BACKGROUND: Gynecomastia and gender-affirming top surgery share similarities in objectives. Traditionally, a subcutaneous mastectomy using a semicircular incision has been recommended for most gynecomastia patients. In recent years, mastectomies and nipple grafting have become the most popular treatment for gender-affirming top surgery. Nipple grafting is very debilitating to this unique body part, which also provides erogenous sensation. This study was undertaken to compare methods, complications, and outcomes using an approach that preserves nipples in 100% of patients.

BACKGROUND: Despite high rates of online misinformation, transgender and gender diverse (TGD) patients frequently utilize online resources to identify suitable providers of gender-affirming surgical care.
AIM: The objective of this study was to analyze the webpages of United States academic plastic surgery programs for the types of gender-affirming surgery (GAS) procedures offered and to determine how this correlates with the presence of an institutional transgender health program and geographic region in order to identify potential gaps for improvement.
METHODS: Online institutional webpage

Gender-affirmation therapies for transgender and gender diverse (TGD) individuals has been increasing in numbers in recent years. TGD patients usually undergo various forms of hormonal replacement therapy (HRT) followed by bilateral orchiectomy. We aimed to characterize the histological and biochemical alterations in these specimens. From 2018-2023, 63 TGD individuals undergone gender-affirming orchiectomy. Atrophic changes, decreased spermatogenesis, and reduced mean tubule diameter with basement membrane thickening, as well as maturation arrest and aspermatogenesis were identified. A subset

BACKGROUND: Research on motivations and impact of gynaecological gender-affirming surgeries (GGS)-such as hysterectomy and oophorectomy-on transmasculine and gender diverse individuals remains limited.
AIM: This study aimed to evaluate patient-reported outcomes of GGS, focusing on motivations and the surgeries' effects on dysphoria and quality of life.
METHODS: A prospective cohort study was conducted in which transmasculine and gender diverse individuals undergoing GGS were included between August 2022 and February 2024. Preoperative and postoperative (3 and 9 months) data were collected th

BACKGROUND: The purpose of mastectomy for the transgender patient is to produce a masculine appearance of the chest. A number of algorithms have been proposed for selecting the surgical technique. A holistic and surgical approach to transgender men includes our experience-based classification system for selecting the correct surgical technique.
OBJECTIVES: To present and discuss the Transgender Standard of Care and our personal experience.
METHODS: Data were collected from the files of female-to-male transgender persons who underwent surgery during 2003-2019. Pictures of the patients were al

OBJECTIVE: To identify perioperative outcomes of transgender orchiectomy (TGO) and to broadly compare outcomes of TGO to cisgender orchiectomy (CGO) for nononcologic indications.
METHODS: Using the National Surgical Quality Improvement Program (NSQIP) database from 2010 to 2020, a retrospective study was performed on patients with ICD-9/10 codes for gender dysphoria, testicular torsion, and testicular pain who underwent simple orchiectomy. Demographics and surgical outcomes were summarized. Welch two-sample t test and chi-square test were used for group analysis. A trend analysis was performe

Transgender and nonbinary (TGNB) patients are a population with unique urologic needs that are often overlooked in urologic training. Most urology residents think that learning to take care of this population is important in their training; however, there is much variation in this educational content within urology residency programs. This review provides information on the current state of teaching regarding the unique needs of caring for TGNB patients as urologists and overviews some basic principles that every urologist should know.

Background: Gender-affirming facial surgery (GFS) is pursued by transgender individuals who desire facial features that better reflect their gender identity. Currently, there are a few objective guidelines to justify and facilitate effective surgical decision making. Objective: To quantify the effect of sex on adult facial size and shape through an analysis of three-dimensional (3D) facial surface images. Materials and Methods: Facial measurements were obtained by registering an atlas facial surface to 3D surface scans of 545 males and 1028 females older than 20 years of age. The differences b

Purpose: Gender dysphoria (GD) is an incompatibility between biological sex and personal gender identity; individuals harbor an unalterable conviction that they were born in the wrong body, which causes personal suffering. In this context, surgery is imperative to achieve a successful gender transition and plays a key role in alleviating the associated psychological discomfort. In the current study, a retrospective cohort, we report the 20-years outcomes of the gender-affirming surgery performed at a single Brazilian university center, examining demographic data, intra and postoperative compli

BACKGROUND: Masculinizing chest reconstruction is the most common gender-affirming surgery in transgender males. Despite the current literature's acknowledgment of the vital role that proper placement of the nipple-areola complex (NAC) plays in a masculine chest contour, there is still much debate regarding the best anatomical landmarks to achieve the desired result.
OBJECTIVES: The primary aim of this study is to determine which landmarks for NAC placement can be applied across diverse body types and aid surgeons in creating a masculine chest.
METHODS: Twenty-five formaldehyde-embalmed male

PURPOSE: Transgender and gender-diverse (TGD) people have increased cancer morbidity and mortality relative to cisgender individuals. Existing breast cancer prevention guidelines fail to adequately address gender-affirming care implications, especially in the context of gender-affirming mastectomies (top surgery). Evaluation of familial and germline genetic risk before top surgery is recommended by some transgender health guidelines. This evaluation can facilitate shared surgical decision making regarding breast tissue resection extent. In this study, we use in-depth interviews with TGD people

BACKGROUND: The transgender patient population is expanding, and gender affirming surgery (GAS) volume is increasing. Accurate, comprehensive, and easily navigable resources on GAS are lacking. We aim to evaluate the readability of online materials for specific gender affirming surgical procedures to identify mechanisms of improving information access for transgender patients.
MATERIALS AND METHODS: "Facial feminization", "facial masculinization", "MTF breast augmentation", "FTM chest masculinization", "MTF vaginoplasty", "metoidioplasty", and "FTM phalloplasty" were searched on Google. Per k

INTRODUCTION: Chest masculinization, or top surgery, is a commonly sought procedure for transgender men and gender-diverse individuals. This surgery typically involves the removal of breast tissue with reconstruction of the nipple-areola complex (NAC). However, sensory loss following surgery is common and may negatively impact postoperative satisfaction, intimacy, and sexual function. As a result, techniques for targeted nipple reinnervation (TNR) have gained increasing attention.
OBJECTIVES: To provide coverage of the current literature on TNR in gender-affirming chest surgery, including sur

AIM: To understand the preferences of individuals with gender incongruence (GI) regarding gender-affirming surgery (GAS), including gonadectomy.
METHODS: A prospective, multicenter survey was conducted in Japan targeting individuals aged 18 years or older with GI who had not yet undergone GAS. Participants completed a questionnaire about their GAS preferences, desired surgical procedures, the reasons for their preferences, and willingness to undergo gonadectomy even if it was not legally required.
RESULTS: In total, 107 participants (82 assigned female at birth [AFAB] and 25 assigned male at

INTRODUCTION: Chest masculinization is often the first operation requested by patients with female-to-male gender dysphoria. Despite increasing demand for this procedure, no recommendation of care is unanimous. We present our experience with the use of a simple and effective algorithm based on periareolar and subcutaneous mastectomy with free nipple graft techniques and compare complication rates between techniques.
AIMS: The main objective of this study was to compare the postoperative complications associated with these two techniques.
METHODS: Patients who underwent chest masculinization

The occurrence of phantom pain after surgical removal of male reproductive organs is a potential outcome. Indications for surgery may predict the probability of phantom pain after the procedure. These may vary between individuals undergoing tumor orchiectomy and those undergoing gender-affirming surgery. This study investigates the incidence of phantom pain after gender-affirming surgery. A retrospective questionnaire-based study was conducted with 55 transgender women who had undergone gender-affirming surgery, and the results were compared with those of 54 men who had undergone tumor orchiec

Genital gender-affirming surgery is a crucial yet often overlooked component of gender-affirming care for transgender individuals. Genital gender-affirming surgery is a rapidly evolving field, and it is not uncommon for surgeons - sometimes without prior experience in gender-affirming care - to encounter transgender individuals in clinical practice; hence, it is essential that all healthcare providers possess a foundational understanding of gender-affirming procedures and individual's needs.Research has shown that genital gender-affirming surgery significantly improves the quality of life for

INTRODUCTION: As we increasingly encounter transgender patients in the perioperative setting, it is important to be cognizant of appropriate medical management of the transgender patient. There is limited literature on the appropriate perioperative techniques to implement for adequate pain control in the transgender patient presenting for gender affirming procedures.
METHODS: In this scoping review, the authors identify publications that address perioperative pain control techniques that can be implemented by the anesthesiologist such as regional anesthesia, multimodal medications, or non-pha
