Will an orange a day keep the doctor away?

An 80 year old man, who relied on a home based meals-on-wheels service was admitted to hospital with non-specific symptoms, but had clinical and biochemical evidence of scurvy. Subsequently, all new admissions (n=37) to the department over a three week period were assessed for evidence of undernutrition. It was found that 73% had hypovitaminosis C, with 30% having concentrations suggestive of scurvy. There were no significant associations between level of vitamin C and type of accommodation, food provision, or age. The commonest symptom associated with vitamin C deficiency was anorexia, but ov

Race, Ethnicity, and Language Differences in Inpatient Discharge Prescriptions for Alcohol Use Disorder at an Academic Medical Center.

BACKGROUND: Disparities exist in alcohol use disorder (AUD) diagnosis, morbidity, and mortality based on patient demographics. Medications for alcohol use disorder (MAUD) have been demonstrated to improve AUD outcomes, and multiple agents are approved for this indication by the Food and Drug Administration (FDA). Hospitalizations are opportunities for MAUD prescribing. This study identifies race, ethnicity, and language-based differences in discharge MAUD prescribing at an academic medical center. METHODS: This was a retrospective cohort study of adult hospitalizations with alcohol withdrawal

The Effect of a Home Delivery Meal Service of Energy- and Protein-Rich Meals on Quality of Life in Malnourished Outpatients Suffering from Lung Cancer: A Randomized Controlled Trial.

Undernutrition is prevalent in cancer patients and associated with increased incidence of complications and mortality. We investigated the effects of a home delivery meal service, providing a selection of energy-dense, protein-rich meals, on quality of life (QoL) in malnourished lung cancer patients. Forty lung cancer patients with nutritional risk score ≥3 (NRS-2002) were randomized to control or intervention. The intervention group was offered energy- and protein-rich main meals and snacks, delivered 3 times per week. The control group continued their habitual diet. Primary endpoint, QoL, an

Measles outbreak from Hong Kong International Airport to the hospital due to secondary vaccine failure in healthcare workers.

OBJECTIVE: To report an outbreak of measles with epidemiological link between Hong Kong International Airport (HKIA) and a hospital. METHODS: Epidemiological investigations, patients' measles serology, and phylogenetic analysis of the hemagglutinin (H) and nucleoprotein (N) genes of measles virus isolates were conducted. RESULTS: In total, 29 HKIA staff of diverse ranks and working locations were infected with measles within 1 month. Significantly fewer affected staff had history of travel than non-HKIA-related measles patients [10 of 29 (34.5%) vs 28 of 35 (80%); P < .01]. Of 9 airport staf

Changes in Emergency Department Concordance with Guidelines for the Management of Food-Induced Anaphylaxis: 1999-2001 versus 2013-2015.

BACKGROUND: Awareness about food allergy and food-induced anaphylaxis (FIA) has increased dramatically over the past decade. It remains unclear, however, whether concordance with guidelines for FIA management has improved over time. OBJECTIVE: Our objective was to describe changes in emergency department (ED) concordance with guidelines for FIA management. METHODS: We analyzed data from 2 multicenter retrospective studies of patients with food-related acute allergic reactions seen in 1 of 17 EDs during 2 time periods: 1999 to 2001 and 2013 to 2015. Visits were identified similarly across yea

Severe food anaphylaxis: 107 cases registered in 2002 by the Allergy Vigilance Network.

BACKGROUND: The prevalence of food allergies increases, relating to diet modifications. The consumption of new foods--exotic foods or foods originally used for animal feed, new proteins, neo allergens due to the use of new technologies and soon, Genetically Modified Foods--are in the spotlight. OBJECTIVE: It is essential to develop a system of food allergy vigilance encompassing the full range of foods being consumed. Understanding this imperative leads logically to the suggestion of developing an allergy vigilance network taking advantage of the ongoing experience of allergists "on the groun

Doctor-patient interactions that exclude patients experiencing homelessness from health services: an ethnographic exploration.

BACKGROUND: People experiencing homelessness have poor health indices and poor access to health care. Their health service utilisation (HSU) is typified by: late illness presentations; poor attendance rates at appointments; low usage of primary care services and outpatient departments; and high utilisation of emergency departments and inpatient services. Why people experiencing homelessness have these particular HSU patterns is poorly understood. AIM: This research sought to explore barriers to health service usage for people experiencing homelessness. DESIGN & SETTING: The authors conducted

Optimising nutrition in residential aged care: A narrative review.

In developed countries the prevalence of protein-energy malnutrition increases with age and multi-morbidities increase nutritional risk in aged care residents in particular. This paper presents a narrative review of the current literature on the identification, prevalence, associated risk factors, consequences, and management of malnutrition in the residential aged care (RAC) setting. We performed searches of English-language publications on Medline, PubMed, Ovid and the Cochrane Library from January 1, 1990 to November 25, 2015. We found that, on average, half of all residents in aged care ar

To the ER? Can Patients Treat Their Anaphylaxis at Home?

PURPOSE OF REVIEW: To discuss if all patients who use self-injectable epinephrine outside the hospital setting require immediate emergency care. RECENT FINDINGS: Prior to 2023, anaphylaxis management guidance universally recommended that patients who use self-injectable epinephrine outside of the hospital or clinic setting immediately activate emergency medical services and seek further care. Additional food-induced anaphylaxis management recommendations specified that all patients always carry 2 auto-injector devices and give a second dose of epinephrine if there was not immediate response w

Evaluation of Prehospital Management in a Canadian Emergency Department Anaphylaxis Cohort.

BACKGROUND: Studies assessing the use of antihistamines and corticosteroids for the treatment of anaphylaxis have not supported a conclusive effect. OBJECTIVE: To assess prehospital management of anaphylaxis by measuring the effect of epinephrine use compared with antihistamines and corticosteroids on negative outcomes of anaphylaxis (intensive care unit/hospital ward admission, multiple doses of epinephrine in the emergency department [ED], and intravenous fluids given in the ED). METHODS: The Cross-Canada Anaphylaxis Registry is a cohort study that enrolls anaphylaxis cases presenting to E

Fresh funds for moms: feasibility of a 12-week online food as medicine grocery prescription program for women with food insecurity and gestational diabetes.

INTRODUCTION: Pregnant women with food insecurity experience high rates of gestational diabetes mellitus (GDM). Food as medicine and grocery prescription (GPx) programs have been successful in increasing food access and managing chronic disease; however, they are often not implemented during pregnancy. METHODS: This was a pilot study of Fresh Funds for Moms, an online grocery prescription (GPx) program. Pregnant women between 20 and 28 weeks' gestation were purposefully recruited from one large hospital system in an urban community in Kentucky. Eligibility included: positive screening for foo

Evaluation of Subcutaneous Phenobarbital Administration in Hospice Patients.

Phenobarbital is used in hospice and palliative care to treat refractory symptoms. In end-of-life care, Food and Drug Administration approved routes of administration may be unreasonable based on patients' status. In these cases, phenobarbital may be administered subcutaneously for symptom management. However, according to the American Hospital Formulary Service, subcutaneous administration of commercially available injectable phenobarbital is cautioned due to possible skin reactions. This study evaluates the tolerability of phenobarbital administered subcutaneously. Of 69 patients and 774 dis

Gender-just reductions in greenhouse gas emissions in maternity care services through the 'baby- friendly hospital initiative': a scoping review.

BACKGROUND: The use of commercial milk formula (CMF) in facilities providing maternity and newborn services disrupts establishment of breastfeeding and adds to greenhouse gas (GHG) emissions. We identified evidence on GHG emissions in these services and explored implications for a gender-just transition to GHG emissions reductions by scaling-up the Baby-Friendly Hospital Initiative (BFHI) to reduce CMF use. METHODS: We performed a scoping review and narrative synthesis on three interrelated topics: 1) GHG emissions impact of CMF use in maternity services, 2) gender-just transitions in agrifoo
