Relationship between ankle invertor H-reflexes and acute swelling induced by inversion ankle sprain.

STUDY DESIGN: Single group, post-test design using the uninvolved lower extremity as the experimental control. OBJECTIVES: To determine relationships between ankle swelling and flexor digitorum longus and peroneus longus H-reflex amplitude and latency. BACKGROUND: Primary capsuloligamentous injury, neural injury, and joint effusion and swelling may contribute to H-reflex changes following inversion ankle sprain. The relationship between ankle swelling and invertor or evertor H-reflexes has not been reported. METHODS AND MEASURES: Fifteen subjects with acute grade I or II inversion ankle spr

Caloric stimulation-induced augmentation of H-reflexes in normal subjects, but not in spinal cord-injured patients.

This study examined the effects of ice water caloric stimulation on H-reflex amplitude in normal subjects and three complete spinal cord-injured patients. H-reflexes were obtained by stimulating the tibial nerve at the popliteal fossa and recording the H-response from the gastrocnemius muscle. All normal subjects who experienced nystagmus or vertigo demonstrated significant augmentation in H-reflex amplitude with ice water irrigation of the ear canal. In the three spinal cord-injured patients, there was no significant change of H-reflex with the ice water stimulus. The results suggest that des

Repeated transspinal stimulation decreases soleus H-reflex excitability and restores spinal inhibition in human spinal cord injury.

Transcutaneous spinal cord or transspinal stimulation over the thoracolumbar enlargement, the spinal location of motoneurons innervating leg muscles, modulates neural circuits engaged in the control of movement. The extent to which daily sessions (e.g. repeated) of transspinal stimulation affects soleus H-reflex excitability in individuals with chronic spinal cord injury (SCI) remains largely unknown. In this study, we established the effects of repeated cathodal transspinal stimulation on soleus H-reflex excitability and spinal inhibition in individuals with and without chronic SCI. Ten SCI a

Soleus H-reflex modulation during body weight support treadmill walking in spinal cord intact and injured subjects.

The soleus H-reflex modulation pattern was investigated in ten spinal cord intact subjects during treadmill walking at varying levels of body weight support (BWS), and nine spinal cord injured (SCI) subjects at a BWS level that promoted the best stepping pattern. The soleus H-reflex was elicited by tibial nerve stimulation with a single 1-ms pulse at an intensity that the M-waves ranged from 4 to 8% of the maximal M-wave (M(max)). During treadmill walking, the H-reflex was elicited every four steps, and stimuli were randomly dispersed across the gait cycle which was divided into 16 equal bins.

Somatosensory graviception inhibits soleus H-reflex during erect posture in humans as revealed by parabolic flight experiment.

The purpose of this study was to investigate how gravity level affects the excitability of the soleus muscle (SOL) motoneuron pool to Ia afferent input while erect posture is maintained in humans. Three healthy male subjects participated in an experiment whereby three different gravity conditions [microgravity (MG), normal gravity (NG), and hypergravity (HG)] were imposed using a parabolic flight procedure. The SOL H-reflex was evoked every 2 s while the subjects kept an erect posture. The stimulus intensity was controlled automatically on a real-time basis by personal computer to induce the c

Soleus H-reflex size versus stimulation rate in the presence of background muscle activity: a methodological study.

Hoffmann reflex (HR) operant conditioning (HROC) is an important intervention for neurorehabilitation. Current HROC paradigms elicit HRs at low rates (~ 0.2 Hz), minimizing rate-dependent depression (RDD). We investigated the impact of higher stimulation rates on HR size. Fifteen healthy participants maintained low background soleus electromyographic activity (EMG) while standing. Soleus HR and M-wave recruitment curves were obtained at rates of 0.2, 1, and 2 Hz twice, from which Mmax and Hmax were calculated. Seventy-five HRs were collected for each rate at a target M-wave size (~ 10 to 20% o

Post-activation depression from primary afferent depolarization (PAD) produces extensor H-reflex suppression following flexor afferent conditioning.

Suppression of the extensor H-reflex by flexor afferent conditioning is thought to be produced by a long-lasting inhibition of extensor Ia afferent terminals via GABAA receptor-activated primary afferent depolarization (PAD). Given the recent finding that PAD does not produce presynaptic inhibition of Ia afferent terminals, we examined in 28 participants if H-reflex suppression is instead mediated by post-activation depression of the extensor Ia afferents triggered by PAD-evoked spikes and/or by a long-lasting inhibition of the extensor motoneurons. A brief conditioning vibration of the flexor

Association between hypotension during 24 h ambulatory blood pressure monitoring and reflex syncope: the SynABPM 1 study.

AIMS: Diagnostic criteria for ambulatory blood pressure monitoring (ABPM) in patients with suspected reflex syncope are lacking. The study hypothesis was that patients with reflex syncope have a higher prevalence of systolic blood pressure (SBP) drops on ABPM. METHODS AND RESULTS: ABPM data from reflex syncope patients and controls, matched by average 24 h SBP, age, sex, and hypertension were compared. Patients with constitutional hypotension, orthostatic hypotension, and predominant cardioinhibition during carotid sinus massage or prolonged electrocardiogram monitoring or competing causes of

Soleus H-reflex excitability changes in response to sinusoidal hip stretches in the injured human spinal cord.

Imposed static hip stretches substantially modulate the soleus H-reflex in people with an intact or injured spinal cord while stretch of the hip flexors affect the walking pattern in lower vertebrates and humans. The aim of this study was to assess the effects of dynamic hip stretches on the soleus H-reflex in supine spinal cord injured (SCI) subjects. Sinusoidal movements were imposed on the right hip joint at 0.2 Hz by a Biodex system. H-reflexes from the soleus muscle were recorded as the leg moved in flexion or extension. Stimuli were sent only once in every hip movement cycle that each la

Abnormalities of the soleus H-reflex in lumbar spondylolisthesis: a possible early sign of bilateral S1 root dysfunction.

Using routine electrodiagnostic procedures, the authors searched for physiologic evidence of nerve root compromise in patients with chronic mechanical perturbation to the lumbar spine. They examined 37 patients with spondylolisthesis and various degrees of degenerative changes in the lumbar canal. Clinical and neurophysiologic findings were compared with data obtained from 36 healthy persons. The soleus H-reflex appeared to be a sensitive indicator of sensory fiber compromise at the S1 root level, because changes correlated well with the focal sensory signs and preceded clinical and electromyo

Effects of TENS and topical skin anesthesia on soleus H-reflex and the concomitant influence of skin/muscle temperature.

The purpose of this study was to determine, in ten healthy subjects, the extent of soleus motoneuronal excitability during conditions of increased (transcutaneous electrical nerve stimulation [TENS]), decreased (Xylocaine [lidocaine]a anaesthesia) and normal (placebo anaesthesia) cutaneous inputs. Increased cutaneous activity was evoked using a TENS unit, with the two pairs of electrodes placed respectively over the Achilles (S2 dermatome) and tibialis anterior (L5 dermatome) tendons. Experimental and placebo topical anaesthesia were obtained after rubbing Xylocaine (5%) and Vaselineb ointment

Two patterns of results in polyneuropathies investigated with the H reflex. Correlation between proximal and distal conduction velocities.

The Hoffman (H) reflex of the soleus muscle has been used to study peripheral neurological disorders in 79 cases of chronic renal insufficiency prior to the initiation of haemodialysis, and in 94 cases of chronic alcoholism. In both groups, care was taken to ensure that the subjects examined could not be suspected of any other type of neuropathy. The results have been compared with those obtained in 42 normal subjects. The nerve conduction velocity in the reflex pathway was evaluated by an H index formula in which the conduction time (interval between M and H responses) is related to the lengt

Effects of tripolar TENS on slow and fast motoneurons: a preliminary study using H-reflex recovery curve method.

This study aimed at examining the effect of tripolar TENS of vertebral column on the activity of slow and fast motoneurons on 10 healthy non-athlete women aged 22.7 +/- 2.21 yrs. H-reflex recovery curve of soleus (slow) and gastrocnemius (fast) muscles were recorded before and after applying tripolar TENS. For recording of this curve, rectangular paired stimuli were applied on tibial nerve (with 40-520 ISI, frequency of 0.2 Hz and pulse width of 600 micros). Our findings showed that maximum H-reflex recovery in gastrocnemius muscle appeared in the shorter ISI, while in soleus muscle, it appear

Sensorimotor cortex ablation prevents H-reflex up-conditioning and causes a paradoxical response to down-conditioning in rats.

Operant conditioning of the H-reflex, a simple model for skill acquisition, requires the corticospinal tract (CST) and does not require other major descending pathways. To further explore its mechanisms, we assessed the effects of ablating contralateral sensorimotor cortex (cSMC). In 22 Sprague-Dawley rats, the hindlimb area of left cSMC was ablated. EMG electrodes were implanted in the right soleus muscle and a stimulating cuff was placed around the right posterior tibial nerve. When EMG remained in a specified range, nerve stimulation just above the M response threshold elicited the H-reflex

The Soleus H-Reflex as a Biomarker of Post-Injury Spasticity in Spinal Cord Injury: A Conceptual Pilot Study.

BackgroundSpinal cord injury (SCI) disrupts neuronal pathways, alters spinal reflexes, and often leads to spasticity-a condition of increased muscle tone and involuntary contractions that interfere with daily function. Spasticity typically emerges progressively after injury; however, its mechanisms remain poorly understood, particularly regarding why it develops in some individuals but not in others. Increased spinal excitability has been implicated, but prospective evidence remains scarce.ObjectiveThis pilot, proof-of-concept study explored whether early electrophysiological markers of spinal
