Metabolic and anatomic thyroid emergencies: a review.

PURPOSE: To review the diagnosis and management of thyroid diseases, both metabolic (thyroid storm and myxedema coma) and anatomic (substernal goiter, lymphoma, anaplastic carcinoma) that may require intensive care therapy. DATA SOURCES: English-language articles were identified through a search of the MEDLINE and Index Medicus databases. Bibliographies of retrieved articles were examined for relevant articles. STUDY SELECTION: Approximately 250 articles were reviewed. Those articles deemed most representative were utilized. Case reports were included to highlight rare, but potentially letha

[Total thyroidectomy technique: suggestions and proposals of surgical practice].

Total thyroidectomy at present depicts a diffuse surgical procedure in the management of benign and malignant disease of thyroid gland. It is followed by a low incidence of iatrogenic damages (nervous lesions or permanent hypoparathyroidism), just like subtotal thyroidectomy and lower than surgery for nodular recurrences. Authors present the surgical technique they follow to perform total thyroidectomy, used in over 400 cases of benign thyroid diseases operated since 1986. The most important points of this surgical procedures are represented by exposure and sparing of inferior laryngeal nerve

Chorea due to hyperthyroidism in old age. A case report.

The authors discuss the case of an elderly female patient with thyrotoxicosis and chorea. T3 and T4 serum values were high. Since the chorea disappeared with euthyroidism but reappeared whenever there was a further increase in T3 and T4, its relationship on hyperthyroidism and its reversibility are clearly demonstrated. The chorea therefore appears to be another clinical condition present in hyperthyroidism.

A huge forgotten retrosternal goiter causing a superior vena cava syndrome after 20 years of thyroidectomy: A case report.

INTRODUCTION: Retrosternal goiters are thyroid enlargements that extend into the mediastinum and may compress adjacent structures. Although uncommon, residual thyroid tissue after total thyroidectomy can remain asymptomatic for years and present later with serious complications such as superior vena cava (SVC) syndrome. CASE PRESENTATION: A 51-year-old woman presented with progressive facial swelling, neck vein distension, and dyspnea. She had undergone total thyroidectomy 20 years prior for multinodular goiter. Physical examination revealed facial plethora and a positive Pemberton's sign. CT

How to Do Thoracoscopy-Assisted Thyroidectomy for Retro Sternal Goitre.

Extra-cervical approach to remove substernal goitre is by sternotomy or thoracotomy. The morbidity associated with open procedures can be minimized by a thoracoscopic approach. It can be performed with less blood loss and less time duration, with the resected specimen retrieved through the neck incision.

Retrosternal goiter.

INTRODUCTION: Retrosternal goiter is an enlarged thyroid mass of which more than 50% is located in the mediastinum. Indications for surgery of retrosternal goiter include ineffective pharmacological treatment, mechanical syndrome, suspicion of malignancy and thyrotoxicosis. Computed tomography is the gold standard in the diagnosis of retrosternal goiter. The surgery can be performed from a cervical incision, using sternotomy or thoracotomy. METHODS: The aim of our study was to evaluate our own group of retrosternal goiters. In 2011-2019, 1739 thyroid surgeries were performed at the 3rd Depart

Surgical aspects of 175 mediastinal goiters.

OBJECTIVE: Clinical picture and surgical management of 175 mediastinal goiters are discussed in this retrospective study. METHOD: Between 1979 and 1996, 175 patients with intrathoracic goiters were operated on at the Thoracic Surgical Clinic in Budapest. The majority of the goiters were cervicomediastinal (n = 138, 79%), past the level of aortic arch, and the others were complete aberrant lesions (n = 37, 21 %). Of the patients, 40% (n = 70) were symptom-free, in the others the clinical picture was dominated by compressive symptoms, among them, in five instances, the initial false, long-lasti

Substernal goitre presenting with upper and lower extremity oedema.

Substernal goitre is characterised by compressive symptoms of the airway and oesophagus. Chronic, progressive symptoms usually result in surgical removal. We report a rare presentation of substernal goitre in a male in his early 70s who suffered from severe bilateral lower extremity (LE) lymphoedema, resulting in immobility and nursing home placement, and left upper extremity lymphoedema. Our initial assessment led to a filariasis work-up, which was negative, due to the patient's prior 2-year residence in India and service overseas. Chest CT scan revealed an incidental substernal goitre extend

[Asthma mimic - a clinical case report].

Most mediastinal goiters are extensions of cervical goiters. Patients are generally female and only occasionally have symptoms. Patients most commonly complain of a mass-like sensation, dyspnoea, dysphagia and cough. The authors describe the case of a 67 year-old female who presented at the emergency room with dyspnoea, anterior cervical discomfort and tightness which had onset a few months prior but which had worsened in the last week. The patient cited a history of bronchial asthma, for which she was under medication, but the true diagnosis was mediastinal goiter.

Retrosternal goiter associated with chylothorax.

We describe a case of chylothorax in a female patient with a large retrosternal goiter. After subtotal thyroidectomy by means of a Kocher cervicotomy, the chylothorax disappeared in 48 h. At present, 2 years after surgical removal, the patient remains asymptomatic, without recurrent pleural chylous effusion. Until now, to our knowledge, only one such case had been reported.

[Surgical treatment of intrathoracic struma].

Surgical treatment was applied to 18 patients for intrathoracic struma. Dyspnoea, dysphagia, recurrent palsy, and dilatated cervical veins with facial flushing were indications for surgery. Surgical access routes depended on localisation of the intrathoracic struma and its connection to the thyroid gland. Goitre located in the anterior mediastinum (substernal) can be extirpated, using the cervical approach (Kocher). Sternotomy was found to be necessary only in cases with tracheal resection. Goitres located in the posterior mediastinum were removed by means of right or left thoracotomy. Postope

Follicular thyroid carcinoma and mediastinal goiter in a case of situs inversus viscerum.

A 55-year-old woman with total situs inversus viscerum was subjected to total thyroidectomy for a well-differentiated follicular thyroid carcinoma. After surgery the patient remained euthyroid for two months and serum TSH did not rise. A whole body scan with iodine-131 (131I) showed mediastinal uptake. A chest computed tomography visualized a sequestered substernal goiter. Upon the patient's refusal to undergo surgery, she was treated with 150 mCi of 131I. A further dose of 100 mCi of 131I was administered six months later. Ten years after treatment, the patient is still alive and disease-free

Evaluation of multinodular goiter and primary hyperparathyroidism leads to a diagnosis of AL amyloidosis.

BACKGROUND: Amyloid goiter, defined as excess amyloid within the thyroid gland in such quantities that it produces a clinically apparent goiter, is a very rare manifestation of systemic amyloidosis with cases commonly seen in the setting of Amyloid A (AA) amyloidosis. Amyloid goiter as the primary clinical manifestation secondary to Amyloid light chain (AL) amyloidosis is very rare. We present a case of AL amyloidosis with initial manifestation as goiter with amyloid deposition in the thyroid and the parathyroid gland. CASE PRESENTATION: A 73 year old male presented with goiter and compressiv

To explore the risk factors and preventive measures affecting the treatment of retrosternal goiter: An observational study.

The definition of substernal goiter (SG) is based on variable criteria, leading to considerable variations in the reported incidence (from 0.2% to 45%). The peri- and postoperative complications are higher in total thyroidectomy (TT) for SG than that for cervical goiter. This study aimed to evaluate the preoperative risk factors associated with postoperative complications. From 2002 to 2014, 142 (8.5%; 98 women and 44 men) of the 1690 patients who underwent TT had a SG. We retrospectively evaluated the following parameters: sex, age, histology, pre- and retro-vascular position, recurrence, and

[Risk factors in recurrent nerve paralysis: a statistical analysis of 7566 cases of struma surgery].

Between 1979-1988, 7566 operations for benign diseases of the thyroid gland were performed. The frequency of the paresis of the recurrent nerve examined between the third and fifth postoperative day was 5.2% for patients and 3.2% for resected lobes. The incidence was correlated to performed operative procedure (enucleation, subtotal resection, lobectomy) to size (weight of specimen) and to the expansion of the goiter (substernal, intrathoracic, retrotracheal) as well as to sex. These factors influence the risk of injury with high statistic significance. The side of resection and the ligature o

Device implantation complicated by a retrosternal goiter.

Central venous obstruction in the cardiac implantable electronic devices (CIED) population is commonly due to thrombosis and fibrosis secondary to the passage of pre-existing leads. However, vein occlusion before CIED implantation is uncommon, and one cause is retrosternal goiters. We report a case where the failure of the initial implantation of a primary CIED led to an unusual implantation route without goiter excision. The patient had an indication for cardiac resynchronization therapy (CRT) given his left ventricular (LV) function was impaired and had second-degree heart block Mobitz Type
