Severe recurrent hypernatremia and probable lithium-associated nephrogenic diabetes insipidus following pancreatectomy in a patient with schizophrenia: a case report.

BACKGROUND: Nephrogenic diabetes insipidus (NDI) is characterized by impaired renal responsiveness to arginine vasopressin, causing defective urinary concentration and large volumes of dilute urine. Long-term lithium exposure is an important acquired cause. Perioperative fasting, restricted free-water intake, and ongoing fluid losses may unmask compensated lithium-associated urinary concentrating dysfunction and precipitate severe hypernatremia. CASE PRESENTATION: A 61-year-old man underwent laparoscopic resection of a pancreatic tail lesion. His history included schizophrenia, type 2 diabete

Visual disturbances during prolonged space missions.

PURPOSE OF REVIEW: During prolonged spaceflight, astronauts often experience ocular changes, due to constant head-ward fluid shifts in space as compared with Earth. This article reviews symptoms, likely causes, and potential solutions, such as lower body negative pressure, to counteract space-associated neuroocular syndrome (SANS). RECENT FINDINGS: Low gravity conditions and other aspects of spaceflight affect the eye detrimentally, causing SANS which is characterized by optic disc edema, choroidal thickening, cotton wool spots, and a hyperopic shift. SANS is probably caused by altered hemody

Paving the way to better understand the effects of prolonged spaceflight on operational performance and its neural bases.

Space exploration objectives will soon move from low Earth orbit to distant destinations like Moon and Mars. The present work provides an up-to-date roadmap that identifies critical research gaps related to human behavior and performance in altered gravity and space. The roadmap summarizes (1) key neurobehavioral challenges associated with spaceflight, (2) the need to consider sex as a biological variable, (3) the use of integrative omics technologies to elucidate mechanisms underlying changes in the brain and behavior, and (4) the importance of understanding the neural representation of gravi

Ultrasound evaluation of sinus fluid levels in swine during microgravity conditions.

BACKGROUND: Acute rhinosinusitis is a common problem that could occur in space secondary to absence of gravity-dependent drainage or odontogenic or external sources of infection. The purpose of this study was to determine the efficacy of ultrasound to determine sinus fluid distribution levels in swine and to assess the accuracy of ultrasound in the animal during normal and microgravity conditions. METHODS: Anesthetized swine had a catheter placed through a frontal bone window to allow aliquots of a viscous solution to be injected at 1 G (N = 4) or during brief microgravity parabolic flights (

Development as adaptation: a paradigm for gravitational and space biology.

Adaptation is a central precept of biology; it provides a framework for identifying functional significance. We equate mammalian development with adaptation, by viewing the developmental sequence as a series of adaptations to a stereotyped sequence of habitats. In this way development is adaptation. The Norway rat is used as a mammalian model, and the sequence of habitats that is used to define its adaptive-developmental sequence is (a) the uterus, (b) the mother's body, (c) the huddle, and (d) the coterie of pups as they gain independence. Then, within this framework and in relation to each o

The origin of contraversive pushing: evidence for a second graviceptive system in humans.

BACKGROUND: Stroke patients may exhibit the peculiar behavior of actively pushing away from the nonhemiparetic side, leading to lateral postural imbalance and a tendency to fall toward the paralyzed side. This phenomenon has been called the "pusher syndrome." OBJECTIVE: The current study analyzes the mechanism leading to contraversive pushing. METHODS: The subjective postural vertical (SPV) and subjective visual vertical (SVV) were determined in five consecutively admitted patients with severe contraversive pushing and in controls. Whereas adjustment of the SPV reflects the perceived upright

Differential muscle strength decline in osteoarthritis of the knee. A developing hypothesis.

The ratio of quadriceps to hamstrings muscle strength (Q/H ratio) is important for the stability of the knee and for protection from excessive stress. The purpose of this study was to explore whether the Q/H ratio is altered in patients with osteoarthritis (OA). Subjects were 43 patients with physician-diagnosed OA of the knee (mean age, 65.61 years; SD, 12.74). Isometric knee flexor and extensor strength was measured with a hand-held dynamometer. Both muscle groups were weak, with relatively greater weakness in the quadriceps muscles. The mean Q/H ratio of 1.43 (SD, 0.39) was below ratios rep

Effect of pregnancy on anthropometrics, comfort, and functional performance for women living with transfemoral limb loss: Case report.

Women living with limb loss (amputees) have unique needs that increase during pregnancy. Comfort, pain, and activity are exponentially compromised. Changes in mass and altered center of gravity increase the likelihood of falls and compromise safety for the mother and unborn baby. Subjective and physical functional performance measures data was collected through 2 separate pregnancies on a 36-year-old woman with a right transfemoral amputation. Physiological changes during pregnancy effected comfort, pain, and functional performance. Clinical practice guidelines for prosthetic management should

Assessment of muscle strength in Duchenne muscular dystrophy.

Muscle strength in 23 patients with Duchenne dystrophy was tested against gravity and manual resistance during an 8-year period. The data show striking linearity in rate of loss of strength with age for any given patient. The tempo does not appear altered during growth spurts, bracing, or loss of ambulation. Variability in disease severity was documented clearly by 7 years of age and appears to be related to earlier age at onset of symptoms. This long ignored method of muscle strength assessment provides a precise measure of disease progression and, since the technique is widely used by physic

[Disorders of microcirculation in patients with acute surgical diseases of the biliary tract].

The authors provide the results of studying the microcirculation in 85 patients with acute cholecystitis and 56 patients with mechanical jaundice caused by choledocholithiasis. The data were obtained with the aid of bulbar capillaroscopy during conservative and surgical treatment. Considerable alterations were detected in all the components of the microcirculation (perivascular, vascular, intravascular), correlating with the gravity of destructive lesions in the gallbladder, duration and degree of jaundice. The times and degree of the normalization of microcirculatory disorders were defined.

[Autonomic regulation in the acute period of tick-borne encephalitis in children].

The clinico-epidemiological analysis and comparison of the data of a comprehensive study of suprasegmental vegetative regulation revealed an appreciable tension of the adaptation mechanisms in the acute period in 55 children with the meningeal form of tick-borne encephalitis (TE). Alterations in vegetative regulation in children suffering from TE were found to be coupled with impairment of the stem-hemispheric interrelations, reduction in the level of nonspecific activation, rise of convulsive preparedness of the brain. A correlation was established between alterations in the main parameters o

[Clinico-statistical data on the sexualtiy of the male diabetic].

In 60 male diabetics aged between 16 and 60 followed up for 5 years, libido was found to be reduced in 58% and potency in 63%. The alteration in sex function would not appear to be related to the duration or gravity of the diseases, nor with the existence of vasculopathy; it was, however, related to age (a prevalence in elderly subjects) and to the existence of neuropathy. In one third of subjects, impotence preceded or was contemporaneous with clinical diagnosis of diabetes. Spermiogenesis alterations, present in more than half the cases, were closely related to gonadotropinuria LH; this find

Medical monitoring during the NASA Artificial Gravity-Bed Rest Pilot Study.

The NASA artificial gravity-bed rest pilot study (AGPS) was designed to investigate the efficacy of daily exposure to a +Gz acceleration gradient for counteracting the physiologic decrements induced by prolonged bed rest. Test subjects were continuously monitored by a physician for signs and symptoms of pre-syncope, motion sickness, and arrhythmias while on the centrifuge. In this article, we have summarized the medical monitoring observations that were made during the AGPS and included an assessment of the relative usefulness of the information provided by the various monitoring tools in maki

Hypothesis on the outflow of optic nerve cerebrospinal fluid in spaceflight associated neuro ocular syndrome.

Spaceflight-associated neuro-ocular syndrome (SANS) has been well documented in astronauts. However, its pathogenesis is not fully understood. New findings indicate the impaired outflow of the optic nerve cerebrospinal fluid may participate or contribute to some changes in SANS. In this perspective, we generated a hypothesis that the outflow of cerebrospinal fluid through the optic nerve sheath may be impaired under micro-gravity and then may potentially lead to SANS-related alterations.

Update on Clinical Physiology and Pathomechanisms for Vestibulo-Autonomic Interplay.

The vestibulo-autonomic interaction refers to the neural interplay between the vestibular and autonomic systems. In particular, the vestibular system plays an active role in adjusting blood distribution during movement and changes in posture, thereby complementing the baroreflex. This review summarizes recent clinical evidence highlighting the interaction between the vestibular and autonomic systems, including altered vestibular-evoked myogenic potentials in patients with postural orthostatic tachycardia syndrome, orthostatic hypotension, and neurodegenerative disorders. Additionally, the revi

Convergence of vestibular and proprioceptive signals in the cerebellar nodulus/uvula enhances the encoding of self-motion in primates.

The integration of different sensory streams is required to dynamically estimate how our head and body are oriented and moving relative to gravity. This process is essential to continuously maintain stable postural control, autonomic regulation, and self-motion perception. The nodulus/uvula (NU) in the posterior cerebellar vermis is known to integrate canal and otolith vestibular input to signal angular and linear head motion in relation to gravity. However, estimating body orientation and motion requires integrating proprioceptive cues with vestibular signals. Lesion studies demonstrate that

Is sleep affected after microgravity and hypergravity exposure? A pilot study.

Sleep is known to be affected in space travel and in residents of the international space station. But little is known about the direct effects of gravity changes on sleep, if other factors, such as sleep conditions, are kept constant. Here, as a first exploration, we investigated sleep before and after exposure to short bouts of microgravity and hypergravity during parabolic flights. Sleep was measured through actigraphy and self-report questionnaires in 20 healthy men and women before and after parabolic flight. Higher sleep fragmentation and more awakenings were found in the night after the

Chronic Hypergravity Induces a Modification of Histone H3 Lysine 27 Trimethylation at TCRβ Locus in Murine Thymocytes.

Gravity changes are major stressors encountered during spaceflight that affect the immune system. We previously evidenced that hypergravity exposure during gestation affects the TCRβ repertoire of newborn pups. To identify the mechanisms underlying this observation, we studied post-translational histone modifications. We first showed that among the four studied post-translational histone H3 modifications, only lysine 27 trimethylation (H3K27me3) is downregulated in the thymus of mice exposed to 2× g for 21 days. We then asked whether the TCRβ locus chromatin structure is altered by hypergravit

Phytochrome-mediated agravitropism in Arabidopsis hypocotyls requires GIL1 and confers a fitness advantage.

Plants use specialized photoreceptors to detect the amount, quality, periodicity and direction of light and to modulate their growth and development accordingly. These regulatory light signals often interact with other environmental cues. Exposure of etiolated Arabidopsis seedlings to red (R) or far-red (FR) light causes hypocotyls to grow in random orientations with respect to the gravitational vector, thus overcoming the signal from gravity to grow upwards. This light response, mediated by either phytochrome A or phytochrome B, represents a prime example of cross-talk between environmental s
