Genetics Navigator: protocol for a mixed methods randomized controlled trial evaluating a digital platform to deliver genomic services in Canadian pediatric and adult populations.

INTRODUCTION: Genetic testing is used across medical disciplines leading to unprecedented demand for genetic services. This has resulted in excessive waitlists and unsustainable pressure on the standard model of genetic healthcare. Alternative models are needed; e-health tools represent scalable and evidence-based solution. We aim to evaluate the effectiveness of the Genetics Navigator, an interactive patient-centred digital platform that supports the collection of medical and family history, provision of pregenetic and postgenetic counselling and return of genetic testing results across paedi

Lessons learned from testing cardiac channelopathy and cardiomyopathy genes in individuals who died suddenly: A two-year prospective study in a large medical examiner's office with an in-house molecular genetics laboratory and genetic counseling services.

This is a comprehensive review and analysis of 254 cases tested consecutively in the in-house College of American Pathologist-accredited molecular genetics laboratory within the New York City Office of Chief Medical Examiner between October 2015 and February 2018, using a multigene cardiac panel composed of 95 genes associated with cardiac channelopathy and cardiomyopathy. Demographics, autopsy findings, medical history, and postmortem genetic testing results were collected for each case. The majority of decedents were adults (>25 years old, 52.7%), followed by infants (<12 months, 25.6%), you

TestMiGenes: A hybrid effectiveness-implementation mixed methods study evaluating cancer genetic service delivery models in primary care at a federally qualified health center.

BACKGROUND: National guidelines recommend genetic testing for individuals at risk for hereditary cancer syndromes, but access to cancer genetic services (CGS) remains limited by persistent barriers. While implementation efforts have focused on oncology settings, primary care represents a critical entry point for expanding access, particularly among populations historically underserved in genomics. However, gaps exist in the development and evaluation of evidence-based CGS delivery models for primary care. Scalable approaches that address disparities while efficiently leveraging existing resour

Child With β-Thalassemia Major Born Following Hematopoietic Stem Cell Transplantation in the Mother: A Case Supporting the Expansion of Genetic Counseling Services in Lebanon and the Middle East.

We present a case of a woman who underwent a successful bone marrow transplant for beta-thalassemia major and later gave birth to a child who is compound heterozygous with HBB c.93-21G>A and HBB c.316-106C>G mutations to illustrate the critical gaps in reproductive risk assessment and genetic counseling. The prevalence of consanguinity, socioeconomic challenges, and the lack of a cohesive genetics health system in a thalassemia-endemic country all contribute to poor implementation of genetic care. Strengthening the education of healthcare practitioners, implementing centralized medical record

Shaping priorities in genetic medicine.

... Such moral problems raise the question whether society should make all or only some of the potential and existing genetic technologies widely available. Add to this the very real concern that the throng of products and services generated through this research has the potential to put significant further pressure on a health care system that already cannot provide adequate services to all persons. Given current restructuring of U.S. health services along the lines of managed health care, the need and desire of public and private parties to manage health resources aggressively by controlling

Genetic counselling and testing in Europe.

The Genetic Enquiry Centre in Manchester has designed a three-pronged health services research programme to address current issues in genetics. The issues are: whether doctors who are not trained in genetics can manage the genetic problems they meet in their practice; whether there are enough resources in specialist centres to cope with current and imminent referrals; and whether providers of primary care recognise genetic problems and refer patients appropriately. The three studies providing the basis of the programme--the National Confidential Enquiry into Counselling for Genetic Disorders,

Why Is Cancer Genetic Counseling Underutilized by Women Identified as at Risk for Hereditary Breast Cancer? Patient Perceptions of Barriers Following a Referral Letter.

Family history information comprises an important tool in identifying and referring patients at risk for hereditary breast and ovarian cancer (HBOC) to cancer genetic counseling. Despite recommendations and support provided by numerous professional organizations, cancer genetic counseling services are underutilized by atrisk patients. This study aimed to: (1) determine the rate of genetic counseling utilization following a referral letter, (2) characterize factors (barriers and supports) which influenced uptake of services, and (3) identify potential strategies for increasing utilization. Communities and community genetics in Ethiopia.

The rates of congenital and genetic disorders in low and middle income countries are similar or might be higher than in high income countries due to a multitude of risk factors and the dearth of community genetic services. To direct effective preventive, diagnostic and counseling services, collecting data on the incidence and prevalence of various congenital and genetic disorders and their risk factors is a pre-requisite for establishing genetic services at the community level and mainly at the primary health care setting. This brief review is meant to assess the available epidemiological data

The Paradox of Progress: Genetic Counselling in a South African Context and the Challenges of Workforce Retention.

This invited commentary explores the paradox of progress in genetic counselling in South Africa, where established training programmes and professional recognition have not yet translated into sustainable clinical genetic services. Drawing on workforce data and insights into training, employment and service delivery, the authors highlight how limited funded public-sector posts, geographic concentration of services and loss of professionals from patient-facing roles constrain equitable access to genetic counselling. The commentary aims to support coordinated workforce planning and investment to

Integrating hereditary breast and ovarian cancer genetic counselling and testing into mainstream clinical practice: Legal and ethical challenges.

Health professionals not specialized in genetics are expected to take an increasing role in genetic services delivery. This article aims to identify legal and ethical challenges related to a collaborative oncogenetics service model, where non-genetic health professionals provide genetic services to patients. Through a scoping literature review, we identified issues to the provision of hereditary breast and ovarian cancer, or other hereditary adult cancers, genetic testing under this model. Concerns that arose in the literature were informed consent, lack of adherence to best practice guideline

Genetic Testing Experiences of People Living with Inherited Retinal Degenerations: Results of a Global Survey.

INTRODUCTION: Obtaining a genetic diagnosis via genetic testing (GT) is a fundamental step in determining the eligibility of a patient to be enrolled in emerging clinical trials and research studies. Besides, the knowledge of genetic outcome allows patients to plan for significant life choices. However, critical barriers exist to an equitable access to genetic services globally. The objective of this study was to explore patient experiences while seeking genomic services for inherited retinal degenerations (IRDs). METHODS: An online survey was designed based on a focus group conducted by Reti

Interests and Experiences of Young Adults with Muscular Dystrophy in Receiving Genetic Information.

Growing public awareness of genetic research, increased availability of genetic testing options, and the recent arrival of several gene-targeted therapies have combined to make genetic testing a topic of interest within many disease communities, including muscular dystrophies. We sought to investigate the genetic information received, the genetic services provided, the genetic information and services of interest to young adults with muscular dystrophy, and to identify existing gaps in the delivery of genetic information and services. Young adults with muscular dystrophy were surveyed about th
