The clinical care of frail, older adults.

Frailty and its management represent an emerging area of clinical care in older adults. Geriatricians have long recognized a syndrome of multiple comorbid conditions, immobility, weakness, and poor tolerance of physiologic stressors in older adults. Patients with these characteristics are described as frail and suffer increased adverse clinical outcomes. This article reviews the clinical spectrum of frailty in older adults, its biologic etiology, and potential clinical interventions. Several operational definitions of frailty and the associated clinical signs, symptoms, and outcomes are outlin

Headache in the elderly.

Headache in an elderly patient can be a sign of serious, potentially life-threatening disorders. All patients require a full assessment, including a complete neurologic examination. Particular emphasis should be placed on excluding subarachnoid hemorrhage, subdural hematoma, giant cell arteritis, intracranial neoplasm, cerebrovascular accident, acute-angle-closure glaucoma, and infectious etiologies such as meningitis and encephalitis. Once life-threatening disorders are excluded, the geriatrician can focus on more benign etiologies such as migraine, tension headache, and medication withdrawal

Multidisciplinary approach for the management of post-herpetic neuralgia in elderly patients.

Post-herpetic neuralgia is associated with significant distress and morbidity. The management of acute neuritis and/or post-herpetic neuralgia can be particularly difficult. A multidisciplinary approach is required. A team consisting of the primary care physician, pain specialist, neurologist, geriatrician, pain psychologist, psychiatrist, and a physiatrist with an integrated approach will provide the best results. Early interventional therapy with sympathetic nerve blocks may significantly decrease the need for long-term opioid therapy, as well as long-term use of anticonvulsants, antidepress

Pregabalin Induced Maculopapular Eruption in an Elderly Male.

BACKGROUND: Pregabalin is used in the treatment of neuropathic pain of various etiologies and as an adjuvant in epilepsy. Blockade of the α2δ subunit of L and N-type Ca-channels is its main mechanism of neurotropic action. Compared to other antiepileptics like phenytoin, valproate and lamotrigine, and other neuropathic pain medications such as amitriptyline and duloxetine, pregabalin has a relatively favorable safety profile and hence is a drug of choice for many geriatricians. CASE PRESENTATION: Here we describe a case of maculopapular rash induced by pregabalin in an older man, which resolv

Validation of a Large Language Model Enhanced Frailty Index.

BACKGROUND: Efficient, validated frailty screening tools are needed to identify high-risk older adults. The electronic frailty index (eFI), derived from diagnostic codes, may under-capture functional deficits, such as those documented in physical therapy (PT) notes. OBJECTIVE: This study evaluated the performance of a standard eFI and an enhanced eFI— augmented with deficits extracted from PT notes using a large language model (LLM)—against a geriatrician-performed frailty index based on a comprehensive geriatric assessment (FI-CGA). DESIGN: Retrospective cross-sectional study. PARTICIPANTS: A

Better outcomes for the older surgical patient trial (BOOST): protocol for a hybrid type 1 effectiveness-implementation trial of an embedded geriatric perioperative service within emergency and elective surgery in an Australian tertiary referral hospital.

INTRODUCTION: Older patients admitted under surgical care have longer length of stay (LOS) and are at risk of functional decline, hospital-acquired complications and geriatric syndromes. Embedded specialist geriatrician models within surgical care teams can reduce length of stay and perioperative complications. Evidence gaps remain regarding the implementation of these models of care and their impact on patient outcomes. This study aims to measure hospital, patient and implementation outcomes of an embedded perioperative geriatric service in a large Australian tertiary referral hospital. METH

[Value of coordinated geriatric and cardiological expertise in elderly patients' eligibility for percutaneous aortic valve replacement].

BACKGROUND: Transcatheter aortic valve implantation (TAVI) is the gold standard treatment for aortic stenosis in the elderly. Pre-identification of patients likely to benefit from this procedure remains crucial. A standardised geriatric assessment is used to identify the major geriatric syndromes likely to influence postoperative outcomes. OBJECTIVE: To identify factors associated with lack of TAVI management and to compare one-year survival in TAVI vs. non-TAVI patients. METHODS: Retrospective study, between 2016 and 2020, at the Arras hospital. Patients aged 70 years and older with symptom

Qualitative Evaluation of a Pilot Physician Training Program in Long-Term Care Facilities in Switzerland.

Increasing demand for long-term care facilities (LTCFs) and increasingly complex medical needs of LTCF residents necessitate recruiting new physician graduates and developing programs which provide them with experience and specific skills in geriatric and palliative medicine. We developed an educational program for physicians in training, which combines 1 year of immersion at 20% full-time equivalent with (1) theoretical teaching, (2) clinical exposure under the supervision of a senior LTCF physician, and (3) structured case presentations and debriefing with experienced geriatricians. [Femoral neck fracture].

FEMORAL NECK FRACTURE. Femoral neck fractures represent the third most common type of fracture in traumatology. The diagnosis is based on the clinic and radiographs of the pelvis and the hip. The choice of treatment depends on the type of fracture (with displacement in coxa vara, coxa valga, or without displacement), the age of the patient, his autonomy and his co-morbidities. In elderly patients with a displaced fracture in coxa vara, arthroplasty will be preferred, whereas for non-displaced or coxa-valga fractures, as well as in younger patients, osteosynthesis will be performed. Cross-sectoral follow-up after hospital discharge from a geriatric ward: a study protocol of a randomised controlled trial (G-UD).

BACKGROUND: Advancing age is associated with multimorbidity, polypharmacy and functional impairments and older adults with these characteristics are at increased risk of acute admission and readmission. Readmission rate of geriatric patients is high, especially within the first week after discharge. Previous studies on transitional care interventions have mainly showed positive effects, but no study has assessed the effect of a home visit done jointly by a geriatric nurse and a community nurse, with a systematic review of health functions, objective clinical assessment and bedside blood analys

A prospective multi-site cohort study on the prevalence of frailty in patients aged over 70 years presenting after serious injury and implications for outcomes.

INTRODUCTION: Major Trauma Hospitals are receiving increasing numbers of older patients after serious injury. Outcomes in these patients vary with the nature of the injury and other factors such as frailty. We aimed to determine the prevalence of frailty and adverse events in older patients managed by acute trauma services during the index hospital admission, and the frequency of adverse outcomes at three, six and twelve months after discharge in an Australian setting. METHODOLOGY: This study assessed the prevalence of frailty in a prospective multicentre cohort study of seriously injured pat

Diagnoses, problems and healthcare interventions amongst older people with an unscheduled hospital admission who have concurrent mental health problems: a prevalence study.

BACKGROUND: Frail older people with mental health problems including delirium, dementia and depression are often admitted to general hospitals. However, hospital admission may cause distress, and can be associated with complications. Some commentators suggest that their healthcare needs could be better met elsewhere. METHODS: We studied consecutive patients aged 70 or older admitted for emergency medical or trauma care to an 1800 bed general hospital which provided sole emergency medical and trauma services for its local population. Patients were screened for mental health problems, and those

Expert Evaluation of the Perceived Accuracy, Relevance, and Safety of Large Language Model-Generated Patient Information in Geriatrics: Cross-Condition Study.

BACKGROUND: Large language models (LLMs) are increasingly used to generate patient-oriented medical information. In geriatrics, such information must balance accuracy, relevance, and safety, as older adults may be particularly susceptible to misleading or harmful advice. However, systematic evaluations of expert perceptions across multiple geriatric conditions remain limited. OBJECTIVE: This study aimed to explore geriatricians' perceptions of the accuracy, relevance, and potential harm of LLM-generated patient information across common geriatric conditions and to examine variability and inte

Which Comprehensive Geriatric Assessment (CGA) instruments are currently used in Germany: a survey.

BACKGROUND: The Comprehensive Geriatric Assessment (CGA) records geriatric syndromes in a standardized manner, allowing individualized treatment tailored to the patient's needs and resources. Its use has shown a beneficial effect on the functional outcome and survival of geriatric patients. A recently published German S1 guideline for level 2 CGA provides recommendations for the use of a broad variety of different assessment instruments for each geriatric syndrome. However, the actual use of assessment instruments in routine geriatric clinical practice and its consistency with the guideline an

Cataract Surgery in Cognitive Impairment: Navigating the Accessibility Challenge in European Healthcare.

This narrative review examines the challenges of providing cataract surgery to people living with cognitive impairment within European healthcare systems. It highlights the significant unmet need for improved ophthalmological management of this vulnerable population, given the established link between visual impairment, cognitive decline, and quality of life. While the benefits of cataract surgery in improving vision and cognition are well-documented, significant inconsistencies exist across European healthcare systems regarding protocols and access. The review examines the relationship betwee

[The « syndrome de glissement »].

The "syndrome de glissement" The "syndrome de glissement » is a controversial notion that is still used by some French geriatricians, although it does not belong to any international disease classification and has very few evidence-based data. Some authors found clinical relevance to this notion, whereas others believe its use is potentially associated with certain risks. The clinical practice and the few available evidence related to syndrome de glissement" tend to suggest that this syndrome usually belong to other disorders including apathy, depression and delusional disorder.
