BACKGROUND: Jones fractures occur in the relatively avascular metadiaphyseal junction of the fifth metatarsal (MT), which predisposes these fractures to delayed union and nonunion. Operative treatment with intramedullary (IM) screw fixation is recommended in certain cases. Incorrect screw selection can lead to refractures, nonunion, and cortical blowout fractures. A better understanding of the anatomy of the fifth MT could aid in preoperative planning, guide screw size selection, and minimize complications.
METHODS: We retrospectively identified foot computed tomographic (CT) scans of 119 pat

BACKGROUND: For unstable proximal humerus fractures, both plates and nails may be recommended. We introduce an anterograde nail designed for the treatment of 3- and 4-parts proximal humerus fractures. The aim of this study is to compare the biomechanics of this nail versus a plate and then to analyze the relationships of the screws with the axillary nerve. Our working hypotheses are as follows: (1) this nail is biomechanically equal or better to the reference plate and (2) it does not endanger the axillary nerve.
METHODS: Biomechanical study: using 40 sawbones, a reproducible 4-part fracture

UNLABELLED: The fracture of the fifth metacarpal neck (also called a boxer's fracture) is the most common fracture of the hand1,3. Displaced fractures often result in volar angulation of the metacarpal head, shortening, and residual malrotation4-7. The present video article demonstrates the steps of performing intramedullary single-Kirschner-wire fixation of the fifth metacarpal neck1, with the aim of the procedure being to achieve a closed reduction and internal stabilization of such a fracture. Although many fractures can be treated with a splint only, surgery should be performed in patients

INTRODUCTION: Clavicle fractures are frequently encountered by orthopaedic surgeons. Though multiple treatment techniques have been described in literature, open reduction and internal fixation with plating and intramedullary nailing are preferred in adults. This study analyses the functional outcome, complications, duration of bony union of mid-shaft clavicle fractures treated with plate fixation versus intramedullary fixation.
MATERIALS AND METHODS: A quasi-experimental study was conducted on 38 patients with mid-shaft clavicle fracture aged between 18 to 60 years at a tertiary care centre

PURPOSE: The biomechanics of surgically treated metacarpal base fractures are poorly described. We hypothesized that intramedullary threaded nail (IMTN) fixation and dorsal plate and screw (DPS) fixation would perform similarly in multiple fracture locations approaching the metacarpal base.
METHODS: Sixty-four metacarpals from eight matched pair cadaveric hands were stripped. Matched pairs were randomly divided into four cohorts based on fracture distance from the metacarpal base. Transverse osteotomies were created at 50%, 35%, 25%, and 15% of the total metacarpal length, measured from the m

The operative treatment of distal fibula fractures is debated. This study compared intramedullary nailing (IMN) versus open reduction and internal fixation (ORIF) in patients with distal fibula fractures. The outcomes of interest were patient-reported outcome measures (PROMs) and the rate of anatomic reduction, bony union, complication, and nerve injury. This study was conducted according to the 2020 PRISMA statement. In October 2024, the following databases were accessed: PubMed, Web of Science, Google Scholar, and Embase, with no time constraint. All the clinical studies comparing IMN versus

This is a randomised study to compare two types of osteosynthesis to mobilise wrists after distal fractures of the radius. Inclusion criteria were Older type 2 and 3 fractures. External fixation was managed with Hoffmann II compact non-bridging. Internal fixation was managed with Micronail. Patients were followed up for 12 weeks. The primary outcome was the results of the disabilities of arm, shoulder and hand (DASH) questionnaire. The secondary outcomes were answers to the patient-rated wrist evaluation (PRWE), grip strength, satisfaction, radial length, and volar tilt. Thirty patients were r

CASE: A 12-year-old boy with an isolated both bone forearm fracture was treated with closed reduction and flexible intramedullary nailing 11 days postinjury and after the loss of initial fracture alignment. On nail insertion, an intraoperative distal radial physeal fracture was encountered, thereby warranting modification in treatment.
CONCLUSION: Physeal injury is a rare intraoperative complication of intramedullary fixation of forearm fractures. This report provides insight regarding the management and prevention of this rare complication.

BACKGROUND: Distal ulnar fractures can be challenging to manage, and noncompressive intramedullary screws may be a potential solution to this problem. The purpose of this study was to review our institutional experience with retrograde screw fixation of distal ulna fractures compared with conventional plating methods.
METHODS: A retrospective cohort study was designed to compare adult patients who had undergone retrograde intramedullary screw fixation for a distal third ulna fracture in a single health system between June 2021 and February 2024 with controls who underwent conventional plate a

Proximal phalangeal fractures occur at a higher frequency than other phalanges, but the optimal management of these injuries remains subject to debate. The use of intramedullary screw (IMS) fixation has gained popularity over the last decade and aims to minimize extensor apparatus violation whilst providing adequate construct stability to allow early motion and rehabilitation. This systematic review aims to provide an updated analysis of patient- and clinician-reported outcome measures following fracture fixation using IMS in the proximal phalanx. A literature search was conducted using PubMed

Newer intramedullary (IM) nails have become another option in the fixation of proximal tibia fractures. There is limited data on the successful use of these implants in intra-articular and extra-articular fractures of the proximal tibia, and no studies assessing the ability of these implants to maintain alignment with early weight bearing. Our objective was to determine whether immediate weight bearing after IM fixation, with or without supplemental plate or screw fixation, of proximal third tibial fractures (OTA/AO 41A-C) results in a change in alignment prior to union. 35 patients with 39 pr

Ankle fractures are a common intra-articular trauma. The gold standard treatment for unstable ankle fractures has been Open Reduction Internal Fixation with plating and lag screw(s). Advocates for intramedullary nailing of the fibula note decreased wound complications and greater biomechanical strength. We hypothesized that time to union and time to weight bearing would be decreased with a fibular intramedullary nail compared to prior ankle ORIF literature. A retrospective cohort study was conducted of 65 consecutive ankle fracture patients treated with a fibular nail. The median follow-up tim

INTRODUCTION: Compartment syndrome complicating intramedullary nailing of closed tibia fractures has been described as early as the 1980s, but currently remains less described in literature compared to compartment syndrome directly following trauma. This study aims to review this potentially disabling complication and highlight the importance of timely diagnosis and management of compartment syndrome following fracture fixation, not just after fracture itself, via a review of three cases.
MATERIAL AND METHODS: A retrospective study of a series of three cases was conducted. The type of fractur

There is growing literature supporting the use of intramedullary fixation for fracture care because of its smaller incisions, improved biomechanical outcomes, and faster time to weightbearing than traditional internal fixation methods. The aim of this study is to investigate the postoperative outcomes in ankle fractures treated with intramedullary nail fixation in the largest patient cohort to date. From 2015 to 2021, 151 patients were evaluated following surgical treatment of fibular fractures with intramedullary nail fixation. Patients were identified through a medical record database search
