Analysis of speech and tongue motion in normal and post-glossectomy speaker using cine MRI.

OBJECTIVE: Since the tongue is the oral structure responsible for mastication, pronunciation, and swallowing functions, patients who undergo glossectomy can be affected in various aspects of these functions. The vowel /i/ uses the tongue shape, whereas /u/ uses tongue and lip shapes. The purpose of this study is to investigate the morphological changes of the tongue and the adaptation of pronunciation using cine MRI for speech of patients who undergo glossectomy. MATERIAL AND METHODS: Twenty-three controls (11 males and 12 females) and 13 patients (eight males and five females) volunteered to

Rehabilitation and nutritional support for sarcopenic dysphagia and tongue atrophy after glossectomy: A case report.

Swallowing dysfunction is related to long-term weight loss and reduced body mass index in patients with head and neck cancer. We describe a 76-y-old woman who had severe sarcopenic dysphagia and atrophy of the reconstructed tongue for 17 mo after subtotal glossectomy due to tongue cancer and lost 14 kg during that period. Upon admission, the patient received diagnoses of malnutrition in the context of social or environmental circumstances with insufficient energy intake, loss of muscle mass, localized fluid accumulation, weight loss, and sarcopenia due to reduced skeletal muscle mass (skeletal

Glossectomy as an adjunct to correct an open-bite malocclusion with shortened maxillary central incisor roots.

A young man, 19 years of age, with the chief complaint of an anterior open bite, came for orthodontic treatment with a skeletal Class I relationship, anterior open bite, shortened maxillary incisor roots, and relative macroglossia. The malocclusion was treated by extracting the maxillary first premolars and using a fixed edgewise appliance. A partial glossectomy was performed before the orthognathic surgery with a 3-piece segmental LeFort I mandibular setback, and advancement was achieved with a reduction genioplasty. A functional and esthetic occlusion with an improved facial profile was esta

Serial suture ligation glossectomy for squamous cell carcinoma in the setting of a massive lymphovascular malformation.

BACKGROUND: We describe a novel surgical technique for resection of squamous cell carcinoma in the setting of a massive tongue lymphovascular malformation (LVM). METHODS AND RESULTS: A 75-year-old man with a history of long-standing massive LVM of the tongue presented with a 4-cm squamous cell carcinoma of the involved oral tongue. Given the aberrant vascular anatomy of this lesion, traditional surgical treatment could not be undertaken. Therefore, serial suture ligation was performed over a 2-week period, allowing complete excision of the squamous cell carcinoma and a large portion of the to

[Psychological responses of patients after partial or total glossectomy due to carcinoma of the oral cavity].

The necessity of partial or total glossectomy due to carcinoma of the oral cavity means that the patient suffers from severe and life-threatening disease and the medical team aims at eliminating that threat. The major goal of surgical intervention is to stop the growth of the cancer. However, it can be associated with considerable injury of the ill person. After the operation, the patient goes through the rehabilitation process during which he or she may experience suffering and various limitations resulting from injury. The patient also tries to adjust to the new situation in life. It is the

A functional speech impression used to fabricate a maxillary speech prosthesis for a partial glossectomy patient.

Surgery for cancer of the floor of the mouth often results in alteration of the muscles of the tongue and floor of the mouth. Both primary and secondary surgical procedures often result in scar formation with reduced mobility of the tongue during speech and deglutition. Speech is often used as a diagnostic tool in the placement of the anterior teeth during fabrication of a prosthesis. Speech can similarly be used to help determine the proper placement of a speech portion of the prosthesis. The prosthetic rehabilitation approach described lowers the palatal vault with a false palate to enable t

Salvage total glossectomy and total glosso-laryngectomy: Are they worth it? A GETTEC French multicenter study.

BACKGROUND: Salvage total glossectomy (TG) or total glosso-laryngectomy (TGL) remain controversial, as highly morbid procedures. The objective was to describe oncological and functional outcomes after salvage TG or TGL. METHODS: We performed a multicenter retrospective study, including patients with previous neck irradiation undergoing TG or TGL for squamous cell carcinoma involving the base of tongue. RESULTS: We included 42 patients: 27 in the TG group and 15 in the TGL group. For the entire cohort, median OS and DFS were estimated at 19 months (95% IC [14-44]) and 10 months (95% IC [7-13]

Complexity of vocal tract shaping in glossectomy patients and typical speakers: A principal component analysis.

The glossectomy procedure, involving surgical resection of cancerous lingual tissue, has long been observed to affect speech production. This study aims to quantitatively index and compare complexity of vocal tract shaping due to lingual movement in individuals who have undergone glossectomy and typical speakers using real-time magnetic resonance imaging data and Principal Component Analysis. The data reveal that (i) the type of glossectomy undergone largely predicts the patterns in vocal tract shaping observed, (ii) gross forward and backward motion of the tongue body accounts for more change

Prosthodontic rehabilitation for total glossectomy with a magnetic detachable mandibular tongue prosthesis: a clinical report.

Total glossectomy can result in significant functional impairments in mastication, swallowing, and speech. In addition to these functional problems, severe psychological problems may follow complete loss of the tongue. Placement of a mandibular tongue prosthesis obturates this large defect, increases the patient's ability to produce intelligible sounds, and assists with a return to a normal diet. Prosthetic rehabilitation can also improve the user's appearance and psychosocial adjustment. This clinical report describes a magnetically attached two-piece tongue prosthesis used to treat a patient

The efficacy of palatal augmentation prostheses for speech and swallowing in patients undergoing glossectomy: a review of the literature.

This article provides a review of the literature to assist the clinician in determining the efficacy of palatal augmentation prosthesis regarding speech and swallowing for the patient undergoing glossectomy. A MEDLINE search was conducted. Peer-reviewed articles published from 1966 to July 2002 that addressed the question of the efficacy of this prosthesis to improve speech and swallowing after partial or total glossectomy were included. Studies reviewed had to incorporate an objective evaluation of one or both functions. Nine of 130 studies met the selection criteria (4 retrospective, 1 case

Investigating Neotongue Atrophy and Postoperative Outcomes in Patients Undergoing Glossectomy With Free Flap Tongue Reconstruction.

OBJECTIVE: Following glossectomy, tongue reconstruction via autologous free flap can reapproximate native form and function, significantly improving postoperative quality of life. Various studies have examined functional outcomes between the commonly employed radial forearm free flap (RFFF) and the anterolateral thigh free flap (ALTFF); however, changes in neotongue volumetric bulk are less clearly understood. STUDY DESIGN: We aimed to assess postoperative outcomes and volume changes in patients undergoing glossectomy. SETTING: This is a retrospective chart review of patients who underwent g

Using decalcified collagen scaffolds to improve healing after transoral partial glossectomy for squamous cell carcinoma.

INTRODUCTION: Transoral partial glossectomy procedures for small squamous cell carcinomas of the anterior tongue often leave significant defects which are challenging to reconstruct. Post-operative scarring leads to adhesions, reduced tongue mobility and limitations of function. We present a series of cases where a decalcified collagen scaffold was successfully used in this challenging reconstructive setting. METHODS: After standard oncological diagnostic and workup, five patients underwent transoral partial glossectomy procedures. Decalcified collagen scaffolds were used to reconstruct the s
