and involuntary defecation in children and adolescents. It is not only a physical disease but also a psychological and behavioral disorder. FI poses a serious burden on individuals and their families and therefore has become a social problem. Unfortunately, the management of FI among children is still a challenge because the etiology varies widely. Constipation has been found to be the most common cause, while sphincter dysfunction and neurogenic abnormalities may also play a role. Current

Review of the diagnosis, management and treatment of fecal incontinence.

Fecal incontinence is a common problem affecting women but is underreported because of patients' reluctance to discuss their symptoms and an inconsistent use of screening tools by physicians. Obstetric injury from vaginal delivery is the principal cause of fecal incontinence among young women. Prevalence rates are highest in the elderly, especially those with declining cognitive function. There are multiple diagnostic tests including anal manometry, endosonography, defecography, and pudendal nerve latency testing to assist physicians in the workup of patients and aid in the selection of approp

Fecal incontinence in systemic sclerosis is secondary to neuropathy.

OBJECTIVES: Systemic sclerosis (SSc) is a chronic multi-system autoimmune disorder with gastrointestinal tract (GIT) involvement in up to 90% of patients and anorectal involvement occurs in up to 50% of patients. The pathogenesis of gastrointestinal abnormalities may be both myogenic and neurogenic. We aimed to identify which anorectal physiological abnormalities correlate with clinical symptoms and thus understand the pathophysiology of anorectal involvement in SSc. METHODS: In total, 44 SSc patients (24 symptomatic (Sx) (fecal incontinence) and 20 asymptomatic (ASx)) and 20 incontinent cont

Bowel management for the treatment of pediatric fecal incontinence.

Fecal incontinence is a devastating underestimated problem, affecting a large number of individuals all over the world. Most of the available literature relates to the management of adults. The treatments proposed are not uniformly successful and have little application in the pediatric population. This paper presents the experience of 30 years, implementing a bowel management program, for the treatment of fecal incontinence in over 700 pediatric patients, with a success rate of 95%. The main characteristics of the program include the identification of the characteristics of the colon of each

Fecal incontinence in an inpatient population: a not uncommon finding.

The prevalence of fecal incontinence (FI) in hospitalized patients was determined, as well as the frequency with which physicians query about FI. Using a standardized questionnaire, subjects admitted to general medicine services at three university hospitals were questioned about FI. Investigators also reviewed the subjects' charts to determine if presence or absence of FI was documented. One hundred ninety-nine patients were enrolled, and 46% (92/199) reported ongoing FI. Advanced age, looser stools, symptoms of fecal urgency, urinary incontinence, and history of forceps delivery were found t

The utility of endovaginal sonography in the evaluation of fecal incontinence.

OBJECTIVE: The endoanal sonography in female patients with faecal incontinence is sometimes difficult and can lead to diagnostic errors. The aim of this study is to evaluate the value of endovaginal sonography in such cases. MATERIAL AND METHOD: Thirty female patients complaining of faecal incontinence are included in the study. Anal endosonography was performed in all of them in a single ambulatory session, pictures were taken from all along the anal and results were analyzed afterward. Vaginal endosonography was then performed using the same equipment. Result from both techniques were compa

A case of idiopathic fecal incontinence. Evaluation and management.

Idiopathic fecal incontinence is a clinical entity described mostly in the colorectal surgical literature and seldom encountered by the physiatrist. Evidence exists suggesting neuropathic injury to the external anal sphincter. Several reports have linked this syndrome to the descending perineum syndrome. A final answer to this diagnostic puzzle is yet to be found. This article presents a case of probable "idiopathic" fecal incontinence presenting during an evaluation for low back pain. Full evaluation yielded a denervating lesion of unknown etiology to the pudendal nerve. The patient was manag

Results of treatment for rectal prolapse and fecal incontinence.

Fifty-six patients were treated for rectal prolapse or incontinence. Rectal prolapse was present in 32 patients and was associated with fecal incontinence in 24 (75 per cent). Incontinence without prolapse was present in 24 patients, 12 of whom were less than 40 years old. Rectopexy was used for treatment of rectal prolapse. Surgical treatment of fecal incontinence was by postanal repair; external sphincter reconstruction and surgery was advised only if control of diarrhea and electrical therapy had been of no benefit. Rectopexy was completely successful at controlling rectal prolapse in all c

Pudendal Neuropathy Alone Results in Urge Incontinence Rather Than in Complete Fecal Incontinence.

BACKGROUND: Conscious external anal sphincter contraction is mediated by the pudendal nerve. Pudendal neuropathy is, therefore, believed to result in fecal incontinence. Until urge sensation is experienced, fecal continence is maintained by unconscious external anal sphincter contraction, which is regulated by the anal-external sphincter continence reflex. The innervation of unconscious contraction is yet unknown. OBJECTIVE: We aimed to determine whether unconscious contraction is mediated by the pudendal nerve and whether age influences unconscious contraction. DESIGN: This was a retrospect

Prevalence and risk factors of fecal incontinence in women undergoing stress incontinence surgery.

OBJECTIVE: The objective of the study was to determine the prevalence and identify potential risk factors for monthly fecal incontinence (FI) in women presenting for stress urinary incontinence (UI) surgery. STUDY DESIGN: From 655 women enrolled in the Stress Incontinence Surgical Treatment Efficacy Trial, baseline data were analyzed. FI was defined as occurring at least monthly. Independent variables included sociodemographics, health status and history, physical examination, and validated UI questionnaires. Multivariable logistic regression models compared women with FI with women with only

Fecal incontinence, urinary incontinence, and priapism associated with multifocal distemper encephalomyelitis in a dog.

A 4-year-old castrated mixed-breed dog had atypical signs of chronic distemper encephalomyelitis. The predominant signs were related to visceral rather than somatic dysfunction. The visceral dysfunction was largely attributable to distemper-associated inflammatory lesions in the spinal cord and resulted in fecal incontinence, urinary incontinence, and priapism. Empirical treatment of the dog resulted in transitory improvement prior to euthanasia performed because of the progressive nature of the encephalomyelitis.

Fecal Incontinence Outcomes Following Transvaginal Posterior Vaginal Wall Repair.

INTRODUCTION AND HYPOTHESIS: Knowledge regarding rates of improvement of fecal incontinence (FI) after repair of posterior compartment prolapse is limited. We aimed to estimate the rate of resolution or improvement of fecal incontinence postoperatively following transvaginal posterior compartment repair. METHODS: This was a retrospective cohort study including patients with diagnosis of fecal incontinence who underwent transvaginal posterior repair at a single academic institution between 1/2016 and 1/2022. Patients who underwent concomitant anal sphincteroplasty served as controls. The prima

The association between vitamin-D deficiency and fecal incontinence.

BACKGROUND: Vitamin-D is essential for musculoskeletal health. We aimed to determine whether patients with fecal incontinence (FI): (1) are more likely to have vitamin-D deficiency and, (2) have higher rates of comorbid medical conditions. METHODS: We examined 18- to 90-year-old subjects who had 25-hydroxy vitamin-D levels, and no vitamin-D supplementation within 3 months of testing, in a large, single-institutional electronic health records dataset, between 2017 and 2022. Cox proportional hazards survival analysis was used to assess association of vitamin-D deficiency on FI. KEY RESULTS: Of

Sacral nerve stimulation for fecal incontinence: long-term outcomes.

BACKGROUND: Numerous studies advocate the short-term benefits of sacral nerve stimulation for fecal incontinence, but there has been a paucity of studies on longer-term outcomes. OBJECTIVE: The objective of this study was to report the long-term outcome of sacral nerve stimulation performed for fecal incontinence at a single institution. PATIENTS AND DESIGN: Between January 2004 and May 2007, 53 patients underwent definitive sacral nerve stimulation for fecal incontinence at our institution. Hirschsprung disease and fecal incontinence: diagnostic and management strategies.

PURPOSE: Ideally, fecal incontinence after operative management for Hirschsprung disease should not occur. If it does, it presents a formidable challenge. The purpose of this study was to describe the causes of fecal incontinence and present our algorithm for its treatment. METHODS: We reviewed 68 patients with Hirschsprung disease and fecal incontinence referred to us after surgery at other institutions. Patients were evaluated by contrast enema and by an examination under anesthesia to look specifically for the integrity of the anal canal. They were designated as having a dilated colon and

[Sacral nerve stimulation, an effective treatment for fecal incontinence].

Sacral nerve stimulation (SNS) became recently an essential therapeutic modality in the treatment of anal incontinence. Indications range from anorectal functional disorders to limited sphincteric lesions. An electrode is inserted through a sacral foramen, generally S3, to stimulate the corresponding nerve root. Patient's selection needs a meticulous multidisciplinary approach. Improved continence of more than 50% is frequently encountered and quality of life is significantly improved after implantation. SNS is a minimal-invasive technique, with very few risks of complications.

Fecal incontinence. Studies on physiology, pathophysiology and surgical treatment.

The thesis consists of ten previously published studies and a review. The physiological and pathophysiological mechanisms in fecal incontinence has been studied by anal manometry, both by standard static anal manometry and by a new method, dynamic anal manometry, where anal sphincter pressure can be measured during simultaneous opening and closing of the anal canal. Patients with fecal incontinence showed abnormal sphincter pressures more frequently when dynamic anal manometry was used compared to standard anal manometry. The physiology and pathophysiology of the rectum was studied using recta

Fecal incontinence in anorectal malformations, neuropathy, and miscellaneous conditions.

In the majority cases, fecal soiling in children is functional and usually associated with severe constipation. Fortunately, functional soiling is a self-limiting problem and usually disappears at puberty. Organic fecal incontinence is a consequence of congenital malformations affecting the anorectum, anal sphincters, or the spinal cord. Inability to control bowel function may be permanent, as in patients with myelodysplasia; self-limiting, as in patients who have fecal soiling after a pull-through operation for Hirschsprung's disease; or partial, as in many patients who have undergone repair
