Clinical Implications of Intraoperative Fluid Therapy in Pancreatic Surgery.

BACKGROUND: Recent studies have suggested that intraoperative fluid overload is associated with a worse outcome after major abdominal surgery. However, evidence in the field of pancreatic surgery is still not consistent. The aim of this study was to evaluate whether intraoperative fluid management could affect the outcome of a major pancreatic resection. METHODS: Prospective analysis of 350 major pancreatic resections performed in 2016 at the Department of General and Pancreatic Surgery-The Pancreas Institute, University of Verona Hospital Trust. Patients were dichotomized according to intrao

Bolus fluid therapy and sodium homeostasis in paediatric gastroenteritis.

AIM: The study aims to assess the risk of developing hyponatraemia when large-volume bolus fluid rehydration therapy is administered. METHODS: We conducted a prospective randomised study in a tertiary-care centre emergency department. Participants included children with gastroenteritis and dehydration requiring intravenous rehydration. They were randomised to receive 60 mL/kg (large) or 20 mL/kg (standard) 0.9% saline bolus followed by maintenance 0.9% saline for 3 h. Biochemical tests were performed at baseline and 4 h. The primary outcome measure was the development of hyponatraemia at 4 h.

Diuretic therapy in fluid-overloaded and heart failure patients.

Diuretics are the most commonly used drugs to treat clinically diagnosed fluid overload in patients with heart failure. There is no conclusive evidence that they alter major outcomes such as survival to hospital discharge or time in hospital compared to other therapies. However, they demonstrably achieve fluid removal in the majority of patients, restore dry body weight, improve the breathlessness of pulmonary edema and are unlikely to be subjected to a large double-blind randomized controlled trial in this setting because of lack of equipoise. The effective and safe use of diuretics requires

Preoperative fluid and electrolyte management with oral rehydration therapy.

PURPOSE: We hypothesized that oral rehydration therapy using an oral rehydration solution may be effective for preoperative fluid and electrolyte management in surgical patients before the induction of general anesthesia, and we investigated the safety and effectiveness of oral rehydration therapy as compared with intravenous therapy. METHODS: Fifty female patients who underwent breast surgery were randomly allocated to two groups. Before entry to the operation room and the induction of general anesthesia, 25 patients drank 1000 ml of an oral rehydration solution ("oral group") and 25 patient

Iatrogenic hypocalcaemia during parenteral fluid therapy of diarrhoeic calves.

Acid-base balance and electrolyte concentrations, including ionised calcium, were monitored during intravenous fluid therapy of 11 collapsed diarrhoeic suckler calves aged five to 10 days. Six healthy calves of similar age and type were used to provide control data. All the diarrhoeic calves were severely acidotic (TCO2<12 mmol/litre). Isotonic sodium bicarbonate (1-3 per cent) was administered until the metabolic acidosis was half corrected, as indicated by the TCO2 increasing to 17 to 24 mmol/litre when the infusion was changed to an extracellular volume replacement fluid containing 144 mmol

Urinary arginine vasopressin in asthma: consideration of fluid therapy.

To elucidate the role of antidiuretic hormone (ADH) on water and electrolyte balance in patients with asthmatic attacks, urinary arginine vasopressin (AVP) was assayed in 28 asthmatic patients. In a 3-year-old girl with status asthmaticus who developed a grand mal seizure in association with hyponatremia, urinary AVP levels remained high and fluctuated before convulsion; the cause of the convulsion was considered to be water intoxication due to inappropriate ADH secretion. In 19 of 28 patients with moderately severe asthmatic attacks, increases in urinary AVP levels occurred before treatment (

Fluid therapy in severe systemic reaction to radiopaque dye.

Two patients developed severe vascular collapse after left ventriculography with organic iodides. Hemodynamic monitoring showed marked reduction in systemic pressures. In one patient there was no response to the standard therapeutic measures in anaphylactic reactions, and prompt response to fluid administration was obtained. In the second patient response was prompt to fluids, adrenalin, and hydrocortisone.

Managing fluid status in patients receiving renal replacement therapy.

PURPOSE OF REVIEW: Net ultrafiltration (UFNET) may help correct fluid overload in patients receiving renal replacement therapy (RRT). Yet, how to set it safely at the bedside remains unsettled. This review addresses the hemodynamic aspects of UFNET setting, its time-dependent adjustments in relation phase-dependent balance target, and the contribution of fluid inputs to fluid balance control. RECENT FINDINGS: Hemodynamic instability associated with RRT, the principal obstacle to active fluid removal, arises from vasomotor and cardiac mechanisms, which may or may not be related to UFNET. Fluid therapy and traumatic brain injury: A narrative review.

Traumatic brain injury (TBI) is an important health and social problem. The mechanism of damage of this entity could be divided into two phases: (1) a primary acute injury because of the traumatic event; and (2) a secondary injury due to the hypotension and hypoxia generated by the previous lesion, which leads to ischemia and necrosis of neural cells. Cerebral edema is one of the most important prognosis markers observed in TBI. In the early stages of TBI, the cerebrospinal fluid compensates the cerebral edema. However, if edema increases, this mechanism fails, increasing intracranial pressure

[Intravenous fluid therapy and acute kidney injury - What's new?].

Optimization of intravascular volume is crucial for patients who are at risk or undergo Acute Kidney Injury. In sepsis or after acute fluid loss extensive fluid expansion is mostly needed. However, in cardiorenal syndroms fluid overload can even lead to AKI itself and reduction of intravascular volume is needed. Thus, an individualized fluid guidance in terms of a "fluid management stewartship" for the right patient, the right drug, the right route and the right dose 1 has to be applied.

Uninterpretable cerebrospinal fluid absorbance scans caused by antibiotic therapy.

BACKGROUND: The revised national guidelines for analysis of cerebrospinal fluid for bilirubin in suspected subarachnoid haemorrhage (UK) provide an objective means of assessing cerebrospinal fluid samples to determine the risk of subarachnoid haemorrhage. The guidelines are intended for general use, but samples rendered uninterpretable due to the presence of the antibiotic doxycycline have been described. Here, further cases of antibiotic-based interference, and their implications, are presented. METHODS: An archival search of cerebrospinal fluid spectra performed at Hallands County Hospital

Four phases of intravenous fluid therapy: a conceptual model.

I.V. fluid therapy plays a fundamental role in the management of hospitalized patients. While the correct use of i.v. fluids can be lifesaving, recent literature demonstrates that fluid therapy is not without risks. Indeed, the use of certain types and volumes of fluid can increase the risk of harm, and even death, in some patient groups. Data from a recent audit show us that the inappropriate use of fluids may occur in up to 20% of patients receiving fluid therapy. The delegates of the 12th Acute Dialysis Quality Initiative (ADQI) Conference sought to obtain consensus on the use of i.v. fluid

Intravenous fluid therapy - an under-recognized patient safety opportunity.

Inappropriate fluid regimens are rarely documented as being responsible for patient harm, although there is considerable circumstantial evidence to suggest that fluid overload may be a considerably under-estimated source of patient morbidity and mortality. The GIFTASUP fluid management guidelines published in 2008 offer a valuable opportunity to consider a more standardised approach to fluid management, possibly in the context of developing routine hospital services to support good practice.

[Swedish practice routines for fluid therapy are considerably changing].

The optimal fluid therapy for plasma volume support at major surgical procedures and in the treatment of critically ill patients has been a matter of considerable controversy during the last few years. The impact of the ongoing debate on Swedish routines has been assessed from a detailed questionnaire sent to all operation (OP) and intensive care units (ICU) in Sweden (n=130) in September 2004. Response was obtained from 92% of the units. It was found that major changes in fluid treatment regimes had occurred in recent years. In spite of the anti-colloid debate in the 1990s the continued use o

Renal function and fluid therapy in high risk infants.

In recent years, the survival rate of high risk infants has markedly increased. The role of such medical management as fluid, electrolyte and nutritional therapy have assumed a greater importance in assuring optimal quality of the survivors. The very low birth weight infants, particularly those with respiratory distress syndrome and perinatal asphyxia, are at highest risk. The inefficient renal function, unique characteristic of body fluid composition and/or presence of severe clinical illness often make the management of fluid and electrolytes in this group of infants difficult. The numerous
