Giant duodenal gallstone presenting as gastric outlet obstruction: Bouveret's syndrome.

In a 91 year old woman with nausea and vomiting, the diagnosis of Bouveret's syndrome was considered when a barium meal disclosed a cholecystoduodenal fistula and a giant filling defect in the duodenum. Because of her age and underlying medical illness, operative therapy was initially deferred. Repeated attempts to remove the intermittently obstructing duodenal gallstone endoscopically were unsuccessful using both endoscopic retrograde cholangiopancreatography retrieval baskets and an endoscopic mechanical lithotripter. The patient was referred for definitive operative therapy, and was dischar

Preliminary experience with hydrostatic balloon dilation of gastric outlet obstruction.

In treating a group of 15 patients with gastric outlet obstruction, 12 (80%) had good to excellent relief of symptoms. Two patients required surgical intervention. One patient has symptoms that were persistent but mild enough to forego surgery. Balloon dilation offers an alternative to the surgical management of gastric outlet obstruction.

Gastric outlet obstruction due to X-linked chronic granulomatous disease.

Children with chronic granulomatous disease (CGD) may develop a peculiar form of gastric outlet obstruction due to involvement of the stomach wall with granulomatous tissue. An illustrative case is presented (which occurred despite adequate control of infection with antibiotherapy), including details of surgical management, distinctive radiographic and pathologic findings, and a review of other reported cases.

Delayed Presentation of Gastric Volvulus with Gastric Outlet Obstruction after Laparoscopic Sleeve Gastrectomy.

BACKGROUND: Globally among bariatric procedures, SG popularity has grown significantly. New complications arise as a result of the rapid growth in the numbers of LSG. Once SG has been performed, the stomach is left with no fixations along the entire greater curvature, which may predispose to volvulus. Case Report. The 44-year-old patient had laparoscopic sleeve gastrectomy 2 years and 6 months before presentation in our emergent department complaining of episodic intolerance to oral intake for 3 months before presentation. Gastrografin swallow study showed gastric outlet obstruction. The patie

A Rare Case of Gastric Lipoma Presenting with Gastric Outlet Obstruction Treated Endoscopically.

An 85-year-old male referred to the Gastroenterology (GI) clinic with three-month history of failure to thrive and three-week history of nausea, vomiting, and melanotic stools. Ulcerative mass obstructing gastric outlet was found on endoscopy and on follow-up CT abdomen a homogeneous submucosal mass in the gastric antrum was identified. Radiological diagnosis of giant gastric lipoma was established and patient was evaluated for surgery and, however, was rendered unfit for surgery due to his comorbid conditions. Patient was taken for endoscopic resection of the mass. On endoscopy, only partial

Gastric outlet obstruction secondary to paraesophageal herniation of gastric antrum after laparoscopic fundoplication.

The most common causes of acute gastric outlet obstruction (GOO) are duodenal and type 3 gastric ulcers. However, mechanical or functional causes may also lead to this pathology. Acute GOO is characterized by delayed gastric emptying, anorexia, or nausea accompanied by vomiting. Herein we report a 56-year-old man diagnosed with GOO secondary to paraesophageal hiatal herniation of gastric antrum after laparoscopic fundoplication. Because of the rarity of this disease, common gastrointestinal complaints may mislead the emergency physician to diagnose a nonsurgical gastrointestinal disease if a d

Radiological findings of gastric adenomyoma in a neonate presenting with gastric outlet obstruction.

Gastric adenomyoma is a rare tumour-like lesion composed of glandular components and smooth muscle bundles. We report a case of gastric adenomyoma in a 1-week-old neonate who presented with gastric outlet obstruction. To the best of our knowledge, this is the youngest child reported with gastric adenomyoma and a unique case demonstrating radiological findings of gastric adenomyoma in a young infant. At US, the lesion was seen as an asymmetrical mass-like wall-thickening of the pylorus. Upper gastrointestinal series showed findings similar to those seen in a case of hypertrophic pyloric stenosi

A Complex Differential for Gastric Outlet Obstruction: Bezoar or Bouveret?

Bezoars are a rare but important cause of gastric outlet obstruction (GOO). Recognizing this diagnosis promptly is important to prevent complications, such as ulceration, bleeding, and perforation. Typical presenting symptoms include postprandial nausea, vomiting, and epigastric abdominal pain. However, these findings can be non-specific and imitate other rare causes of GOO. Here, we describe a case of a 77-year-old male who presented with GOO due to a phytobezoar mimicking Bouveret's syndrome. This case highlights the diagnostic and therapeutic challenges of GOO.

Gastric Outlet Obstruction Secondary to Groove Pancreatitis Mimicking Pancreatic Cancer.

Groove pancreatitis is a rare complication of chronic pancreatitis that can lead to severe manifestations, including gastric outlet obstruction. A 37-year-old man with a history of chronic alcohol use presented with recurrent epigastric pain radiating to the back, accompanied by nausea and vomiting. Imaging revealed features of chronic pancreatitis such as pancreatic calcifications, pseudocyst formation, and a mass-like lesion in the pancreatic head causing duodenal stenosis. Despite medical management and endoscopic interventions like biliary and duodenal stenting, the patient experienced per

Point-of-care Ultrasound Diagnosis of Gastric Outlet Obstruction Syndrome.

Gastric Outlet Obstruction (GOO) is a clinical syndrome characterized by postprandial vomiting, epigastric pain, and abdominal distension due to mechanical or motility disorders. The suspicion will mainly rely on abdominal radiological imaging (computed tomography, barium studies) that might not be widely available or even be contraindicated. We report a 65-year-old male who developed progressive epigastralgia, anorexia, and vomiting. Physical examination revealed mild abdominal distension and epigastric tenderness on deep palpation. With the presumptive diagnosis of gastric outlet obstruction

No way out: Causes of duodenal and gastric outlet obstruction.

Upper gastrointestinal obstruction (UGIO), obstruction occurring at the level of the stomach or duodenum, represents only about 5% of bowel obstructions. As with other bowel obstructions, timely diagnosis is necessary to prevent complications including ischemia and death. Because the presenting symptoms of UGIO can be vague and nonspecific, the diagnosis may not be suspected clinically. The radiologist therefore provides immense value as the diagnosis and often the etiology of the obstruction can be ascertained through imaging. Here we present a simple classification scheme of etiologies of UG

Malignant gastric outlet obstruction: Which is the best therapeutic option?

Malignant gastric outlet obstruction (MGOO) is a clinical condition characterized by the mechanical obstruction of the pylorus or the duodenum due to tumor compression/infiltration, with consequent reduction or impossibility of an adequate oral intake. MGOO is mainly secondary to advanced pancreatic or gastric cancers, and significantly impacts on patients' survival and quality of life. Patients suffering from this condition often present with intractable vomiting and severe malnutrition, which further compromise therapeutic chances. Currently, palliative strategies are based primarily on surg

Bouveret syndrome as a rare cause of gastric outlet obstruction.

Biliary-enteric fistula is a rare complication of cholelithiasis that can lead to gallstone ileus. Gallstone impaction in the duodenum and pylorus is extremely rare and can lead to gastric outlet obstruction, a condition known as Bouveret syndrome. Bouveret syndrome needs to be diagnosed and managed in a timely fashion, as it has a high mortality rate. We describe a case of an elderly patient who presented with Bouveret syndrome secondary to impaction of the biliary calculus in the first part of duodenum.

Giant hepatic cyst as a cause of gastric outlet obstruction.

We describe the case of a 58-year-old female who presented to her primary care provider with lifelong anorexia, 6-week history of liquid only diet and new onset epigastric abdominal pain radiating to the back accompanied by nausea and abdominal distension. An initial computed tomography scan with contrast demonstrated a massive simple hepatic cyst with mass effect compression of the duodenal sweep. Repetitive treatment with aspiration sclerotherapy using hypertonic saline provided initial resolution of symptoms and led to substantial reduction of cyst diameter. Repeat imaging demonstrated comp

EUS-guided gastroenterostomy in management of benign gastric outlet obstruction.

BACKGROUND AND STUDY AIMS: Endoscopic ultrasound-guided gastroenterostomy (EUS-GE) in malignant gastric outlet obstruction (GOO) appears to be promising; however, its role in benign GOO is unclear. The aim of this study was to ascertain the clinical efficacy and safety of EUS-GE in benign GOO. PATIENTS AND METHODS: This was an international retrospective series involving 5 tertiary centers. Consecutive patients who underwent EUS-GE between 1/2013 - 10/2016 for benign GOO were included. The primary endpoint was the rate of clinical success defined as ability to tolerate oral intake without vom

An uncommon cause of gastric outlet obstruction: A case report.

RATIONALE: Metastatic involvement of the gastrointestinal tract is an uncommon scenario encountered in the clinical practice. Our case represents a gastric outlet obstruction (GOO) as a consequence of distant Transitional cell carcinoma (TCC) metastasis without any lymph node involvement in association with inflammatory stranding leading to extrinsic duodenal obstruction. PATIENT CONCERNS: We report an unusual case of a 73-year-old male presented with a five-week history of nausea, vomiting and abdominal pain due to the metastatic extension from TCC that had been considered in remission. Trichobezoar presenting as a gastric outlet obstruction: A case report.

INTRODUCTION: Rapunzel syndrome is a rare intestinal condition that starts with the ingestion of a trichobezoar. The condition is predominately found in females and can be associated with trichotillomania, or the compulsive urge to pull one's own hair out. There are less than 40 cases described in the literature with the prevention of recurrence aimed at psychological treatment. PRESENTATION OF CASE: The patient is a 7 year-old girl with a history of trichotillomania with trichophagia as a young child who presented with abdominal pain, nausea, and vomiting, consistent with a gastric outlet ob
