Dysbiosis of small intestinal microbiota in liver cirrhosis and its association with etiology.

Cirrhosis-associated duodenal dysbiosis is not yet clearly defined. In this research, duodenal mucosal microbiota was analyzed in 30 cirrhotic patients and 28 healthy controls using 16S rRNA gene pyrosequencing methods. The principal coordinate analysis revealed that cirrhotic patients were colonized by remarkable different duodenal mucosal microbiota in comparison with controls. At the genus level, Veillonella, Megasphaera, Dialister, Atopobium, and Prevotella were found overrepresented in cirrhotic duodenum. And the duodenal microbiota of healthy controls was enriched with Neisseria, Haemoph

Microbial basis of oral malodor development in humans.

To better understand the microbial basis of oral malodor development in humans, we used a cross-sectional and longitudinal study design and the pyrosequencing approach to track and compare the tongue microbiota associated with oral malodor in 29 Chinese adults who underwent a consecutive three-day evaluation for the amount of H2S excreted orally. Three levels of the oral malodor state (healthy, oral malodor, and severe oral malodor) were defined based on the H2S level. Community structure of the tongue plaques was more sensitive to changes of malodor state than to interpersonal variations or d

Salivary and tonsillar microbiota across multiple sites in paediatric tonsillar hypertrophy and their associations with clinical factors.

BACKGROUND: Tonsillar hypertrophy (TH) and adenotonsillar hypertrophy (ATH) are primary risk factors for paediatric obstructive sleep apnoea (OSA), but their salivary microbiota differ. OBJECTIVE: This study aimed to investigate whether the tonsillar surface and core microbiota resemble the salivary microbiota, and whether these microbial profiles are influenced by adenoid hypertrophy and related clinical factors. DESIGN: Forty-nine children undergoing tonsillectomy were enrolled, including 21 children with TH and 28 with ATH. Saliva and tonsillar surface swabs were collected preoperatively,

Cryptogenic HACEK (Haemophilus, Aggregatibacter, Cardiobacterium, Eikenella, and Kingella) CNS Abscess in an Otherwise Healthy Patient With a Patent Foramen Ovale.

Actinomyces and HACEK (Haemophilus, Aggregatibacter, Cardiobacterium, Eikenella, and Kingella) organisms are part of the oral microbiome and rarely affect the central nervous system (CNS). CNS infections with these agents can be caused by contiguous or hematogenous spread. We present a case of a 25-year-old immunocompetent male who presented with a one-week history of progressively worsening generalized headaches, photophobia, nausea, and vomiting. Despite a normal physical exam and the absence of leukocytosis, head imaging showed a right frontal lobe abscess. The patient was started empirical

The composition of the perinatal intestinal microbiota in horse.

The establishment of the intestinal microbiota is critical for the digestive and immune systems. We studied the early development of the rectal microbiota in horse, a hindgut fermenter, from birth until 7 days of age, by qPCR and 16S rRNA gene amplicon sequencing. To evaluate initial sources of the foal microbiota, we characterised dam fecal, vaginal and oral microbiotas. We utilised an amplicon sequence variant (ASV) pipeline to maximise resolution and reproducibility. Stringent ASV filtering based on prevalence and abundance in samples and controls purged contaminants while preserving intest

Exploring the dynamic core microbiome of plaque microbiota during head-and-neck radiotherapy using pyrosequencing.

Radiotherapy is the primary treatment modality used for patients with head-and-neck cancers, but inevitably causes microorganism-related oral complications. This study aims to explore the dynamic core microbiome of oral microbiota in supragingival plaque during the course of head-and-neck radiotherapy. Eight subjects aged 26 to 70 were recruited. Dental plaque samples were collected (over seven sampling time points for each patient) before and during radiotherapy. The V1-V3 hypervariable regions of bacterial 16S rRNA genes were amplified, and the high-throughput pyrosequencing was performed. A

Endocarditis due to rare and fastidious bacteria.

The etiologic diagnosis of infective endocarditis is easily made in the presence of continuous bacteremia with gram-positive cocci. However, the blood culture may contain a bacterium rarely associated with endocarditis, such as Lactobacillus spp., Klebsiella spp., or nontoxigenic Corynebacterium, Salmonella, Gemella, Campylobacter, Aeromonas, Yersinia, Nocardia, Pasteurella, Listeria, or Erysipelothrix spp., that requires further investigation to establish the relationship with endocarditis, or the blood culture may be uninformative despite a supportive clinical evaluation. In the latter case,

Role of cervicovaginal microbiome and metabolome in ALA-PDT-mediated HPV clearance in patients with cervical HSIL.

The cervicovaginal microbiome and its associated metabolites play a pivotal role in the persistence of human papillomavirus (HPV) infection. Here, we investigated the contribution of the cervicovaginal microbiome and metabolome to 5-aminolevulinic acid photodynamic therapy (ALA-PDT)-mediated HPV clearance in patients with cervical high-grade squamous intraepithelial lesions (HSIL). 16S ribosomal RNA sequencing analysis identified significant differences in cervicovaginal microbiota before ALA-PDT between the HPV clearance and non-clearance groups. The HPV non-clearance group exhibited higher a

Microbial signatures of dental caries in the incarcerated elderly: a salivary microbiota study in a restricted environment.

BACKGROUND: Dental caries is driven by microbial dysbiosis and influenced by diet and lifestyle. Incarcerated populations living under regulated regimens offer a unique model to study the oral microbiota in older adults by minimising environmental confounding variables. OBJECTIVES: This study aimed to characterise the salivary microbiota of older incarcerated adults and identify bacterial taxa associated with caries status and severity. DESIGN: Twenty-eight incarcerated men (aged ≥ 50 years) were stratified into caries-active (CA) and caries-free (CF) groups (n = 14 each). The salivary micro

Extended Intrapleural Tissue Plasminogen Activator (tPA)-Dornase Alfa (DNase) Therapy Beyond the Standard Multicenter Intrapleural Sepsis Trial 2 (MIST-2) Regimen in a High-Risk Patient With Complex Empyema: A Case Report.

Pleural infection, including complicated parapneumonic effusion and empyema, remains a major management challenge, especially in patients who are poor surgical candidates. The Multicenter Intrapleural Sepsis Trial 2 (MIST-2) supports a six-dose regimen of intrapleural tissue plasminogen activator (tPA) plus dornase alfa (DNase), but the optimal duration of therapy in patients with persistent loculations remains uncertain. We describe a 62-year-old man with morbid obesity, New York Heart Association class III diastolic heart failure, and poorly controlled type 2 diabetes mellitus who presented

Bacteremia caused by Slackia exigua: A report of two cases and literature review.

Slackia exigua is an obligate anaerobic coccobacillus associated with dental infection, but rarely causes extraoral infection. We report two cases of monomicrobial bacteremia caused by S. exigua isolated from two institutions. The first case involved community-acquired bacteremia associated with pleural empyema in a 69-year-old man. The second case involved hospital-acquired bacteremia secondary to postoperative intra-abdominal abscess in a 73-year-old man with primary intestinal diffuse large B-cell lymphoma. S. exigua was finally identified by 16S ribosomal RNA gene sequencing analyses in bo

Isolated tricuspid valve endocarditis in nonaddicted patients: a diagnostic challenge.

Isolated native nonrheumatic tricuspid valve endocarditis rarely is described in the absence of intravenous drug use, intracardiac catheters, or cardiac anomalies. We diagnosed tricuspid valve endocarditis in two elderly nonaddicted patients with recurrent pulmonary infiltrates, anemia, and microscopic hematuria that occurred during several months and was caused by Gemella morbillorum and Candida glabrata, respectively. We have reviewed 27 other cases of nonaddicted patients with tricuspid valve endocarditis from the literature and discussed etiology, clinical characteristics, and outcome. Mea

Women's multisite microbial modulation during pregnancy.

The composition of female microbiome varies with age, physiological and socio-behavior conditions. Also, changes in microbiome composition are observed as pregnancy progresses, especially in the vaginal site. Together with the physiological adaptations of gestation, changes in microbiome composition seem to be fundamental for proper fetal development. This study aimed at simultaneously evaluating the vaginal, gut, and oral microbiome of healthy pregnant women, and comparing it with those observed in healthy non-pregnant women of reproductive age. In a cross-sectional study, vaginal, oral and g
