Altered shape of the proximal femur (cam morphology) or acetabulum (pincer morphology) is indicative of femoroacetabular impingement, which can result in hip pain and osteoarthritis of the hip. As mechanical load during growth affects the resulting bone shape, there is strong evidence in males that cam morphology develops during skeletal growth while physes are open, rather than as an adaptation after growth plates are closed (skeletal maturity). This adaptation is particularly evident in athletes who participate at elite levels prior to skeletal maturity. The research providing this evidence,

A case report of a Marine Recruit with bilateral atypical tensile-sided femoral neck stress fracture. The patient was treated nonoperatively with protected weightbearing following an established protocol that included close observation with serial exams and advanced imaging. He recovered uneventfully and was able to return to running. He was eventually diagnosed with bilateral femoral acetabular impingement syndrome at the end of his treatment course demonstrating a growing association warranting further investigation.

BACKGROUND: After treatment with in situ stabilization, slipped capital femoral epiphysis (SCFE) patients have variable degrees of deformity that can contribute to femoroacetabular impingement (FAI). To evaluate the severity of residual deformity most physicians currently use biplanar radiographs of an anteroposterior pelvis which profiles the lateral head-neck deformity and a frog lateral view which profiles the anterior head-neck deformity. However, the assessment of FAI morphology commonly relies on the 45-degree Dunn view to profile the anterolateral head-neck junction where FAI deformity

Computed tomography (CT) is most commonly used to produce three-dimensional (3D) models for evaluating bone and joint morphology in clinical practice. However, 3D models created from magnetic resonance imaging (MRI) data can be equally effective for comprehensive and accurate assessment of osseous and soft tissue structure morphology and pathology. The quality of 3D MRI models has steadily increased over time, with growing potential to replace 3D CT models in various musculoskeletal (MSK) applications. In practice, a single MRI examination for two-dimensional and 3D assessments can increase th

Magnetic resonance imaging (MRI) is a common choice among various imaging modalities for the evaluation of hip conditions. Conventional MRI with two-dimensional acquisitions requires a significant amount of time and is limited by partial-volume artifacts and suboptimal fluid-to-cartilage contrast. Recent hardware and software advances have resulted in development of novel isotropic three-dimensional (3D) single-acquisition protocols that cover the volume of the entire hip and can be reconstructed in arbitrary planes for submillimeter assessment of bony and labro-cartilaginous structures in the

BACKGROUND: Femoroacetabular impingement (FAI) and acetabular dysplasia lead to acetabular cartilage damage that commonly results in the chondral flaps seen during hip arthroscopy.
PURPOSE: To compare the acetabular chondral flap morphology seen during hip arthroscopy ("outside-in" vs "inside-out") with clinical and radiographic parameters underlying FAI and hip dysplasia.
STUDY DESIGN: Cohort study; Level of evidence, 3.
METHODS: Patients who underwent hip arthroscopy by the senior author between 2013 and 2017 with a finding of Outerbridge grade IV acetabular chondral flap were included. E

Although generally more common in adults, lower extremity impingement and friction syndromes are also observed in the pediatric age group. Encompassing femoroacetabular impingement, iliopsoas impingement, subspine impingement, and ischiofemoral impingement around the hip; patellar tendon-lateral femoral condyle friction syndrome; iliotibial band friction syndrome; and medial synovial plica syndrome in the knee as well as talocalcaneal impingement on the hindfoot, these syndromes frequently cause pain and may mimic other, and occasionally more ominous, conditions in children. OBJECTIVE: This article reviews the surgical treatment options for femoroacetabular impingement (FAI), including labral repair and osteochondroplasty, and the expected postoperative appearance on MRI. Complications, including residual osseous deformities, chondral injury, adhesions, femoral neck stress fractures, osteonecrosis, instability, malpositioned suture anchors, and infection, will also be discussed.
CONCLUSION: Knowledge of the surgical treatment of FAI can assist in improving our understanding of the expected postoperative MRI appearance and in evaluating surgical complications.

PURPOSE: Primary paraclinical investigation to look for femoral morphological abnormality consistent with cam-type anterior femoroacetabular impingement (FAI) must include specific radiographic projections of the hip from the following: cross-table, frog-leg or Dunn (90° and 45° flexion). We use a frog-leg type view with fixed angles as described by Chiron, obtained with the patient in a supine position, with the hip in 45° flexion-abduction and 30° external rotation. The X-ray beam is anteroposterior and centred on the femoral head. We evaluated this view by comparing it with other views in c

Acetabular dysplasia, a nonarthritic hip joint pain condition in active young and middle-aged adults, is distinct from femoroacetabular impingement syndrome. Despite increasing clinical recognition of acetabular dysplasia over the past decade, it remains a challenging orthopaedic condition for athletic trainers to manage due to the complexities of diagnosis, concurrent psychological implications, and overlap with other hip and spine pathologies. Acetabular dysplasia is more prevalent in females and is diagnosed primarily through clinical presentation and radiographic evidence. Altered femoral

The three primary factors involved in preservation of the hip joint include femoroacetabular impingement (FAI), hip dysplasia/instability, and femoral torsion abnormalities. Each of these factors affects the health of the acetabular labrum and femoroacetabular cartilage. The appropriate surgical treatments for each of these factors include arthroscopic or open femoroplasty/acetabuloplasty for FAI, periacetabular osteotomy for hip dysplasia/instability, and derotational femoral osteotomy for femoral torsion abnormalities. When evaluating patients with prearthritic hip conditions, orthopaedic su

PURPOSE OF THE STUDY Femoroacetabular impingement syndrome is a complex, often post-traumatically developing impairment of the hip joint, characterized by ambiguous symptomatology, which makes early diagnosis diffi cult, especially in the early stages. Experimental retrospective study was carried out to evaluate the usability of a triaxial gyroscopic sensor in routine practice as an additional indication criterion for operative versus conservative treatment procedures. MATERIAL AND METHODS 92 patients were included in the retrospective study, and 62 completed the investigation. All patients si

Endoscopic surgery can be used to address peritrochanteric pathology in patients with greater trochanteric pain syndrome. During management of these patients, surgeons must decide whether adjunctive hip arthroscopy to treat concomitant intra-articular pathology (such as labral tears or chondral lesions) is required, because the prevalence of intra-articular findings may be greater than the clinical significance. A thorough history and physical examination can help distinguish whether the symptoms are arising from a peritrochanteric issue (e.g., gluteal tendinopathy, trochanteric bursitis, exte

The treatment of adult borderline hip dysplasia remains challenging and continues to be a subject of controversy. The preferred treatment for hip instability and acetabular undercoverage is periacetabular osteotomy. However, patients with painful hips and associated femoroacetebular impingement, microinstability, or no instability may benefit from arthroscopic surgery. Short-term studies have reported favorable clinical outcomes. Traditionally, the lateral center-edge angle was used to determine hip dysplasia. More recently the femoro-epiphyseal acetabular roof (FEAR) index was introduced as a

BACKGROUND: Femoroacetabular impingement (FAI) is a leading cause of hip pain in young adults and often leads to degenerative osteoarthritis (OA). A small animal model of hip deformities is crucial for unraveling the pathophysiology of hip OA secondary to FAI.
PURPOSES: To (1) characterize a new minimally invasive surgical technique to create a proximal femoral head-neck deformity in a skeletally immature rabbit model and (2) document the effect of an injury to the medial proximal femoral epiphysis on head-neck morphology at 28 days after the injury.
STUDY DESIGN: Controlled laboratory study

BACKGROUND: Properly performed high-quality imaging is critical in the diagnosis of femoroacetabular impingement syndrome (FAIS). Currently, conventional imaging in the form of an anteroposterior view of the pelvis and at least a second view is still the first step in the diagnosis of FAIS. Here, by determining the various parameters, the acetabular configuration should also be accurately assessed with regard to a combination with dysplasia or acetabular retroversion. MRI: MRI should also be demanded as standard before joint-preserving surgery. It allows for more precise detection of morpholog
