Human urinary gnathostomiasis: a case report from Thailand.

Acase of urinary gnathostomiasis in a 32-year-old Thai woman, caused by an adult male Gnathostoma spinigerum, is described. The patient who had suffered from intermittent, subcutaneous gnathostomialis for about 10 years, developed pneumonia of the right lower lung followed by swelling on the right hypochondrium, and paresis of both legs accompanied by perianal numbness and retention of urine, and it seems reasonable to assume that the parasite migrated to the lung, hypochondrium, and the cauda equina, respectively. She later passed the worm, which we beleive problably came from the bladder. To

Gnathostomiasis: an emerging infectious disease relevant to all dermatologists.

Gnathostomiasis is a parasitic infection caused by the third larval stage of nematodes of the genus Gnathostoma. The disease is endemic in some countries around the world. In the American continent, the majority of cases is concentrated in Mexico, Ecuador, and Peru. However, due to increasing traveling either at the intercontinental or intracontinental level, the disease is seen each time more frequently in tourists. Furthermore, countries, such as Brazil, that have never been considered endemic are reporting autochthonous cases. The disease usually presents as a deep-seated or slightly superf

Current status of gnathostomiasis dorolesi in Miyazaki Prefecture, Japan.

Gnathostomiasis is an important food-borne parasitic zoonosis caused mainly by ingesting uncooked or undercooked flesh of freshwater fishes. Although four distinct species of the genus Gnathostoma were identified as the causative agents for human gnathostomiasis, human infections with G. doloresi have been found only in Japan, concentrated in Miyazaki Prefecture. So far we have found 25 cases in Miyazaki Prefecture. Although most of these patients were of cutaneous gnathostomiasis, two patients presented to the hospital with unusual clinical manifestations; one case was a pulmonary gnathostomi

Gnathostomiasis: shifting epidemiology and persistent challenges in diagnosis and treatment.

PURPOSE OF REVIEW: Gnathostomiasis is a neglected parasitic infection traditionally endemic in Southeast Asia and Latin America. However, in recent years, the disease has been increasingly reported beyond its traditional endemic regions. This review examines recent evidence on its epidemiology and discusses the challenges in diagnosis and management. RECENT FINDINGS: Gnathostomiasis is now frequently reported in a broader global context, reflecting changing patterns of human behavior and global travel. Transmission is still associated with behaviors, such as the consumption of raw or undercoo

Susceptibility-weighted imaging is helpful in diagnosis of cerebral gnathostomiasis.

PURPOSE: To describe the role of SWI compared with other MR imaging sequences and CT in diagnosis of cerebral gnathostomiasis. MATERIALS AND METHODS: CTs and MRIs of patients with cerebral gnathostomiasis were retrospectively reviewed. The types of intracranial hemorrhage, including intraparenchymal hemorrhage (IPH), subdural hemorrhage (SDH), subarachnoid hemorrhage (SAH), and their locations were recorded. RESULTS: Four patients proven as cerebral gnathostomiasis were included. Intracranial hemorrhage was detected in all patients. There was IPH in all patients, SAH in 2 patients, and SDH i

Proteomics of Gnathostomiasis: A Way Forward for Diagnosis and Treatment Development.

Gnathostoma spinigerum is the most common cause of gnathostomiasis in humans. It has a complex life cycle, which requires two intermediate hosts and a definitive host, and poses a high risk for zoonosis. Definitive prognosis of gnathostomiasis relies mainly on the isolation of advanced-stage larvae (aL3), which is very challenging especially if the aL3 is sequestered in difficult-to-reach organs. There is also a lack of a confirmatory diagnostic test for gnathostomiasis. With the ongoing advancement of proteomics, a potential diagnostic approach is underway using immunoproteomics and immunodia

Specific antigen of Gnathostoma spinigerum for immunodiagnosis of human gnathostomiasis.

Sera from four patients with parasitologically confirmed gnathostomiasis, 15 patients with presumptive gnathostomiasis, 64 patients with various parasitic infections and 19 healthy adults were studied by sodium dodecyl sulphate-polyacrylamide gel electrophoresis (SDS-PAGE) and Western blot analysis for their reactivities against somatic extract of Gnathostoma spinigerum third-stage larvae (L3). It was found that the L3 extract was highly complex consisting of more than 20 antigenic components, a few of which gave reactions with sera from the healthy controls. Extensive cross-reactions of the p

Intracameral gnathostomiasis: a cause of anterior uveitis and secondary glaucoma.

Recognition of parasitic diseases by Western physicians is becoming increasingly important because of increased international travel and the recent influx of refugees from South east Asia. We describe two patients who presented with acute anterior uveitis and secondary glaucoma caused by intracameral third stage larvae of Gnathostoma spinigerum. The parasites were successfully removed, with preservation of good visual acuity. The initial infection may occur through consumption of contaminated water and/or raw or undercooked foods, while the eyes may be involved even years later. Accurate diagn

Intraocular gnathostomiasis: A rare case report from Central India.

We report the first case of intraocular gnathostomiasis from Central India. A 29-year-old male from Indore, Madhya Pradesh, presented with pain and redness of the right eye since 1 month. Slit lamp examination revealed anterior uveitis, multiple iris atrophic patches, and a live worm hooked on iris. The worm was removed through a small sclerocorneal tunnel. Microscopy confirmed Gnathostoma spinigerum. The patient was treated with oral albendazole and steroids. The case is reported because of its rarity.

Thoracic Myelopathy Due to Gnathostomiasis Acquired in New Zealand.

Gnathostomiasis is a zoonotic disease endemic in Asia. It most commonly manifests as gastrointestinal and cutaneous disease. Central nervous system involvement is a rare but feared complication, often leaving patients with permanent neurologic deficits. Clinicians outside of Asia and Latin America may have little experience with this illness, causing delays in diagnosis and treatment. We describe a 40-year-old woman who developed a progressive myelopathy over 18 months. She had never traveled outside of New Zealand. Cerebrospinal fluid (CSF) showed marked eosinophilia and Gnathostoma serology

Human gnathostomiasis: infection with an immature male Gnathostoma spinigerum.

Gnathostomiasis, usually caused by larvae of Gnathostoma spinigerum, is a significant cause of morbidity in the Far East. We report a case of gnathostomiasis involving a male Thai residing in Kuwait who presented with acute pain in the right iliac fossa. A resection of the terminal ileum and cecum was performed. Microscopic examination showed a parasite in an eosinophilic abscess. The nematode was identified as an immature male G. spinigerum based on size, morphology, and distribution of cuticular spines.

Double-dose ivermectin vs albendazole for the treatment of gnathostomiasis.

A comparative study was performed for the treatment of gnathostomiasis patients with ivermectin 0.2 mg/kg for 2 days in 15 patients vs albendazole 400 mg twice daily for 21 days in 14 patients. The ivermectin and albendazole gave cure rates of 100% and 78.5%, respectively, however the difference was not statistically significant between the two drugs (Fisher's exact, p=0.0996). One year after treatment, the patients who had no migratory swellings and a drop in ELISA titers or a negative immunoblot test were considered to be cured. The side effect of ivermectin for two days was dizziness. The s

Acute outbreak of gnathostomiasis in a fishing community in Sinaloa, Mexico.

An increasing number of human cases of gnathostomiasis have been reported in Sinaloa, Mexico, most of whom have a custom of eating of raw fish dishes such as 'cebiche'. Here we report five adult patients, three women and two men, having an acute episode of vomiting and abdominal pain a few minutes after eating a dish of cebiche prepared from a spotted sleeper perch (Eleotris picta) fished from a nearby lake in southern Sinaloa. All five patients experienced acute throat pain, chest and joint pains, headache and fever. One patient, a 55-year-old male, was hospitalized with suspected pancreatiti

A case of gnathostomiasis in a European traveller returning from Mexico.

Hundreds of human cases of gnathostomiasis have recently been reported from Mexico, where the disease is becoming a public health problem. We report a case of gnathostomiasis in a French tourist returning from Mexico. Tourists travelling in endemic countries are at risk of gnathostomiasis and should be advised about the risks of eating raw fish as a suspected source of infection.
