Self-inflicted finger cold injury leading to amputation: Report of a case.

A cold injury can result in devastating outcomes, leading to significant morbidity and loss of distal extremities. Amputations are common after severe frostbite injuries with delayed presentation, often mediated by post-injury arterial thrombosis. Ischemic injuries are managed according to the ischemia time. The most controversial aspect of treating a salvage injury is the time of surgical intervention, which used to be based on the previous management dogma freeze in January, amputate in July. Recently, the paradigm has shifted to early surgical management if the level of viability of the dee

Conservative interventions for treating hyperextension injuries of the proximal interphalangeal joints of the fingers.

BACKGROUND: Immobilisation and early motion (protected or unrestricted) are both used following hyperextension injuries to the proximal interphalangeal (PIP) joint of the finger. OBJECTIVES: To assess the effects of conservative interventions (non-surgical management) for treating hyperextension injuries of the proximal interphalangeal joints of the fingers. SEARCH METHODS: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (January 2012), the Cochrane Central Register of Controlled Trials (in The Cochrane Library 2012, Issue 1), MEDLINE (1946 to January Week 2

Computerized measures of finger tapping: reliability, malingering and traumatic brain injury.

We analyzed computerized finger tapping metrics in four experiments. Experiment 1 showed tapping-rate differences associated with hand dominance, digits, sex, and fatigue that replicated those seen in a previous, large-scale community sample. Experiment 2 revealed test-retest correlations (r = .91) that exceeded those reported in previous tapping studies. Experiment 3 investigated subjects simulating symptoms of traumatic brain injury (TBI); 62% of malingering subjects produced abnormally slow tapping rates. A tapping-rate malingering index, based on rate-independent tapping patterns, provided

Impact of single versus dual arterial supply on perfusion and function in finger replantation after complex hand injuries.

Finger amputations following complex hand injuries (CHI) pose a significant challenge in hand surgery due to severe tissue trauma and neurovascular damage, necessitating precise arterial repair. While restoring arterial perfusion is critical, it remains unclear whether reconstructing both proper palmar digital arteries is required for optimal outcomes. This study evaluates whether restoring one or both arteries in finger replantation after complex injuries impacts perfusion and overall outcomes. In this retrospective, cross-sectional, follow-up study, we investigated patients with finger amput

Reliability, validity and responsiveness of composite finger flexion in patients with traumatic hand injuries: A clinical measurement study.

BACKGROUND: Composite finger flexion (CFF) is proposed to be a convenient alternative to total active motion (TAM) and total passive motion (TPM). Passive CFF (PCFF) may be useful for early monitoring in post-operative rehabilitation of traumatic hand injuries. PURPOSE: To determine whether active and passive CFF are reliable, valid, and responsive measures of hand motion and of higher utility to the tester. STUDY DESIGN: Cross-sectional observational clinical measurement study. METHODS: Fifty hand injury patients were recruited from a hospital-based out-patient clinic. TAM, TPM, repeated m

Contribution of lumbrical muscle activity to the paradoxical extension phenomenon induced by injuries to the finger flexor tendons.

The "Extensor habitus" phenomenon occurs in finger flexor tendon injuries and consists of a paradoxical extension of the interphalangeal joints after an attempt to flex the finger. The mechanism of extension is considered to be a contraction of the flexor digitorum profundus that is then transmitted via the lumbrical muscle structure to the extensor expansion. Using electromyography, we recorded the lumbrical muscle activity during the paradoxical extension phenomenon to determine whether the lumbrical muscle contributed to this event. Two patterns of electromyographical activity of the lumbri

Radial sensory neurotization of the thumb and index finger for prehension after proximal median and ulnar nerve injuries.

Proximal median nerve injuries are functionally disabling, secondary to both motor and sensory deficits. Reestablishment of sensation relies on slow axonal regeneration originating from the site of injury after either primary nerve repair or the use of autogenous nerve grafts. This regeneration can take 2 or more years to restore sensation to the hand, depending on injury location. Distal sensory nerve transfers shorten the recovery time by decreasing the required regeneration distance. The authors present two case reports of patients with proximal median nerve injury, who underwent radial sen

Awareness of Repetitive Strain Injury of the Fifth Finger and Its Association With Fifth Finger Pain Among Young Adults Due to Overuse of Smartphones.

Introduction The widespread use of smartphones has substantially increased among young adults because of academic, occupational, and social demands. Prolonged smartphone use, particularly during texting, scrolling, gaming, and other repetitive hand activities, has been associated with musculoskeletal symptoms affecting the hand and fingers. The fifth finger plays a significant role in supporting handheld devices during one-handed smartphone use and may be subjected to sustained mechanical loading, potentially contributing to pain, discomfort, muscle fatigue, and repetitive strain symptoms. Des

[Clinical observation on combined repair of avulsion injury of the distal phalanx of the finger with dorsalis branch of digital artery flap and cross-finger flap].

OBJECTIVE: To investigate the clinical efficacy of combined dorsalis branch of digital artery flap and cross-finger flap in the repair of avulsion injury of the distal phalanx of the finger. METHODS: Between March 2019 and June 2023, 13 patients with degloving injury of the distal phalanx of the finger or amputated finger without replantation conditions were treated with combined homodigital dorsalis branch of digital artery flap and cross-finger flap, including 11 males and 2 females, aged from 21 to 55 years old. Among them, 6 cases were complicated with distal phalanx fracture. The injury

Flow-Through Procedure in Sequela After Complex Injuries of the Hand With Fingers' Amputation.

PURPOSE: Complex injuries of the hand with finger amputation can result in important functional impairment. If the amputated fingers cannot be replanted, the developed sequelae should be further evaluated. Sometimes, in the presence of a few remaining local vascular resources, the use of the flow-through technique can help in solving the problem. METHODS: This study retrospectively reviewed four cases that underwent single or multiple fingers' reconstruction with toe transfers by using true flow-through or flow-through conduit flaps. A thorough description of the two methods is done, and the

Comparison of four different immobilization methods in the treatment of tendinous mallet finger injury.

BACKGROUND: Although there is consensus that closed tendinous mallet finger injuries should be treated conservatively, the best method of immobilization to be used is not clear and the existing data in the literature are not conclusive. The aim of this study is to compare the results of four different immobilization methods used in the conservative treatment of tendinous mallet finger injury. METHODS: Ninety-six patients with tendinous mallet finger injury were treated with four different immobilization methods (stack orthosis, thermoplastic orthosis, aluminum orthosis, and Kirschner wire [K-

Crush Injury-induced Finger Compartment Syndrome: A Case Report and Literature Review.

Isolated finger compartment syndrome is an uncommon condition and is not well-documented. It is usually associated with pain, decreased sensation, and intra-compartmental swelling. We present the case of a finger fracture after a crush injury that developed compartment syndrome, which responded well after surgical fixation and midline skin incision for digital decompression. A 20-year-old male with a history of a 200 lb crush-injury to the left index finger 24 hours prior presented to the emergency department with decreased sensation and range of motion, deformity, increasing pallor, and sever

Practical Management of Common Skin Injuries, Lacerations, Wounds, Trigger Fingers, and Burns.

Primary care clinicians encounter many conditions during their day-to-day visits with patients. A few of these common presentations include burns, lacerations, trauma to the hand, and wounds, some of which do not require an evaluation by a specialist and can be managed outpatient by primary care clinicians. In this article, we share evidence-based tips to avoid common pitfalls in primary care recognition and management of such presentations as well as guide them to manage many of these conditions themselves. We also provide guidance in the decision to refer the patient to a plastic surgeon or

Hand Posturing Is a Nonverbal Indicator of Catastrophic Thinking for Finger, Hand, or Wrist Injury.

BACKGROUND: Prior research documents that greater psychologic distress (anxiety/depression) and less effective coping strategies (catastrophic thinking, kinesophobia) are associated with greater pain intensity and greater limitations. Recognition and acknowledgment of verbal and nonverbal indicators of psychologic factors might raise opportunities for improved psychologic health. There is evidence that specific patient words and phrases indicate greater catastrophic thinking. This study tested proposed nonverbal indicators (such as flexion of the wrist during attempted finger flexion or extens

Sports Injury-Related Fingers and Thumb Deformity Due to Tendon or Ligament Rupture.

BACKGROUND: Hand injuries are very common in sports, such as skiing and ball sports. One of the major reasons causing hand and finger deformity is due to ligament and tendon injury. The aim of this study was to investigate if the high-resolution 3T magnetic resonance imaging (MRI) can demonstrate the complex anatomy of the fingers and thumb, especially the tendons and ligaments, and provide the accurate diagnosis of clinically important fingers and thumbs deformity due to ligamentous and tendinous injuries during sport activities. METHODS: Sixteen fresh un-embalmed cadaveric hands were harves
