A human complementary DNA whose protein product is considered to be the major component of scrapie-associated fibrils in Creutzfeldt-Jakob disease, kuru, and Gerstmann-Straussler syndrome has been identified and characterized. The extensive homology of this gene sequence to the hamster PrP 27- to 30-kilodalton prion protein complementary DNA clone, and its existence as a single copy in the human genome, leads to the conclusion that this is the human prion gene. This human prion gene has been mapped to human chromosome 20, negating a direct link between the prion protein and Down's syndrome or

From Creutzfeldt-Jakob disease (CJD) to variant CJD through Gerstmann-Sträussler-Scheinker syndrome, kuru and fatal familial insomnia, the journey leading to current understanding of the basic aspects of human prion diseases has been full of unexpected, but often dramatic and always fascinating twists. Recent progress in modeling prion diseases and characterization of the various prion protein forms reveal that such a wide spectrum of the diseases is associated with the chameleon-like conformational features of prions.

A common presentation of inherited prion disease is Gerstmann-Sträussler-Scheinker syndrome, typically presenting with gait ataxia and painful dysaesthesiae in the legs evolving over 2-5 years. The most frequent molecular genetic diagnosis is a P102L mutation of the prion protein gene (PRNP). There is no explanation for why this clinical syndrome is so distinct from Creutzfeldt-Jakob disease, and biomarkers of the early stages of disease have not been developed. Here we aimed, first, at determining if quantitative neurophysiological assessments could predict clinical diagnosis or disability an

Lexical agraphia is thought to result from impairment of the lexical spelling system and is characterized by better spelling of nonwords and regularly spelled words than irregularly spelled words. Previous reports have suggested a localization of this syndrome in the region of the left posterior-superior angular gyrus and parietal-occipital lobule, sparing the supramarginal gyrus. We describe a right-handed man who after open-heart surgery displayed lexical agraphia without aphasia, alexia, apraxia, or Gerstmann's syndrome. Computed tomography disclosed a lesion not on the left but on the righ

In this study we investigated the acalculic condition of a patient, C.G., with the classical signs of Gerstmann's Syndrome: finger agnosia; right-left disorientation; a profound agraphia (but with an equally profound alexia) and a remarkably dense acalculia. Using a series of number processing and number knowledge tasks, a selective impairment for numbers was demonstrated. Within the category of numbers C.G. showed a largely preserved ability to deal with numbers below 4, in all tasks and in all modalities, while she was totally unable to deal with numbers above 4. The consistency of responses

Amyloid fibrils in brains of patients with Alzheimer's disease and Down's syndrome were examined by light and electron microscopy. In addition, replicas of amyloid fibrils produced by a quick freezing method from the brain of a patient with Down's syndrome were examined by electron microscopy. The amyloid fibrils were shown to consist of hollow rods. These were composed of filaments arranged as a tightly coiled helix, each turn of which consisted of five globular subunits. This structure appears to be similar to the prion filament observed in Creutzfeldt-Jakob disease (CJD). The possibility th

A large family with autosomal dominant segregation of presenile dementia, and other neurological and behavioural features is described. At various times, family members have carried diagnoses of Alzheimer's disease, Huntington's disease, Parkinson's disease, myoclonic epilepsy, atypical dementia, Pick's disease, Creutzfeldt-Jakob disease and Gerstmann-Sträussler syndrome. Molecular genetic studies have enabled classification of this disease at the molecular level as one of the group of inherited prion diseases. Here we describe the phenotype of inherited prion disease (PrP 144 bp insertion).

Pharmacological treatment with polyanions or amphotericin B in hamsters with experimental scrapie reveals that it is possible to delay the appearance of the disease only when the drug is given before the invasion of the agent into the clinical target areas of the brain. We suggest such early treatment may be possible for individuals at high risk of acquiring the disease, such as healthy mutation-positive relatives of patients with familial Creutzfeldt-Jakob disease or Gerstmann-Sträussler syndrome, or recipients of potentially contaminated pituitary-extracted human growth hormone.

BACKGROUND: Many patients with early-onset Alzheimer's disease (EOAD; age of onset <65 years) have non-amnestic presentations involving language (logopenic primary progressive aphasia, lvPPA), visuospatial abilities (posterior cortical atrophy, PCA), and even asymmetric symptoms consistent with corticobasal syndrome (CBS). An inferior parietal lobule variant of EOAD commonly presents with progressive difficulty with calculations.
METHODS: We reviewed 276 EOAD patients for presentations with predominant acalculia. These patients were diagnosed with clinically probable Alzheimer's disease (AD)

We present two cases of progressive early-onset dementia with apraxia and visuospatial disability as initial manifestations. In the later stages of the illness Gerstmann's and Balint's syndromes developed. Structural neuroimaging demonstrated parieto-occipital atrophy and functional imaging revealed bilateral hypometabolism and hypoperfusion in these areas. These cases fulfil the diagnostic criteria of posterior cortical atrophy (PCA). Frontal lobe involvement became evident as the disease progressed. Alzheimer's disease also typically features this anterior spread and possibly this is the und

Prion diseases are a phenotypically diverse set of disorders characterized by protease-resistant abnormally shaped proteins known as prions. There are three main groups of prion diseases, termed sporadic (Creutzfeldt-Jakob disease [CJD], sporadic fatal insomnia, and variably protease-sensitive prionopathy), genetic (genetic CJD, fatal familial insomnia, and Gerstmann-Straussler-Scheinker syndrome), and acquired (kuru, variant CJD, and iatrogenic CJD). This article will review the pathophysiology, genetics, clinical presentations, and diagnostic challenges in patients with prion disease. Case d

Australia-wide prospective surveillance of human transmissible spongiform encephalopathies (TSEs) has been conducted by the Australian National Creutzfeldt-Jakob Disease Registry (ANCJDR) since October 1993. In addition, the Registry retrospectively ascertained TSE cases within Australia from 1970. Referrals of all suspect cases of human prion diseases or TSEs are investigated by the ANCJDR and include Creutzfeldt-Jakob disease (CJD), Gerstmann-Sträussler-Scheinker syndrome, fatal familial insomnia and variant CJD. This semi-annual progressive update presents epidemiological findings of the AN

This paper reviews current data on prions, their complex molecular biology, especially in relation to the actual discussions on bovine spongiform encephalopathy and it possibly being connected with development of certain diseases in humans. We discussed various forms of Creutzfeldt-Jakob disease (classical, iatrogenic, atypical forms) in detail, but also other diseases connected with pathogenic effects of prions (Gerstmann-Straussler-Scheinker Syndrome (GSS), fatal familial insomnia etc.). This report describes clinical characteristics and diagnostic possibilities discussing their epidemiology

Recently there has been achieved a substantial progress in research concerning pathogenesis of subacute spongiform viral encephalopathies. For this group of disorders, there has been found a common unconventional virus - proteinaceous infectious particle (PrP). There has been recognised a molecular mass, biochemical composition and physicochemical properties of this infectious form. Mutations of scrapie amyloid precursor gene discovered in Gerstmann-Sträussler-Scheinker syndrome (GSS) and familial cases of Creutzfeldt-Jakob disease (CJD) have been revealed. Nowadays examinations in the field o

At age 54, a patient presented Wernicke's aphasia and a Gerstmann's syndrome with right hemianopia. CT scan showed a low density area in the left hemisphere in the posterior parts of T1 and T2 and parts of T3, extending into the gyrus angularis and gyrus supramarginalis. Wernicke's aphasia, digital agnosia and confusion between left and right hand improved six months later, but agraphia remained total and calculation disorders, both for mental and written calculation, were still very severe. Operation processes were retained. A dramatic improvement of performances could be obtained with writte

Besides conventional slow viruses, a category of pathogens exist, called "unconventional slow viruses". The latter evoke laughing death syndrome, Creutzfeldt-Jakob disease (resp. its special form of Gerstmann-Sträussler's syndrome) in man, in animal scrapie, infectious encephalopathy of the mink, and chronic wasting disease. They all are relatives, and their characteristics are longtime incubation, lacking immune reactions, high chemical and physical resistance, no proof of nucleic acids till now, a lacking proof in electron microscopy as well as a ceasing change of infectivity by immunologic

Neuropathological diagnostic criteria for Creutzfeldt-Jakob disease (CJD) and other human transmissible spongiform encephalopathies (prion diseases) are proposed for the following disease entities: CJD--sporadic, iatrogenic (recognised risk) or familial (same disease in 1st degree relative): spongiform encephalopathy in cerebral and/or cerebellar cortex and/or subcortical grey matter; or encephalopathy with prion protein (PrP) immunoreactivity (plaque and/or diffuse synaptic and/or patchy/perivacuolar types). Based on internal medicine and psychiatry and in close connection with pathology, the neurosciences in Austria began to develop in the 18(th) century, e.g. with the description of inflammation of the central nervous system by J. P. Franck (1745-1823) and the "phrenology" by F. J. Gall (1745-1823). Under the influence of the great pathologist C. Rokitansky (1804-1878), the tripode of the Vienna neurology - L. Türck (1810-1868), as initiator, Th. v. Meynert (1833-1892) the activator, and H. Obersteiner (1847-1922) as the founder of the Vienna Neurological Institute, presented basic contributions

This review addresses recent research on social and nonverbal learning disabilities. Involvement of right hemispheric dysfunction in these disabilities has been hypothesized, as studies with adults have suggested that documented right hemisphere damage may lead to deficits in social skills, prosody, spatial orientation, problem solving, and recognition of nonverbal cues. Studies of children purported to evidence nonverbal learning disabilities are reviewed and compared with the results from studies of adults with right hemisphere damage. Specific subtypes of nonverbal learning disabilities are
