OBJECTIVES/HYPOTHESIS: Clinical decision making for facial nerve schwannoma is particularly complicated in patients with good facial nerve function; however, an early nerve-sparing tumor resection stripping technique minimizes facial deficits associated with treatment. The present study characterized the optimal candidate for this nerve-sparing surgical strategy in patients with good facial function.
STUDY DESIGN: Retrospective study.
METHODS: Nerve-sparing stripping surgery was performed on 28 patients with facial nerve schwannoma. The House-Brackmann grading system was used to assess pre-

Reactivation of the varicella-zoster virus (VZV) along the sensory nerves innervating the ear, including the geniculate ganglion, is responsible for herpes zoster oticus (HZO). In some cases, HZO is associated with polyneuropathy of the cranial nerves, although the mechanism of this involvement is not known. To explain this phenomenon and based on some clinical considerations, the present authors hypothesize an intersynaptic spread of VZV along the reflex pathways of the brainstem.

Bell's palsy, which is defined as idiopathic peripheral facial paralysis of sudden onset, accounts for > 50% of all cases of facial paralysis. Different theories on the etiology of Bell's palsy have been proposed and investigated. Various clinical studies have suggested an etiological link between Bell's palsy and herpes simplex virus (HSV). In addition, animal experiments have shown the ability of HSV to induce facial paralysis. In our opinion, the possible link between Bell's palsy and HSV can only be explored properly by studying the human facial nerve, and especially the geniculate ganglio

The chorda tympani taste nerve is prone to damage in humans. Chorda tympani damage results in taste loss accompanied by altered taste sensations, e.g., phantom tastes. To understand taste alterations this study explores the central and peripheral anatomical consequences of taste nerve injury in an animal model. The chorda tympani was severed in the middle ear of hamsters and the animals were allowed to survive for 2-161 days when sections of the brain were stained for degenerating axons with the Fink-Heimer method. Degenerating axons were present in the chorda tympani termination zone in the n

Greater petrosal and deep petrosal nerves unite in foramen lacerum to form vidian nerve which innervate lacrimal, buccal, nasopharynx, and nasal glands. Moreover, the superior part of this foramen transmits lacerum part of internal carotid artery which irrigates major part of brain. Change in the morphology of foramen lacerum may result in compression of neurovascular structures creating neurovascular complications which at times may be fatal/morbid. In addition to this, there is paucity of literature elucidating the morphology of foramen lacerum. So, the aim of the study is to describe the mo

The facial nerve schwannoma is a rare tumor and it seldom extends into the middle cranial fossa. The typical clinical presentations are progressive facial paralysis and hearing disturbance. We report here a case of huge facial nere facial nerve schwannoma extending into the middle cranial fossa without facial palsy. A 50-year-old man presented with left hearing disturbance. Neurological examination on admission revealed no deficits except for sensorineural hearing loss. MRI demonstrated a cystic tumor extending into the ddle cral fossa from the petrous bone CT the middle cranial fossa from the

The diagnosis and management of facial nerve disorders are challenging aspects of neurotological practice. In Bell's palsy and Herpes Zoster oticus facial nerve decompression is indicated in approximately 10 per cent of cases within the first three weeks if 90-95 per cent degeneration has occurred. The presence of a dry eye determines whether the lesion is proximal to the geniculate ganglion and a middle fossa approach is necessary. Injury to the facial nerve occurs most commonly as a result of temporal bone fractures, from either blunt trauma or penetrating wounds. Occasionally iatrogenic inj

Schwannomas of the facial nerve are rare slowly growing lesions that have a predilection for the geniculate ganglion. Radiological evaluation is important in their diagnosis and in the assessment of their extent. In our series of 4 cases the facial nerve schwannoma was seen on high resolution CT as a soft tissue mass bounded anteriorly by a thin rim of bone. This bony crescent sign is a previously undescribed sign of facial nerve schwannoma which appears on the basis of this small series to be strongly indicative of the presence of this tumour. Schwannomas are relatively uncommon intracranial

Reactivation of herpes simplex virus type 1 (HSV-1) in the vestibular ganglion (VG) is the suspected cause of vestibular neuritis (VN). Recent studies reported the presence of HSV-1 DNA not only in human VGs but also in vestibular nuclei, a finding that indicates the possibility of viral migration to the human vestibular labyrinth. Distribution of HSV-1 DNA was determined in geniculate ganglia, VGs, semicircular canals, and macula organs of 21 randomly obtained human temporal bones by nested PCR. Viral DNA was detected in 48% of the labyrinths, 62% of the VGs, and 57% of the geniculate ganglia

A 39-year-old female patient with rheumatoid arthritis developed Ramsay Hunt syndrome after infliximab treatment. This condition is caused by the reactivation of varicella zoster virus infection in the geniculate ganglion of facial nerve in the host's immunosuppression. She was treated immediately with valaciclovir and hydrocortisone, and the complete recovery was achieved at 6 months after the onset. This is the first report of Ramsay Hunt syndrome as an adverse effect of infliximab in rheumatoid arthritis.

Postcontrast T1-weighted MR images in a patient with Ramsay Hunt syndrome showed an enhancing lesion in the region of the nucleus of the pontine facial nerve and abnormal enhancement of the intrameatal, labyrinthine, and tympanic facial nerve segments and of the geniculate ganglion, as well as enhancement of the vestibulocochlear nerve and parts of the membranous labyrinth. This enhancement most probably resulted from a primary neuritis of the intrameatal nerve trunks of the seventh and eighth cranial nerves.

After injury to the intracranial portion of the facial nerve, undegenerated myelinated nerve fibres were seen to form a distinct bundle in the peripheral part of the transverse fascicular area at the level distal to the geniculate ganglion. The numbers and diameters of the fibres were similar to those seen in the nervous intermedius portion of the facial nerve in experimental animals. The average number of these apparently non-motor fibres was 2500. Their average diameter was 2.4 micrometer.

PURPOSE: This study is to evaluate the duration of facial nerve enhancement in gadolinium-enhanced temporal bone MRI after the onset of acute facial palsy.
METHODS: Gd-enhanced MRI imagines were examined in 13 patients with idiopathic acute facial palsy within 14 days after the onset. The degree of facial nerve function was measured according to the House-Brackmann (H-B) grading system at their first visit at outpatient clinic. The follow-up MRI was taken about 16.5 months (7-24 months) after onset of disease. The degree of facial nerve enhancement was measured with signal intensity (SI) whic

OBJECTIVES/HYPOTHESIS: Surgical approaches to the lateral skull base, internal auditory canal (IAC), and petrous bone are widely known and have been extensively recorded. Despite the benign nature and limited dimensions of lesions located in this anatomical region, extirpative surgical approaches are often required to reach and remove the disease. The aim of present report was to describe our initial experiences with minimally invasive endoscopic approaches to the lateral skull base.
STUDY DESIGN: Retrospective review of patients' charts and video recordings from surgery.
METHODS: Twelve pat

INTRODUCTION: The visual signals evoked at the retinal ganglion cells are modified and modulated by various synaptic inputs that impinge on lateral geniculate nucleus cells before they are sent to the cortex. The selectivity of geniculate inputs for clustering or forming microcircuits on discrete dendritic segments of geniculate cell types may provide the structural basis for network properties of the geniculate circuitry and differential signal processing through the parallel pathways of vision. In our study, we aimed to reveal the patterns of input selectivity on morphologically discernable

Choristomas, masses of normal tissues in aberrant locations, contain smooth muscle fibers and fibrous tissues. We describe the MR imaging features of two choristomas located in the internal auditory canals and arising from the facial and vestibulocochlear nerves. Both lesions enhanced with contrast material. In one case, enhancement was seen in the geniculate ganglion and greater superficial petrosal nerve. In the other, a medial component enhanced less than the lateral component did.

In this study we have examined the trajectories taken by populations of ganglion cell axons and the spatial gradients of terminal arbor maturity within the lateral geniculate nucleus (LGN) during the prenatal development of the cat's visual system. To do so, an in vitro method of labeling optic tract axons from fetal brains between embryonic day 37 (E37) and postnatal day 2 (P2) with horseradish peroxidase (HRP) was used. At the earliest ages studied (E37-E53), optic axons leave the optic tract to run across the LGN toward their sites of termination in straight trajectories parallel to each ot

Neurons of the dorsal lateral geniculate nucleus (LGNd) in the microphthalmic rat were examined by the Golgi-Cox method. LGNd neurons in the microphthalmic rat were classified into the multipolar (I) and bipolar (II) types as in the normal rat. The multipolar type was further divided into two subclasses (Ia and Ib) on the basis of their dendritic patterns. The proximal portion of their primary dendrites was thinner than in normal LGNd neurons. The Ia cells had 6-7 primary dendrites extending radially, while the Ib cells had 3-4 primary dendrites spreading primarily parallel to the optic tract.

The temporal bones of a 24-day-old female neonate with an interstitial deletion of the long arm of chromosome 2 [del(2)(q31q33)] were studied histopathologically, focusing mainly on the inner ear. This is, to our knowledge, the first report of a temporal bone study in a case of this chromosomal aberration. The abnormalities included a shortened cochlea with underdeveloped modiolus in both ears. Total absence of the spiral ganglion cells and the cochlear nerve bundle, accompanied by obliteration of the fundus of the internal auditory meatus by a bony plate, and dislocation of some geniculate ga
