Functional reconstruction of the upper and lower lips and commissure with a forearm flap combined with a free gracilis muscle transfer.

After resection of an arterio-venous malformation of the upper and lower lips and commissure we performed reconstruction with a forearm flap combined with a free gracilis muscle transfer. First the motor nerve of the gracilis muscle was anastomsed to a buccal nerve branch in the cheek. In a second operation, the red lip was reconstructed with an oral mucosal graft, and the upper lip skin was reconstructed with a local flap. The patient obtained good oral sphincter function for eating, speaking and air inflation.

Influence of angiotensin II, alpha- and beta-adrenoceptors on peripheral noradrenergic neurotransmission in canine gracilis muscle in vivo.

Interactions between angiotensin II and adrenoceptor-mediated effects on peripheral sympathetic neurotransmission were investigated in constant flow blood-perfused canine gracilis muscle in situ, without and with pretreatment by non-competitive alpha-adrenoceptor blockade. Angiotensin converting enzyme (ACE)-inhibition by benazeprilat increased nerve stimulation (2 Hz, 4 min)-evoked noradrenaline (NA) overflow (+ 21 +/- 5%) with alpha-adrenoceptors intact, but reduced NA overflow (- 18 +/- 6%) when alpha-adrenoceptors were blocked. Vasoconstrictor responses were slightly reduced by benazeprila

Use of the Adductor Magnus Muscle Free Flap for Lower-Extremity Soft Tissue Coverage: An Alternative to the Gracilis Muscle Flap With Unfavorable Vascular Anatomy.

BACKGROUND: The gracilis muscle free flap has proven to be a dependable tool for the reconstructive microsurgeon for coverage of small- to medium-sized lower-extremity soft tissue defects because of its reliable anatomy, ease of elevation, and minimal donor site morbidity. We describe our experience with the adductor magnus muscle free flap which we have utilized in cases when confronted with the rare situation in which the dominant vascular pedicle to the gracilis muscle is insufficient or absent. METHODS: We conducted a retrospective chart review of a single surgeon's experience (EG Melissi

Multivector gracilis muscle transfer for smile reanimation in long-standing facial paralysis: Simultaneous upper and lower lip construction.

BACKGROUND: Traditional facial reanimation with gracilis muscle transfer effectively restores oral commissure excursion but often fails to correct lower lip asymmetry, resulting in compromised full-dental smiles and perioral dysfunction. This study evaluated a multivector gracilis transfer designed to simultaneously reanimate both the upper and lower lips. METHODS: We retrospectively analyzed 29 patients with long-standing unilateral facial paralysis who underwent multivector gracilis free muscle transfer between 2020 and 2024. The gracilis was tailored into a forked configuration, with slips

Gracilis Muscle Interposition to Fill the Perianal Dead Space May Decrease Hospital Length of Stay in Fournier's Gangrene.

PURPOSE: The treatment of Fournier's gangrene (FG) includes aggressive debridement of the affected necrotic area, broad-spectrum antibiotic therapy, and reconstructive procedures, respectively. One of the main reasons of unfavorable outcomes in FG surgery is that the dead space occurs in the perianal region because of destruction of fascias and soft tissues. In this study, we aimed to evaluate the results of gracilis muscle flap transposition to fill the FG-associated perianal dead spaces. METHODS: Patients treated for FG-associated dead spaces in their perianal region between the years 2017

Clinical outcome and quality of life after gracilis muscle transposition for fistula closure over a 10-year period.

PURPOSE: Gracilis muscle transposition (GMT) is an established surgical technique in the treatment of anorectal fistulas and fistulas to the vagina and the urinary system when previous closure options have failed. There is little evidence on the success rate of this procedure in the long term. METHODS: This is a follow-up study on all patients undergoing GMT over a 10-year period at a tertiary referral center for complex fistulas. Postoperative function and quality of life were evaluated by standardized questionnaires (Wexner score, Fecal Incontinence Quality of Life Score (FIQL), SF-12 and a

Facial animation with gracilis muscle transplant reinnervated via cross-face graft: Does it change patients' quality of life?

PURPOSE: Gracilis muscle reinnervated by the contralateral facial nerve via cross-graft technique is nowadays considered to be a first-line procedure for facial animation in unilateral palsies. Despite the wide number of papers published analyzing technical aspects, refinements, functional results, and cosmetic outcomes, only a few authors have focused their publications on the patient's perspective and impact on QOL of these procedures. MATERIAL AND METHODS: Changes in quality of life in 42 patients treated with gracilis muscle transplant reinnervated via cross-face graft were analyzed throu

A Case of Achieving Urinary Continence Using Gracilis Muscle Flap for Recanalization and Fistula After Bladder Neck Closure.

INTRODUCTION: We present the case of a patient who achieved urinary continence after undergoing a gracilis muscle flap (GMF) for recanalization and fistula after bladder neck closure (BNC). CASE PRESENTATION: A male patient with neurogenic lower urinary tract dysfunction complained of difficulty inserting the catheter because of numerous false cavities in the prostatic urethra. We performed BNC to create a continent catheterizable stoma. However, the bladder neck was recanalized in the early postoperative period. Next, we dissected and closed the urethra transperineally. Due to the tiny fistu

Sensory Recovery After Free Muscle Flap Reconstruction-A Clinical Study of Protective and Discriminative Function of Free Gracilis and Latissimus Dorsi Muscle Flaps Without Neurotization.

BACKGROUND/OBJECTIVES: Free gracilis (GM) and latissimus dorsi muscle (LDM) flaps are reliable options for complex defect coverage, but long-term sensory outcomes remain underexplored. Sensory impairment, especially the loss of protective cutaneous sensation, increases the risk of injury, thermal damage, and ulceration in reconstructed areas. This study aimed to systematically assess multidimensional sensory recovery after free muscle flap (FMF) reconstruction. METHODS: In a prospective single-center study, 94 patients (49 GM, 45 LDM) underwent standardized sensory testing following FMF trans

Long-term outcomes of dissection of the depressor labii inferioris muscle to enhance symmetry after free functional gracilis muscle transfer in patients with facial paralysis.

BACKGROUND: In unilateral facial paralysis, overactivity of the contralateral depressor labii inferioris muscle (DLI) often contributes to persistent lower lip asymmetry after free functional gracilis transfer (FFGT). This issue can be addressed through dissection of DLI. We objectively evaluated the long term effects of DLI dissection after FFGT. METHODS: Patients with unilateral facial paralysis who underwent smile reconstruction with FFGT and subsequent DLI dissection between 2014 and 2023 were retrospectively analyzed. Lower lip height deviation was measured at different time points after

Reconstruction of the Scrotum With Bilateral Gracilis Muscle Rotational Flaps in a Patient With Acute Myeloid Leukemia Complicated by Fournier's Gangrene.

We describe the case of a 43-year-old male diagnosed with acute myeloid leukemia complicated by Fournier's gangrene. Multiple debridements led to the complete effacement of the scrotum, with 360 degrees of exposed testes and a narrow base of suspension. It was decided to reconstruct the scrotum using bilateral gracilis muscle rotational flaps, followed by split-thickness skin grafting from the thigh. The gracilis muscle as a donor flap allowed for the protection and support of the testes and suspensory tissue while achieving an aesthetically pleasing result that resembled the normal scrotum. W

Effectiveness and safety of the use of gracilis muscle for dynamic smile restoration in facial paralysis: A systematic review and meta-analysis.

BACKGROUND: Smiling is a fundamental component of social interactions. Significant challenges arise for patients with facial palsy. One of the key procedures for dynamic smile restoration is the microneurovascular transfer of a gracilis muscle. We aimed to assess the effectiveness and safety of dynamic smile reanimation surgery using the free gracilis muscle unit in patients with facial palsy. METHODS: We conducted a systematic review and meta-analysis of studies reporting surgical outcomes of dynamic smile restoration using free gracilis muscles identified from EMBASE, Medline, and Web of Sc
