Statistical sensitivity analysis for the estimation of fetal alcohol spectrum disorders prevalence.

Fetal alcohol spectrum disorders (FASD) are thought to be the leading cause of developmental disabilities worldwide. Accurate estimates of the prevalence of FASD have been lacking. An improved estimate of FASD prevalence in the U.S. was recently reported in a study where multistage methods of active case ascertainment of first-grade children with FASD were utilized in four regions in the U.S. Each method relied on parental consent and therefore had potential non-response bias. We consider weighted approaches, where the weights were formed using the distribution of observed variables in the pop

New approaches for drug and kinetic analysis in the maternal-fetal unit.

The development of more sensitive and selective analytical techniques has greatly improved scientists' ability to study the PK, PD, and metabolism of drugs in pregnancy. This chapter has chronicled the evolution of the study of drug disposition in pregnant sheep in the laboratory. With the advent of more sensitive and selective analytical techniques, researchers have been able to progress significantly beyond measuring the extent of fetal drug exposure by a single-point determination. A number of interesting and significant advances have been made using highly selective and specific analytical

The spectrum of fetal abuse in pregnant women.

Physical assault by a woman or her partner upon their unborn child has received minimal attention in the psychiatric or obstetric literature. The spectrum concept facilitates the application of knowledge about one type of fetal abuse (e.g., physical assault in response to fetal movements) to other types (e.g., neglect or failure to protect the fetus from chemical assault by alcohol, nicotine, or drugs). Three illustrative case histories are presented, including two cases of assault by the mother via the anterior abdominal wall. Fetal abuse may be one antecedent of child abuse, and this paper a

A report on the Fetal Alcohol Spectrum Disorders Study Group meeting of 2012, theme title, "Biomarkers for FASD".

The 2012 meeting of the Fetal Alcohol Spectrum Disorders Study Group (FASDSG) focused on the development and ethics of biomarkers for fetal alcohol exposure. This one-day international conference brought students and trainees together with clinicians and researchers to discuss the latest research on FASD. One keynote speaker discussed the value of profiling epigenetic modifications in readily available fetal tissues to diagnose fetal exposure to environmental agents, while the second speaker discussed the ethics of biomarker development within the context of core principles of justice, autonom

Fetal wound healing.

Wound healing research has produced some startling discoveries during the past decade. Foremost among these is the observation that cutaneous wounds created and healed in utero are histologically indistinguishable from intact, unwounded tissue. Observers have documented that the acute inflammatory response and endogenous immunoglobulins that characterize healing in human beings after birth are absent in the fetal wound. Determination of the cellular and biochemical differences between fetal and postdelivery wound healing offers the promise of improved control over the process of tissue repair.

The three-dimensional ultrasonography of the fetal face--history and progress.

Fetal face was the main concern of researchers who developed three-dimensional ultrasound. 3-D ultrasound beginnings date back to 1973. Parallel concerns of researchers from different parts of the world have led to a constantly changing the equipment and software. 3-D images of the fetal face were obtained since 1986, but those that are available on the ultrasound devices sold today dates from 1992, and the 4-D since 1996. Techniques such as volume rendering and multiplanar volume rendering, led to the development of 3-D ultrasound. Late 2000 revealed study fetal face by multiplane three-dimen

A kick from within--fetal movement counting and the cancelled progress in antenatal care.

Interest for maternal fetal movement counting as a method of screening for fetal well-being boomed during the 1970's and 1980's. Several reports demonstrated that the introduction of counting charts significantly reduced stillbirth rates. However, in 1989, a large study appeared in The Lancet that annihilated research in this field by deeming charts ineffective. In retrospect, it seems evidence was lacking. This review revisits the subject of the significance of fetal movement counting in predicting outcome and reducing stillbirth rates. A structured search was performed to identify studies re

Behavioral perinatology: biobehavioral processes in human fetal development.

Behavioral perinatology is as an interdisciplinary area of research that involves conceptualization of theoretical models and conduct of empirical studies of the dynamic time-, place-, and context-dependent interplay between biological and behavioral processes in fetal, neonatal, and infant life using an epigenetic framework of development. The biobehavioral processes of particular interest to our research group relate to the effects of maternal pre- and perinatal stress and maternal-placental-fetal stress physiology. We propose that behavioral perinatology research may have important implicat

Studies on fetal motor behaviour in normal and complicated pregnancies.

The possibility of studying fetal motor behaviour by ultrasound techniques has provoked research on its potential application for assessment of prenatal neurological conditions. The characteristics ('quality') of one particular movement pattern, the 'general movement', has been shown to be discriminative between uncomplicated pregnancy and major pathology of the developing central nervous system. Some recent studies have investigated whether the quality and/or the quantity of fetal movements correlated with other clinical variables during complicated pregnancies, and whether they provided prog

Cordocentesis.

Access to the fetal circulation via cordocentesis has resulted in many advances in both prenatal diagnosis and fetal therapy over the past decade. In this review, current research in the assessment and treatment of erythrocyte isoimmunization and fetal infection by fetal blood sampling, as well as the role of cordocentesis in the evaluation of fetal platelet abnormalities, intrauterine growth retardation, fetal thyroid function, and genetic investigation, are discussed. Finally, a brief review of complications and loss rates relating to this procedure is given.

Lived experience of fetal alcohol spectrum disorder: A qualitative scoping review.

AIM: To explore the lived experiences of individuals with fetal alcohol spectrum disorder (FASD), their caregivers, and professionals working with individuals with FASD. METHOD: We conducted a scoping review using qualitative methods to explore what it is like to live with FASD from the perspective of those living with FASD, their caregivers, and stakeholders experienced in working with individuals with FASD. We searched electronic databases and grey literature for research published between 2005 and 2022. RESULTS: The 47 studies included in this scoping review show that FASD affects people'

Religious barriers to measles vaccination.

In 2014, the United States has experienced an increase in measles activity, the most since the elimination of the virus in 2000. The measles infection occurs in unvaccinated individuals. Communities and individuals choose to not vaccinate for a number of reasons, primarily citing religious and philosophical motives. Objections based upon religion most often center on the use of aborted human fetus tissue used in the rubella component of the combined vaccine products, and animal derived gelatins used in vaccine production. Objections among religious communities may also not be faith based, rath

What's wrong with using the F word? A systematic integrative review of how the fetus is talked about in situations of fetal demise or high risk of fetal loss.

BACKGROUND: Limited research exists that investigates the language parents and health professionals use when faced with a high-risk likelihood of fetal demise or an instance of fetal loss. This review examines the language used when referring to the 'fetus' in these cases to better understand the meaning different groups might ascribe to the fetus and how their word choice may reflect their strategies for managing in these situations. METHODS: An integrative review of primary, peer-reviewed research was conducted. A systematic search of seven databases was undertaken, articles critiqued and s

Fetal myelomeningocele repair: a narrative review of the history, current controversies and future directions.

Fetal surgery is a relatively new field of medicine. The purpose of this narrative review is to present the history of how fetal surgery became the standard of care for myelomeningocele (MMC), the current controversies of this treatment, and active areas of research that may change how MMC is treated. Fetal surgery for MMC emerged out of the University of California, San Francisco in the 1980s in the laboratory of Dr. Michael Harrison. Initial research focused on testing the hypothesis that the in utero repair of MMC could improve outcomes in the ovine model. Evidence from this model suggested
