Cheek tooth extraction in geriatric versus non-geriatric horses.

BACKGROUND: Age-related dental changes are major contributors to dental disorders in older horses, and cheek tooth extractions are often required to maintain masticatory function and oral comfort. However, limited information is available on clinical and treatment characteristics in this older patient population. OBJECTIVE: To describe clinical features, treatments, complication rates and short-term outcomes of horses undergoing oral cheek tooth extraction and treatment of odontogenic sinusitis, and to compare findings between geriatric and non-geriatric horses. STUDY DESIGN: Retrospective a

Impact of the comprehensive geriatric assessment on treatment decision in geriatric oncology.

BackgroundThe comprehensive geriatric assessment (CGA) is the gold standard in geriatric oncology to identify patients at high risk of adverse outcomes and optimize cancer and overall management. Many studies have demonstrated that CGA could modify oncologic treatment decision. However, there is little knowledge on which domains of the CGA are associated with this change. Moreover, the impact of frailty and physical performance on change in cancer treatment plan has been rarely assessed.MethodsThis is a cross-sectional study of older patients with solid or hematologic cancer referred by oncolo

Geriatric Fever Score: a new decision rule for geriatric care.

BACKGROUND: Evaluating geriatric patients with fever is time-consuming and challenging. We investigated independent mortality predictors of geriatric patients with fever and developed a prediction rule for emergency care, critical care, and geriatric care physicians to classify patients into mortality risk and disposition groups. MATERIALS AND METHODS: Consecutive geriatric patients (≥65 years old) visiting the emergency department (ED) of a university-affiliated medical center between June 1 and July 21, 2010, were enrolled when they met the criteria of fever: a tympanic temperature ≥37.2°C

Acute Care Group-Based Physical Therapy on an Inpatient Geriatrics Unit: Preliminary Data.

BACKGROUND: Hospitalized older adults are at high risk for hospital-associated disability, but one-to-one physical therapy (PT) models often cannot meet demand for skilled mobility progression. Group-based PT is effective in outpatient and rehabilitation settings, but its feasibility in acute inpatient care is unknown. METHODS: We conducted a single-site, prospective quality improvement implementation of acute care group-based PT (AC-GBPT) on a 39-bed medical-surgical geriatrics unit at an academic medical center. Sessions were offered weekly during routine clinical operations without additio

American Geriatrics Society position statement: Making medical treatment decisions for unrepresented older adults.

This paper is an official position statement of the American Geriatrics Society (AGS) and updates the 2017 AGS position statement, Making Medical Treatment Decisions for Unbefriended Older Adults. In this updated position statement, the term "unbefriended" is replaced by "unrepresented" as a term that is more value-neutral, more accurately describes the circumstance in which a person without medical decision-making capacity does not have recognized surrogate representation, and better aligns with increasingly preferred terminology as reflected in recent medical literature. We define unrepresen

Use of a nursing-led geriatrics consult service to deliver age-friendly care.

The Elder Veteran Program (EVP) is a nursing-led approach to deliver inpatient consultative geriatrics care at our academic Midwestern Veterans Hospital. From April to December of 2021, EVP modified its workflow using a Plan-Do-Study-Act approach to include previously under-addressed components of the IHI's "4M's" of Age-Friendly Care (Medication, Mobility, Mentation, and What Matters), with three months of retrospective data review as a Plan phase, three months of monthly Do and Study phases, and a three month Act phase to analyze post-intervention care. We found improvements in frequency of

Toward the realization of a better aged society: messages from gerontology and geriatrics.

UNLABELLED: 1. BACKGROUND: Recent medical advancements, and improvements in hygiene and food supply have led to Japan having the longest life expectancy in the world. Over the past 50 years, the percentage of the elderly population has increased fourfold from 5.7% in 1960 to 23.1% in 2010. This change has occurred at the fastest rate in the world. Compared with France, where the percentage of the elderly population has increased just twofold in the past 100 years, Japanese society is aging at an unprecedented rate. In addition, the percentage of the very elderly (aged 75 years and over), comp

Palliative care module within a required geriatrics clerkship: taking advantage of existing partnerships.

OBJECTIVE: LCME has recently required that all graduating medical students learn about end-of-life care. This program describes the design and integration into an existing geriatrics clerkship of a palliative care module that teaches the foundations of end-of-life and palliative care to medical students. DESCRIPTION: Faculty experts in geriatrics met during a series of weekly meetings in 1999 to design a mandatory four-week-long clinical clerkship in geriatrics. Since the palliative care program is based within the geriatrics department, faculty members with interest and expertise in both ger

[Respiratory medicine in geriatrics. A cultural niche of relevant theoretical and practical importance].

Respiratory medicine in the elderly represents a topic of increasing interest in the developed countries, because of its peculiar clinical and therapeutical implications. In this respect, a deeper knowledge of the biology and physiology of aging lung is desirable: namely, the age-related immunological and structural changes involving the respiratory system do influence clinical presentation and response to the treatment in older people. A better understanding of the pharmacology in the elderly which is recognized to be different for several aspects from that of the general population, is manda

[Would artificial intelligence pass a Geriatrics exam?: evaluating conversational responses and their accuracy].

OBJECTIVE: To evaluate the accuracy of six different artificial intelligence (AI) models in answering questions from Spain's MIR (Médico Interno Residente) exam related to Geriatrics. METHODS: The performance of six AI models was analyzed by comparing their conversational responses with the official answer keys of the MIR exam. The accuracy of each model was measured based on the number of correct answers obtained for the Geriatrics questions. Due to the sample size, a descriptive analysis of the accuracy percentages was performed. The questions were grouped into specific thematic categories,

Impact of inpatient geriatrics consultation on hospital outcomes in older adults with trauma.

BACKGROUND: Older adults presenting with trauma have worse outcomes than younger adults. Starting in 2016, we provided geriatrics consultation (GC) to older adults admitted to the trauma service. We aimed to analyze the impact of GC on patient outcomes. METHODS: We performed a retrospective pre-post study and year-matched cohort study. We identified patients from the trauma registry at our level 1 trauma center. In the pre-post study, we compared patients who received GC (2016-2022) with controls (2011-2015). In the cohort study (2016-2022), we compared patients who received GC with controls.

Common gastrointestinal drug-drug interactions in geriatrics and the importance of careful planning.

INTRODUCTION: Polypharmacy, which uses multiple medications to treat chronic illnesses, is common among elderly patients. However, it can lead to drug interactions, especially with gastrointestinal (GI) medicines that are extensively used. These drug interactions can have severe consequences and pose a significant challenge to healthcare providers. Therefore, it is crucial to identify the underlying mechanisms of these interactions and develop strategies to minimize medication errors. AREAS COVERED: We analyzed databases on GI illnesses common in older adults, including GERD, peptic ulcer dis

[Emergency medicine after a catastrophic disaster: from a view of geriatrics and gerontology].

The megathrust earthquake and the towering tsunami hit the east coast of Japan on March 11th of 2011 after intervals of 1,142 years. About 90 % of nearly 20,000 victims were drowned in devastating waves, while every town and city along the coast turned out to be a ruin. Over 400,000 people were forced to move to the evacuation centers where the evacuees slept on the floor without electricity, running water or heating systems at freezing nights. Emergency medicine, therefore, was more required during the evacuation phase than during the acute phase of the tsunami disaster. Here discussed is the
