Glucocorticoids and the regulation of growth hormone secretion.

Glucocorticoids modulate the secretion of growth hormone (GH) by various and competing effects on the hypothalamus and pituitary gland. The final effects of this modulation depend on hormone concentrations and the duration of exposure. The traditional hypothesis is that chronically raised levels of glucocorticoids suppress the secretion of GH. However, a functional impairment of the GH reserve might also be observed in patients with low levels of glucocorticoids, such as those with secondary hypoadrenalism, which is consistent with the model of biphasic dose-dependent effects of glucocorticoid

Exogenous glucocorticoids and adverse cerebral effects in children.

Glucocorticoids are commonly used in treatment of paediatric diseases, but evidence of associated adverse cerebral effects is accumulating. The various pharmacokinetic profiles of the exogenous glucocorticoids and the changes in pharmacodynamics during childhood, result in different exposure of nervous tissue to exogenous glucocorticoids. Glucocorticoids activate two types of intracellular receptors, the mineralocorticoid receptor and the glucocorticoid receptor. The two receptors differ in cerebral distribution, affinity and effects. Exogenous glucocorticoids favor activation of the glucocort

[Reactivation of hepatitis B infection due to glucocorticoids].

A 55-year-old female started treatment with prednisone for peripheral facial nerve paralysis. After three months she was admitted to hospital with suspected cholecystitis. However, while in hospital her liver function deteriorated. Virological tests showed she had a hepatitis B infection. Another patient, a 71-year-old male, was treated with prednisone for polymyalgia rheumatica. A few weeks after starting therapy he was admitted to hospital with general malaise and weight loss. Laboratory results showed elevated values of liver function tests. Viral serology was positive for hepatitis B infec

Management of glucocorticoids-induced osteoporosis: role of teriparatide.

Glucocorticoids (GC)-induced osteoporosis (GIOP) is the most common cause of secondary osteoporosis, which leads to an increased fracture risk in patients. The normal bone turnover depends on a balance between osteoblasts and osteoclasts activity and GC can cause a rapid bone loss, decreasing bone formation and increasing bone resorption. The decreased bone formation is mainly due to the GC-induced apoptosis of both osteoblasts and osteocytes, while the increased bone resorption is due to the increased life-span of pre-existing osteoclasts. Bisphosphonates are clearly effective in preventing a

[Disease-modifying effects of glucocorticoids in rheumatoid arthritis].

Drugs used for managing rheumatoid arthritis (RA) are designated disease-modifying antirheumatic drugs (DMARDs) if they reduce inflammation and pain, limit joint destruction, and improve long-term disease outcome. Glucocorticoids have long been known to have anti-inflammatory, immunosuppressive, and pain-reducing effects. Moreover, they have been shown in recent clinical trials, and also very recently in a systemic analysis of the results of these studies, to contribute to inhibition of the radiographic progression of RA. For these reasons, glucocorticoids can be considered DMARDs if they are

[Psychological stress in oncology: the role of glucocorticoids].

During the last years, the correlations between biological processes, psychological adjustment and stress disorders have received increasing attention and a growing body of research results has been published in the general literature. In the realm of psycho-oncology, however, conceptual models on this topic and studies aimed at their validation have remained relatively scanty. On the basis of our observations and available literature in the field of post-traumatic and depressive stress disorders in oncology, we have proposed to apply the concept of allostatic load to the study and understandi

Can posttraumatic stress disorder be prevented with glucocorticoids?

Patients with critical illness who are treated in an intensive care unit (ICU) often report traumatic memories from ICU treatment, receive exogenously administered glucocorticoids for medical reasons, and have a relatively high incidence of chronic stress symptoms and posttraumatic stress disorder (PTSD) during follow-up. ICU therapy could therefore represent a useful model for investigating glucocorticoid effects on traumatic memories and PTSD development. Studies in long-term survivors of ICU treatment demonstrated a clear and vivid recall of different categories of traumatic memory such as

Glucocorticoids influence brain glycogen levels during sleep deprivation.

Animal studies indicate that opioids inhibit the firing rate of vestibular neurons, which are important in mediating the vestibulosympathetic reflex. Furthermore, this inhibition appears to be greater in more mature rats. In the present study, we tested the hypotheses that opioids inhibit the vestibulosympathetic reflex in humans and that endogenous opioids contribute to the age-related impairment of the vestibulosympathetic reflex. These hypotheses were tested by measuring muscle sympathetic nerve activity (MSNA), arterial blood pressure, and heart rate responses to otolith organ engagement d

Glucocorticoids in the medical management of Graves' ophthalmopathy.

Graves' ophthalmopathy (GO) is a challenging disease that poses therapeutic dilemmas for endocrinologists and ophthalmologists. Progress in medical therapy has been slow. Numerous new treatments have been reported to have a beneficial effect. However, current evidence indicates that glucocorticoids (GC) represent the only class of drug therapy which either in isolation or combined with other therapies has an unequivocal role in the routine clinical management of patients with GO. Recent advances include better definition of factors predictive of response to treatment and the formulation of spe

Severe Trichinellosis cured with pulse doses of glucocorticoids.

Trichinellosis is a worldwide zoonotic disease caused by a nematode Trichinella spiralis. We studied a case of Trichinella spiralis infection with severe eye involvement, febrile condition, generalised malaise and muscular weakness in a young female patient. Comprehensive ophthalmologic, infectologic, neurological and immunologic examinations including electro diagnostic tests and CT scan of the head were performed, but the diagnosis was confirmed only by histological examination of biopsy specimens of skeletal muscle. The patient did not respond to standard corticosteroid therapy and improved

Differential effects of glucocorticoids on human auditory perception.

It is assumed that glucocorticoids and mineralocorticoids, the hormones produced by the adrenal cortex, act as modulators of central nervous functions in addition to their well-known role in endocrine stress responses. More specifically, it has been suggested that adrenal corticoids cause changes in sensory thresholds. In two double-blind crossover studies we evaluated auditory perception in healthy volunteers after treatment with hydrocortisone, dexamethasone, and the respective placebos. Stapedial reflexes, pure tone and speech audiograms, and brainstem electric response audiometry served as

Receptor for glucocorticoids in RPMI 3460 melanoma cells.

We are investigating the role of steroid hormones in the biological behavior of malignant melanoma. We have recently shown that the growth of RPMI 3460 Syrian hamster melanoma cells in monolayer culture is retarded by the synthetic glucocorticoid dexamethasone. This paper reports the identification of a macromolecule with properties of a glucocorticoid receptor in cytosis prepared from these cells. Sucrose density gradient analyses on six separate cell batches consistently demonstrated substantial levels of saturable [3H]dexamethasone binding [concentration = 195 +/- 47 (S.E.) fmol/mg cytosol

Mouse lymphoma cells: mechanisms of resistance to glucocorticoids.

S49, an established line of mouse lymphoma cells, has several characteristics useful for the genetic analysis of glucocorticoid action: (1) a stable pseudodiploid karyotype; (2) an efficient cytolytic effect of glucocorticoids, which appears to follow the same biochemical pathway as steroid hormone action in other systems; (3) appearance of rare steroid-insensitive clones that exhibit stable, heritable resistance to further glucocorticoid treatment; (4) rapid growth in suspension culture and high cloning efficiency in soft agar, allowing facile isolation of variant clones. Variable efficacy of glucocorticoids in congenital adrenal hyperplasia.

We have examined the suppression of urinary pregnanetriol and 17-ketosteroids during treatment with cortisol, cortisone, prednisone, and dexamethasone in eight patients with congenital adrenal hyperplasia. A large individual variation in response to each agent was observed. In some individuals, cortisone is less effective than its generally accepted potency would indicate. At equivalent glucocorticoid dosage, dexamethasone was twice as effective as the other steroids in suppressing urinary 17-ketosteroids and pregnanetriol. The potency of dexamethasone in suppressing adrenal function was 80 ti

Revolutionary Changes in the Management of Lupus Nephritis: Towards De-Glucocorticoid or No-Glucocorticoid.

Glucocorticoids (GCs) possess potent anti-inflammatory and immunosuppressive properties and are used to treat various diseases, including systemic autoimmune rheumatic diseases, rheumatoid arthritis, and systemic lupus erythematosus (SLE). However, GCs are associated with several adverse events and are considered risk factors for infections and cardiovascular disorders; furthermore, their application as therapeutics has changed with recent progress in molecular-targeted therapies. Although GCs have been the mainstay of SLE treatment for more than 50 years, the latest European Alliance of Assoc
