Percutaneous aortic valve implantation retrograde from the femoral artery.

BACKGROUND: Percutaneous aortic valve implantation by an antegrade transvenous approach has been described but is problematic. Retrograde prosthetic aortic valve implantation via the femoral artery has potential advantages. Percutaneous prosthetic aortic valve implantation via the femoral arterial approach is described and the initial experience reported. METHODS AND RESULTS: The valve prosthesis is constructed from a stainless steel stent with an attached trileaflet equine pericardial valve and a fabric cuff. After routine aortic balloon valvuloplasty, a 22F or 24F sheath is advanced from th

[Ultrasound microbubbles promote thrombolysis of rabbit femoral artery thrombus].

OBJECTIVE: To evaluate the efficacy of intravenous ultrasound microbubbles for thrombolysis of arterial thrombus without using thrombolytic drugs. METHODS: Twelve rabbit models of acute bilateral femoral artery thrombosis were established and 6 of them received transcutaneous ultrasound and intravenous albumin microbubble treatment for thrombosis on one side while only microbubble treatment for the other side. The other 6 rabbits received ultrasound treatment on one side but no treatment on the other to serve as the control group. RESULTS: None of the 6 arteries treated with microbubbles alo

[Revascularization of supra-aortic trunks from the femoral artery].

A seventy-year-old woman had an occlusion of the left sub-clavian and left primitive, internal and external carotid arteries, combined with a pre-thrombotic stenosis of the brachiocephalic trunk. Lesions were so diffused that a cross extra-anatomic bypass was impossible at the cervical level. There was contraindication for the implantation of a prosthesis on the aortic arch, due to a much extended calcified atheroma which did not allow clamping. Supra-aortic trunk revascularization was done through right femoral-axillary bypass without any post-operative complication. After a four year follow-

Patient and limb outcomes 10 years after endovascular revascularization of the superficial femoral artery for peripheral artery disease: The Boston Femoral Artery Endovascular Revascularization Outcomes (Boston FAROUT) study.

BACKGROUND: We aimed to describe patient and limb outcomes in the decade after endovascular revascularization for chronic lower-extremity peripheral artery disease (PAD). METHODS: We assessed outcomes in patients having endovascular revascularization of the superficial femoral artery in two centers between 2003 and 2011 and followed for a median 9.3 (25-75%: 6.8, 11.1) years. Outcomes included death, myocardial infarction, stroke, repeat limb revascularizations, and amputations. We used competing risks analysis with clustering by patient to determine the hazard ratios (HR) and 95% CI for pati

Ultrasound-Guided Retrograde Infrapopliteal Artery Access for Recanalization of Complex Femoral-Popliteal Artery Occlusions.

BACKGROUND: The aim of the study was to review the outcomes of femoral-popliteal artery (FPA) interventions using an ultrasound (US)-guided retrograde infrapopliteal artery access after the failure of an antegrade recanalization. METHODS: From Jan 2016 to Jan 2019, 37 patients with chronic total occlusion (CTO) of the FPA underwent ultrasound (US)-guided retrograde infrapopliteal artery access after failure of an antegrade procedure. Treated limbs were classified as Rutherford class 5 or 6 (29.7%) and class 4 (62.2%). Data collected included success rate and time to access using US. Immediate

Case report of superficial femoral artery and popliteal artery aneurysm repair using brachial vein.

BACKGROUND: Several veins have been well-recognized as acceptable conduits for infrainguinal bypass surgery when the ipsilateral greater saphenous vein is unavailable. However, there is a paucity of literature describing the brachial vein as an adequate alternative. In the absence of other viable autogenous conduits, we describe the use of a brachial vein as a successful alternative for lower extremity revascularization. METHODS: A 70-year-old man presented with a chief complaint of right calf pain. Duplex ultrasound imaging of his right lower extremity revealed right-sided 2.5 cm acutely thr

Large eddy simulation of a stenosed artery using a femoral artery pulsatile flow profile.

Computational fluid dynamics simulation of stenosed arteries allows the analysis of quantities including wall shear stress, velocity, and pressure; detailed in vivo measurement is difficult yet the analysis of the fluid dynamics related to stenosis is important in understanding the likely causes and ongoing effects on the integrity of the vessel. In this study, a three-dimensional Large Eddy Simulation is conducted of a 50% occluded vessel, with a typical femoral artery profile used as the transient inlet conditions. The fluid is assumed to be homogenous, Newtonian and incompressible and the w

Femoral artery plaque characteristics, lower extremity collaterals, and mobility loss in peripheral artery disease.

Little is known about the prognostic significance of specific characteristics of magnetic resonance imaging (MRI) measured plaque in the superficial femoral artery (SFA). Associations of MRI-measured plaque quantity, lumen area, and plaque composition in the SFA with subsequent mobility loss were studied in people with lower extremity peripheral artery disease (PAD). Participants with an ankle-brachial index (ABI) < 1.00 were identified from Chicago medical centers and underwent direct visualization of atherosclerotic plaque in the SFA using MRI. Participants were followed annually for up to 4

Lower limb ischemia after migration of a coronary artery stent into the femoral artery.

A coronary stent may be lost in the peripheral or visceral arterial system with an incidence ranging from 0.9 to 8.4%, however, a limb or organ ischemia after stent migration is very uncommon. We report the case of an 83-year-old man who underwent coronary artery stenting at our Hospital's Cardiological Department. During this difficult procedure, due to the critical stenosis of the circumflex artery, the stent was accidentally lost and found at the level of the insertion of the right common femoral on the external iliac artery. After several attempts to rescue the stent through an omolateral

Limb Salvage Through Endovascular Repair of an Iatrogenic Superficial Femoral Artery Pseudoaneurysm Following Femoral Fracture Fixation.

Vascular injury leading to pseudoaneurysm formation is a rare but potentially limb-threatening complication of lower limb trauma and postoperative recovery. We report a case of a 58-year-old man who developed progressive swelling of the right thigh one week after sustaining a road traffic accident and undergoing intramedullary nailing for a distal femoral shaft fracture. The swelling, initially presumed to represent a postoperative hematoma, progressed to cause severe tension and pain with early signs of threatened compartment syndrome. Upon transfer to our center, urgent surgical exploration

A left femoral artery to right femoral vein bypass graft for hemodialysis access: A case report.

RATIONALE: As survival prospects improve for long-term patients with hemodialysis, it is common for patients to exhaust all upper extremity access options before other avenues need exploration. The purpose of this case report was to describe our experience in creating a prosthetic graft between left femoral artery and right femoral vein in a patient with history of central venous occlusion and bilateral femoral neck fracture. PATIENT CONCERNS: A female patient with hemodialysis exhausted all upper extremity access options along with bilateral femoral neck fracture. DIAGNOSES: Patients with e

Risk of Mortality Related to Recurrent Limb Events After Endovascular Revascularization of the Superficial Femoral Artery for Peripheral Artery Disease: The Boston Femoral Artery Endovascular Revascularization Outcomes (Boston FAROUT) Study.

Endovascular revascularization of the superficial femoral artery (SFA) may lead to recurrent ischemic syndromes, revascularization, or amputation. The impact of these events on mortality is unknown. We followed all patients having SFA endovascular revascularization for claudication or chronic limb-threatening ischemia in 2 cardiovascular (CV) divisions in Boston, Massachusetts. Any recurrent limb event after the initial SFA revascularization included recurrent claudication (67%), limb ulceration or gangrene (13%), repeat endovascular revascularization (61%), surgical revascularization (15%), o

Duplication of superficial femoral artery: an uncommon variation of the lower limb arterial system.

The most common anatomical variation of the superficial femoral artery (SFA) is hypoplasia or aplasia, associated with persistent sciatic artery. Duplication of SFA remains uncommon. We report a case of SFA duplication discovered on an angiogram performed for critical limb ischemia, in an 80-year-old man.

Arterial injuries from femoral artery cannulation with port access cardiac surgery.

Although minimally invasive (MI) cardiac surgery reduces blood loss, hospital stay, and recovery time, some MI approaches require femoral arterial cannulation, which introduces a heretofore unknown risk of femoral arterial injury. This study was performed to examine the risk of femoral arterial injury after Port Access MI cardiac surgery (PA-MICS) with femoral cannulation. Data were prospectively obtained on 739 consecutive patients who had PA-MICS with femoral cannulation between June 1996 and April 2000, identifying any patient with new (<30 days postoperative) arterial insufficiency from th
