County-Level Contributors to Geographic Variation in Medicare Fee-for-Service Stroke Hospitalization Rates: A Cross-Sectional Study.

BACKGROUND: Stroke hospitalization represents underlying stroke incidence and hospital utilization. We sought to identify factors associated with county-level hospitalization rates (HRs) and counties with HRs above or below expectation using publicly available data. METHODS: This cross-sectional study is based on the analysis of county-level 3-year average stroke HRs (principal International Classification of Diseases, Tenth Revision [ICD-10], codes I60.X-I69.X) among Medicare fee-for-service beneficiaries from 2018 to 2020. Linear mixed models were fitted to investigate 6 sets of factors ass

Health outcomes of dually eligible beneficiaries under different Medicare payment arrangements.

OBJECTIVES: Dually eligible beneficiaries (hereafter, Duals) qualify for Medicare and Medicaid due to low income and/or disability. Duals comprise 19% of Medicare beneficiaries but consume 35% of Medicare spending. Identifying high-quality, efficient care arrangements may improve outcomes and reduce costs for Duals. This study evaluated the effect of different payment arrangements on Duals' quality and efficiency outcomes. STUDY DESIGN: Retrospective, cross-sectional analysis using CMS data and health plan contract information from 17 participating physician groups (n = 15,488 primary care ph

Bleeding-Related Hospitalizations Among Direct Oral Anticoagulant Users and Nonusers in Medicare Fee-For-Service: Variation in Prevalence, Burden, and Characteristics.

INTRODUCTION: Bleeding-related hospitalizations represent a major burden for patients and the US healthcare system. Anticoagulant therapies pose a greater risk for bleeding, especially among the older Medicare population. Direct oral anticoagulants (DOACs) have become the most common type of oral anticoagulant used in Medicare due to their clinical advantages. This descriptive study examines the burden of bleeding-related hospitalizations among DOAC users and nonusers (with no evidence of anticoagulant or antiplatelet use) in Medicare fee-for-service (FFS). METHODS: This was an observational

Annual Wellness Visits and Early Dementia Diagnosis Among Medicare Beneficiaries.

IMPORTANCE: Early recognition of cognitive impairment is key to optimal dementia care. No previous research has examined the probability of developing mild cognitive impairment (MCI) or Alzheimer disease and related dementias (ADRD) at 5-year follow-up among older adult Medicare beneficiaries by receipt of an annual wellness visit (AWV). OBJECTIVE: To assess the association of incident AWV with the first ADRD or MCI diagnosis among older adults with Medicare fee-for-service benefits. DESIGN, SETTING, AND PARTICIPANTS: This retrospective population-based cohort study used 100% Texas fee-for-s

The costs of dementia subtypes to California Medicare fee-for-service, 2015.

INTRODUCTION: Dementia is among the costliest of medical conditions, but it is not known how these costs vary by dementia subtype. METHODS: The effect of dementia diagnosis subtype on direct health care costs and utilization was estimated using 2015 California Medicare fee-for-service data. Potential drivers of increased costs in Lewy body dementia (LBD), in comparison to Alzheimer's disease, were tested. RESULTS: 3,001,987 Medicare beneficiaries were identified, of which 8.2% had a dementia diagnosis. Unspecified dementia was the most common diagnostic category (59.6%), followed by Alzheime

Impact of Fee For Service on the Efficiency and Survival of Seguro Popular's Patients With Acute Lymphoblastic Leukemia in Mexico.

PURPOSE: Cost containment and efficiency in the provision of health care are primary concerns for health systems that aim to provide affordable, high-quality care. Between 2005 and 2015, Seguro Poplar's Fund against Catastrophic Expenditures (FPGC) funded ALL treatment in Mexico. Before January 1, 2011, FPGC reimbursed a fixed amount per patient according to risk. In 2011, the per capita reimbursement method changed to fee for service. We used this natural experiment to estimate the impact of the reimbursement policy change on average expenditure and quality of care for ALL treatment in Mexico

Physician payment models and preventive cancer screening: a population-based retrospective cohort analysis from Ontario, Canada.

BACKGROUND: Ontario's primary care reforms have introduced three blended physician payment models: (i) blended fee-for-service (BFFS), (ii) blended capitation without interprofessional teams, and (iii) blended capitation with teams. Each model includes the same pay-for-performance incentives, yet their impact on cancer screening, including that during the COVID-19 pandemic, remains unclear. METHODS: We used linked administrative data (2018-23) to examine the associations between these models and colorectal, cervical, and breast cancer screening rates. Fractional probit regression models, adju

Home Health Care After Skilled Nursing Facility Discharge Following Heart Failure Hospitalization.

BACKGROUND/OBJECTIVE: Heart failure (HF) readmission rates have plateaued despite scrutiny of hospital discharge practices. Many HF patients are discharged to skilled nursing facility (SNF) after hospitalization before returning home. Home healthcare (HHC) services received during the additional transition from SNF to home may affect readmission risk. Here, we examined whether receipt of HHC affects readmission risk during the transition from SNF to home following HF hospitalization. DESIGN: Retrospective cohort study. SETTING: Fee-for-service Medicare data, 2012 to 2015. PARTICIPANTS: Bene

Diagnosis of Alzheimer's disease and related dementias and outmigration of older Puerto Rican Medicare Fee-for-Service beneficiaries, 2012-2019.

OBJECTIVES: There is limited research on whether receiving a dementia diagnosis could prompt later-life migration, particularly among older Medicare beneficiaries in Puerto Rico. Prior research shows older Puerto Ricans are migrating to the mainland United States, which may include those with an Alzheimer's Disease and Related Dementias (ADRD) diagnosis. This study examined yearly outmigration trends among older Puerto Ricans by exploring their movement patterns in 2011-2019. METHODS: We used data from the Medicare Master Beneficiary Summary File and the Puerto Rican Community Survey and focu

Use of Claims Data to Screen for Functional Limitations Among Medicare Beneficiaries.

IMPORTANCE: The only functional impairment data available for all Medicare beneficiaries identify those who qualified for Medicare before age 65 years due to work-related disability. This administrative measure omits impairments arising after age 64 years or affecting nonwork-related activities. Surveys may offer more accurate assessments, but survey data on functional limitations are not routinely available. Claims data may provide a practical, scalable alternative. OBJECTIVE: To describe the development of a survey-based index of functional limitations and a new claims-based model for predi

Tobacco treatment billing and tobacco use disorder diagnosis in healthcare settings in the United States: an analysis of South Carolina medicaid claims.

BACKGROUND: Tobacco use, particularly smoking, remains the leading cause of preventable death in the United States. While Medicaid in all states provides some coverage of tobacco cessation treatments, rates of tobacco use remain much higher among Medicaid beneficiaries (30.0%) compared to those with private insurance (18.0%). The extent to which Medicaid beneficiaries receive cessation counseling services remains unclear. The current study assessed tobacco counseling occurrence among individuals who use tobacco among South Carolina Medicaid fee-for-service beneficiaries from 2019 to 2022. MET

[Young general practitioners in an area with oversupply of doctors: the case of Brussels].

Twelve GP's have been interviewed in Brussels, the capital of Belgium. Brussels is known for its extremely high doctor-population ratio. In Belgium, most doctors have a private practice with fee-for-service payment. The interviews took place in early 1998, the GP's involved were graduated between 1985 and 1994. Most GP's were not satisfied with their low income. They adopted a number of strategies to cope with the challenge: increase of activities including numerous home visits, acceptance of low qualified work, practice of "parallel medicine", decrease of referral to specialists ... and so on
