Comparing the immediate effects of UCBL and modified foot orthoses on postural sway in people with flexible flatfoot.

BACKGROUND: Different types of foot orthoses have been prescribed for patients with flatfoot. Results of several studies have shown that orthoses were able to change balance parameters in people with flatfoot. However, the possible effect of orthosis flexibility on balance has not yet been investigated. OBJECTIVES: The aim of the current study was to investigate the immediate effect of a rigid University of California Berkeley Laboratory (UCBL) foot orthosis, a modified foot orthosis, and a normal shoe on the postural sway of people with flexible flatfoot. STUDY DESIGN: Quasi-experimental.

The effect of ankle-foot orthoses on self-reported balance confidence in persons with chronic poststroke hemiplegia.

BACKGROUND: One intervention often used to address physical impairments post stroke is an ankle-foot orthosis. Ankle-foot orthoses may improve walking speed, stride length, and gait pattern. However, effects on balance, crucial for safe ambulation, are thus far inconclusive. One aspect of balance shown to contribute to functional ability is self-efficacy. Self-efficacy, defined as the belief in one's ability to succeed in particular situations, has been shown to be more strongly associated with activity and participation (as defined by the International Classification of Functioning, Disabilit

Comparison of foot orthoses made by podiatrists, pedorthists and orthotists regarding plantar pressure reduction in The Netherlands.

BACKGROUND: There is a need for evidence of clinical effectiveness of foot orthosis therapy. This study evaluated the effect of foot orthoses made by ten podiatrists, ten pedorthists and eleven orthotists on plantar pressure and walking convenience for three patients with metatarsalgia. Aims were to assess differences and variability between and within the disciplines. The relationship between the importance of pressure reduction and the effect on peak pressure was also evaluated. METHODS: Each therapist examined all three patients and was asked to rate the 'importance of pressure reduction'

Methods for measuring the sagittal resistive torque of ankle-foot orthoses during gait: A systematic review.

The measurement of resistive torque exerted by passive ankle-foot orthoses during walking plays a critical role in understanding their biomechanical effects and in optimizing their functionality. This systematic review investigates existing measurement methods to evaluate how orthotic torque is quantified during gait, offering insights into currently used methods and technologies. Because many methods are closely linked to the use of test benches, these methods are also discussed but are not the scope of this work. A comprehensive systematic search was performed following the Preferred Reporti

An exploration of the factors predicting clinical responses to customized foot orthoses in individuals with plantar heel pain.

BACKGROUND: Customized foot orthoses (CFOs) are commonly used to manage plantar heel pain (PHP); however, individual responses vary. This study aimed to evaluate the effects of CFOs on pain and foot function and identify predictors of treatment response. METHODS: A total of 120 individuals with PHP underwent baseline assessments, including demographics, pain, foot function, and physical examination. Participants wore CFOs for at least 6 hours daily over six months and were reassessed for pain and foot function. Treatment response was assessed using a 15-point Global Rating of Change (GRC) sca

Ankle-foot orthoses for improving walking in adults with calf muscle weakness due to neuromuscular disorders.

BackgroundCalf muscle weakness is a common symptom in slowly progressive neuromuscular disorders that lead to walking problems like instability and increased walking effort. The mainstay of treatment to improve walking in this population is the provision of ankle-foot-orthoses (AFOs). Since we are not aware of an up-to-date and complete overview of the effects of AFOs used for calf muscle weakness in slowly progressive neuromuscular disorders, we reviewed the evidence for the effectiveness of AFOs to improve walking in this patient group, in order to support clinical decision-making.Objectives

A novel apparatus to assess the mechanical properties of Ankle-Foot Orthoses: Stiffness analysis of the Codivilla spring.

Ankle-Foot Orthoses (AFOs) are the most common devices prescribed to support the ankle and restore a quasi-normal gait pattern in drop-foot patients. AFO stiffness is possibly the main mechanical property affecting foot and ankle biomechanics. A variety of methods to evaluate this property have been reported, however no standard procedure has been validated and widely used. This study is reporting the repeatability of a novel apparatus to measure AFO stiffness in ideal frictionless conditions. The apparatus is based on a servo-hydraulic testing machine and allows to apply a displacement-contro

Replicating and redesigning ankle-foot orthoses with 3D printing for children with Charcot-Marie-Tooth disease.

BACKGROUND: Children with the most common inherited neuropathy, Charcot-Marie-Tooth disease (CMT), are often prescribed ankle-foot orthoses (AFOs) to improve walking ability and prevent falls by reducing foot drop, postural instability, and other gait impairments. These externally worn assistive devices are traditionally custom-made using thermoplastic vacuum forming. This labour-intensive manufacturing process often results in AFOs which are cumbersome due to limited design options, and are associated with low acceptability, discomfort, and suboptimal impact on gait. The aim of this study was

The efficacy of shoe modifications and foot orthoses in treating patients with hallux rigidus: a comprehensive review of literature.

Hallux rigidus (HR) is a degenerative disease of the first metatarsophalangeal (MTP1) joint and affects about 2.5% of people older than 50 years. The real etiology of this condition remains under debate. Clinical symptoms, physical exam, and instrumental evidence are important in assessing and grading the disease. The anatomy of the first metatarsal is unique and its configuration may play a significant role in the HR development. The first approach usually begins with shoe modifications and foot orthoses, designed to limit irritation from the dorsal osteophytes, reducing motion and the mechan

The impact of ankle-foot orthoses on toe clearance strategy in hemiparetic gait: a cross-sectional study.

BACKGROUND: Ankle-foot orthoses (AFOs) are frequently used to improve gait stability, toe clearance, and gait efficiency in individuals with hemiparesis. During the swing phase, AFOs enhance lower limb advancement by facilitating the improvement of toe clearance and the reduction of compensatory movements. Clinical monitoring via kinematic analysis would further clarify the changes in biomechanical factors that lead to the beneficial effects of AFOs. The purpose of this study was to investigate the actual impact of AFOs on toe clearance, and determine the best strategy to achieve toe clearance
