A systematic approach to the interpretation of gunshot wound trauma to the cranium.

This chapter employs the fracture assessment triad, a systematic analytical approach, as a vehicle to explore the components used to interpret gunshot trauma to the cranium. First, a list of pertinent observations associated with gunshot trauma to the cranium is presented with a brief description of each. These features include entrance and exit wound defect morphology, keyhole defect, gutter defect, radiating fractures, concentric fractures, bone plugs, and associated foreign material, debris and residue. Second, the intrinsic factors that govern the way bone responds to trauma can be surmise

Does preliminary optimisation of an anatomically correct skull-brain model using simple simulants produce clinically realistic ballistic injury fracture patterns?

Ballistic head injury remains a significant threat to military personnel. Studying such injuries requires a model that can be used with a military helmet. This paper describes further work on a skull-brain model using skulls made from three different polyurethane plastics and a series of skull 'fills' to simulate brain (3, 5, 7 and 10% gelatine by mass and PermaGel™). The models were subjected to ballistic impact from 7.62 × 39 mm mild steel core bullets. The first part of the work compares the different polyurethanes (mean bullet muzzle velocity of 708 m/s), and the second part compares the d

Cranial trauma in handgun executions: Circumferential delamination defect and its relationship with contact/close-range shooting.

Skeletal evidence usually constitutes the only source of information to interpret lesion patterns that help to clarify the circumstances surrounding death. The examination and interpretation of bone trauma are essential to the application and utility of anthropology as a forensic science. When discussing the effect of gunshot wounds in bone, it becomes imperative to differentiate between short and long-distance injuries based on clear, distinct, and observable signs. To contribute to the debate, our focus is directed toward the external analysis of the so-called circumferential delamination de

When backyard fun turns to trauma: risk assessment of blunt ballistic impact trauma due to potato cannons.

Although potato cannons are an area of great interest among internet users, they are almost completely unknown in the medical community. These simple ballistic devices are made from plastic plumbing pipes and are powered with propellant gas from aerosol cans. By combustion of the gas-oxygen mixture, a high pressure is produced which propels the potato chunks through the barrel. It is the aim of this study to investigate the hazardous potential of these shooting devices. Test shots were performed using three illegally manufactured potato cannons that were confiscated by police authorities. Velo

Three-dimensional reconstitution of bullet trajectory in gunshot wounds: a case report.

In the medico-legal assessment of cases of aggression with firearms, imaging techniques have a particularly important role, especially in the study of a bullet's path through the victim's body. The analysis of these trajectories can be performed by the use of three-dimensional reconstitution techniques, namely Three-Dimensional Multi-Slice Computed Tomography (3D-MSCT). This imaging technique has been widely used in fatal cases, as a very important complement of the classical autopsy procedures, becoming known as "virtual autopsy" or "Virtopsy". To our knowledge, no reports describing the use

A neurosurgical reappraisal of President Lincoln's fatal cranial injury.

President Abraham Abraham Lincoln's assassination in April 1865 remains one of the most scrutinized cases in forensic history. We re-examined his fatal head wound through the lens of modern neurosurgery, focusing on projectile trajectory, biomechanics, skull base anatomy, fluid percussion, and periorbital injury, and including the use of artificial intelligence search. A systematic review was conducted using PubMed (n = 19) and Scopus (n = 12), along with 5 additional sources available on the Internet. Following PRISMA guidelines, 3 original studies were included. Historical autopsy documents

Homicides and suicides by firearm in Marseille: An 8-year review.

The regions of Provence-Alpes-Côte d'Azur and Corsica in south-east France are regularly affected by firearm deaths, and a large amount of data has been collected by our forensic institute. We carried out a single-center retrospective descriptive study of the records of our institute between January 1, 2011, and 31 December, 2018, relating to firearm deaths (homicides and suicides). There were 302 cases (218 homicides and 84 suicides). The anatomic locations most frequently involved were the thorax, abdomen and head, in that order, in homicides and the head in suicides. More than 80% of the ho

Gunshot fatalities: correlation between post-mortem multi-slice computed tomography and autopsy findings: a 30-months retrospective study.

In this study we compared retrospectively the autopsy and the CT-scan findings in 47 gunshot victims (96 wounds) in order to assess the accuracy of the two methods in injury and ballistic diagnoses. Firing distance was determined in 46 wounds (47.9%). Firing range was determined by autopsy alone in 37 cases (80.5%) and by CT-scan alone in three cases (6.5%). In the six remaining cases (13%), autopsy and CT-scan reports concluded both to the same firing range. Entrance wounds were detected concordantly by both CT-scan and autopsy in 63 of the 91 penetrating wounds (69.2%). Exit wounds were pres

Trauma attenuating backing improves protection against behind armor blunt trauma.

BACKGROUND: Body armor is used by military personnel, police officers, and security guards to protect them from fatal gunshot injuries to the thorax. The protection against high-velocity weapons may, however, be insufficient. Complementary trauma attenuating backings (TAB) have been suggested to prevent morbidity and mortality in high-velocity weapon trauma. METHODS: Twenty-four Swedish landrace pigs, protected by a ceramid/aramid body armor without (n = 12) or with TAB (n = 12) were shot with a standard 7.62-mm assault rifle. Morphologic injuries, cardiorespiratory, and electroencephalogram

Difference in Upper Limb Length Measurements Between Different Methods at External Postmortem Examination: Implication for Gunshot-Related Deaths.

In gunshot-related deaths, measuring upper limb lengths is commonly performed. At postmortem examination, the upper limbs are commonly flexed from rigor mortis, and, for convenience, lengths are sometimes measured flexed. However, it is unclear whether there is a difference between the upper limb lengths when measured flexed and extended and placed in the standard anatomic position. This prospective study compared the upper limb length in 30 adults when the upper limb was in rigor mortis and flexed to when rigor mortis was broken, upper limbs extended and placed in standard anatomic position.

Minimum velocity necessary for nonconventional projectiles to penetrate the eye: an experimental study using pig eyes.

To satisfy the Criminal Code of Canada's definition of a firearm, a barreled weapon must be capable of causing serious bodily injury or death to a person. Canadian courts have accepted the forensically established criteria of "penetration or rupture of an eye" as serious bodily injury. The minimal velocity of nonconventional ammunition required to penetrate the eye including airsoft projectiles has yet to be established. To establish minimal threshold requirements for eye penetration, empirical tests were conducted using a variety of airsoft projectiles. Using the data obtained from these test

Assessment of polyurethane spheres as surrogates for military ballistic head injury.

SYNBONE® spheres were impacted with 7.62 × 39 mm mild steel core ammunition at a mean impact velocity of 654 m/s, SD 7 m/s, to simulate engagement distances of around 50-100 m. The wounds and fracture patterns were assessed by two forensic pathologists familiar with military cranial injury. The overall fracture pattern was assessed as being too comminuted when compared with actual injury. This suggests the SYNBONE® spheres have less utility for simulating military injury than other purposes described in the literature.

Use of luminescent gunshot residues markers in forensic context.

Chemical evaluation of gunshot residues (GSR) produced by non-toxic lead-free ammunition (NTA) has been a challenge to forensic analyses. Our group developed some luminescent markers specific to the detection of GSR. Here, we evaluated the performance of selected markers in experiments that mimic forensic context and/or routines in which luminescent characteristics would be very useful. We evaluated the influence of markers' addition on the bullet's speed, the rate of shot failure (i.e., when the cartridge case is not fully ejected and/or a new ammunition is not automatically replaced in the g

[Fatal airgun shot].

The authors present a case of an airgun shot. The airgun Magnum Sport 4.5 mm uses wasp-waist Diabolo style pellets. The initial velocity is 260-290 m/s. This type of guns does not need registration because the energy of the bullet is less than 17 J. In the presented case, the airgun shot could have been an accident occurring while playing with the airgun or else it could have been be feigned. The accused maintained that the victim himself had placed the gun in the subclavicular area and keeping the weapon almost vertically, released the trigger. The victim had not been aware the airgun was loa

Collateral damage--penetrating head injury and orbital injury: a case report.

We report a case of an accidental death or potential suicide by revolver with subsequent injury of another person. A 44-year-old man shot himself in the head while manipulating his .38 caliber special revolver in the kitchen in the presence of his wife, standing approximately 1.5 m next to him. After passing through the husband's head, the lead round-nose bullet entered the region underneath his wife's left eye. When the bullet left the man's head, it retained the energy to penetrate the soft tissue at this distance, including the skin and thin bone plates like the orbital wall. Owing to the l

Atypical gunshot wounds resulting in fetal death: two rare case reports.

BACKGROUND: Gunshot wounds (GSW) pose serious risks to both mother and fetus. Determining the trajectory and cause of death in the fetus in GSW is of forensic importance. This study aims to contribute to the literature with two rare GSW cases leading to fetal death through different mechanisms. CASE REPORT: Two cases of pregnant women exposed to firearm violence were analyzed. In the first case, the fetus sustained 3 GSWs. Autopsy findings revealed fatal organ injuries and fractures. The trajectory was reconstructed using characteristic bone fragments on the ribs, confirming the path from fro
