A retrospective study of 645 cholecystectomies performed in a surgical unit over a 10-year period is presented, of which 236 were carried out during an acute admission. Of these 236 cholecystectomies, 195 were performed for acute cholecystitis and 41 for acute gallstone pancreatitis. In the acute cholecystitis group the proportion of patients over 70 years of age was significantly higher (35 per cent) than the corresponding elective group (10.3 per cent). We evaluated the gallbladders of 121 patients who had well-documented primary sclerosing cholangitis. Sonograms, cholangiograms, and CT scans were reviewed, and the findings were correlated with surgical or autopsy findings, when available. Pathologic examination of the gallbladder was available in 55 (45%) of the 121 patients; of these, 49 (89%) had abnormal gallbladders. Surgical treatment of benign gallbladder disease seems relatively simple, but it causes many problems and controversies, which should not be ignored. According to the currently published related spirit of consensus guidelines and clinical practice, the treatment choices for five aspects of clinical problems should be paid attention to, and the problem for "blindly cholecystolithotomy"and "prudent choice cholecystectomy indications and surgical timing" were summarized and analyzed in order to cause enough attention for careful diagnostic and treatment of benign gallbladder diseases.

This paper presents a survey of possibilities and limitations of ultrasound in diseases of the gallbladder and the biliary tract. The diagnostic value of ultrasound is demonstrated and compared to other radiologic procedures as i.v. cholecystocholangiography, transhepatic cholangiography and endoscopic retrograde cholangiography. The diagnosis of gallbladder lesions and extrahepatic biliary tract obstruction can be sufficiently assessed by ultrasound, in most cases there is no need of other invasive procedures.
