[Radiographic study of the rheumatoid hand and etiopathogenic mechanisms of deformities. Apropos of 100 cases].

X-rays of the hands and wrists of 100 patients suffering from rheumatoid arthritis (RA) were compared to those of 65 control subjects matched for age and sex. Osteoarticular destruction was assessed using Larsen's index and by carpal bone deformities; ulnar deviation of the fingers (UDF) and radial deviation of carpal bones (RDC). No significant asymmetry of lesions in the right or left hands can be demonstrated, whatever the subsets of patients and osteoarticular manifestations studied. After a one year course, there is no significant difference between patients and control subjects. Only the

Transfer of the abductor digiti minimi (quinti) in radial deformities of the hand in children.

The abductor digit minimi (quinti) transfer is an opposition plasty which is rarely used because of the large armamentarium of other possible transfers. However, it is a very good technic to use in radial side dysplasia in children, especially in the case of four-fingered hands in which it can improve the results of pollicisation. A series of 14 cases with 13 "good" or "very good" results is presented. An anatomical study has defined the distal insertion, distinct from that of the flexor digiti quinti brevis, the vascularisation (Mathes's type II) and the innervation of the muscle. The divisio

Correction of first web space deficiency in congenital deformities of the hand with the pseudokite flap.

The authors describe a new flap to enlarge or create a first web in patients with congenital deficiencies of the hand. This lozenge-shaped flap is harvested from the dorsoradial aspect of the index finger based on a narrow proximal skin bridge protecting its axial vascularization. A dorsal skin graft of the donor site is avoided by closure using a rhomboid flap. Procedures in 16 patients were performed and reviewed; the only complication was insertion of a small split-thickness graft in five patients to avoid tension at the metacarpophalangeal joint level. The flap lengthened the web fold an a

A new protocol for the treatment of hand deformities in recessive dystrophic epidermolysis bullosa (13 cases).

The recessive form of dystrophic epidermolysis bullosa creates severe hand deformities with disabling functional limitations in the main daily activities. Typically, the thumb is contracted in adduction, the first web space is obliterated, the palm and digits are contracted in flexion and interdigital spaces are lost (pseudo-syndactyly). In this paper, we present our experience with a protocol based on the association of various internationally developed techniques: brachial plexus anesthesia with ketamine sedation, dynamic splinting and coverage of the wounds with allogenic keratinocytes shee

Current Concepts to Muscle-Based Procedures to Treat Upper Limb Spasticity: Forearm, Wrist, and Hand Deformities.

Spastic forearm, wrist, and hand deformities in isolation or collectively can markedly hinder function and/or provide challenges with hygiene. When muscle contracture is the main contributor to the spastic deformity, muscle-based and tendon-based procedures can effectively correct the abnormal posture. Tendons from expendable muscles can be transferred to the weak or paralyzed muscle to improve the static position of the joint or to restore active function. When employed for the correct underlying pathology, muscle-based procedures can effectively correct the spastic deformity, which can lead

Biceps brachii and forearm muscle morphology related to forearm and hand deformities in spastic cerebral palsy.

INTRODUCTION: Previous studies have identified morphological alterations in spastic muscles focusing predominantly on gross motor muscles, with specific emphasis on those of the lower limbs. There is minimal information on the morphology of spastic upper limb muscles in relation to clinical deformity. Therefore, the aim of this study was to investigate the morphological parameters of the biceps brachii and forearm muscles according to the severity of forearm and wrist and finger deformities in children and adolescents with spastic cerebral palsy. METHODS: A total of 87 children and adolescent

Hand Deformities in Hajdu-Cheney Syndrome: A Case Series of 3 Patients Across 3 Consecutive Generations.

Hajdu-Cheney syndrome is a rare condition characterized by acro-osteolysis, osteoporosis, and multiple craniofacial anomalies. The goal of treatment is to reduce the associated symptoms and to prevent osteoporotic fractures. This is a report of 3 patients across consecutive generations demonstrating variable phenotypic severity. The hand surgeon was the first medical care provider visited by the patients because of the shortening of the fingers.

Children with surgically corrected hand deformities and upper limb deficiencies: self-concept and psychological well-being.

We studied self-concept and psychological well-being in children with hand deformities and upper limb deficiencies. Ninety-two children, 53 boys, 39 girls, aged 9-11 years were included. The children were divided into two subgroups - one with milder (less visible) deformities and one with severe (more complex and visible) finger-hand-arm deformities. Of the 92 children, 79 had received reconstructive surgery, and 13 had been treated with prostheses. The Piers-Harris Children's Self-Concept Scale (PHCSCS) was used to measure self-esteem and well-being. Overall PHCSCS scores showed that the whol

Treatment of hand deformities in a long-term survivor with dermolytic bullous dermatosis-recessive (DBD-R).

Treatment of hand deformities in a long-term survivor with dermolytic bullous dermatosis-recessive is described. An "open method" is sufficient for dealing with volar skin defects. There is still no effective treatment for perfect control of blister formation. But appropriate surgery, followed by careful post-operative rehabilitation can give reasonable hand function.

First web-space reconstruction by Caroli's technique in congenital hand deformities with severe thumb ray adduction.

A personal technique is described for reconstruction of the first web-space in congenital hand deformities with severe adduction of the thumb. The technique is based on the use of two local rotation flaps, one designed on the thenar area and the other on the radial half of the back of the hand. Seven patients were treated with such a technique and followed up for an average of 32 months. Excellent results were obtained in five cases.

[Cell death in organogenesis. Creodes and catastrophies. Cell non-death syndromes of face and hand deformities].

Cell death appears to be a paradoxical process but one that is necessary for embryonic and fetal morphogenesis, being a necessary stage in the morphogenetic creode of the organ considered, as dynamically and qualitatively modelled by René Thom. Subtle environmental processes result in "cell suicide" by liberation of the intracellular necrosis enzyme. This phenomenon exists just as naturally during separation of the fingers, fusion of the facial buds, and craniofacial suture growth. Dysfunction produces the syndrome of "cell non-death" as seen for example in Apert's syndrome in which a stomodea

Right-sided Poland syndrome with no classical hand deformity: A case report.

Poland syndrome (PS) is a rare congenital condition characterized by the developmental anomaly of the chest wall, and classically presents with ipsilateral agenesis/hypoplasia of sternocostal head of pectoralis major which remains as the essential feature of the condition. It may or may not be associated with ipsilateral limb abnormalities. Diagnosis is mainly clinical. Imaging modalities like chest X-Ray, CT chest, US, and 3D-VRT CT can be used to know about the extent of involvement of this anomaly. We hereby present a case of a 25-year-old male who visited our hospital with chief complaint

A technique of hand digit reconstruction using scarred and deformed donor segments.

BACKGROUND: Hand digit reconstruction with transposition of hand segments in cases of hand injury is not used frequently by surgeons, at least in part because of potential ischemic complications after cicatricial changes in tissues. The aim of this study was to investigate the opportunities for expanding the set of indications for hand digit reconstruction using scarred and deformed donor segments. METHODS: We developed a technique of ischemic preconditioning and preforming the segment before transfer to improve macro-circulation and micro-circulation in scarred donor segments with a view to

Single-stage management of a neglected radial club hand deformity in an adult.

Radial club hand is an intercalary congenital deformity involving the forearm, wrist and hand. A congenital absence of radius (partial or complete) and ulnar bowing are classical radiographic abnormalities seen in this condition. This deformity is usually treated surgically in infants and young children but the management of this problem in an adult is complex and challenging. We present a neglected case of an adult with severe and rigid deformity that was successfully treated by one-stage correction involving ulnar osteotomy and wrist arthrodesis, simultaneously.

Flexor origin release and functional prehension in adult spastic hand deformity.

Eighteen adult patients with spastic hand deformities underwent release of the flexor-pronator origin and step-cut lengthening of the flexor pollicis longus to improve prehensile hand function. The goal of surgery was to provide active grasp release in order to improve hand function. Using a previously published grading scale, patients were graded before and after operation. At follow-up after an average of 35 months (range 24-64 months), all had improved by at least two functional levels.

Dorsal skin and fingernails on the volar aspect of the hand: an unusual anatomic deformity.

A very unusual hand malformation is reported in which dorsal skin and nails exist on the volar aspect of the hands. This patient also exhibits mental retardation and microcephaly as well as a partial deletion of chromosome 6. Fifty percent of a small series of patients previously reviewed who presented with chromosome 6 abnormalities demonstrated abnormalities of the hands. Unusual deformities of the hands associated with mental retardation and microcephaly may be related to an abnormality of chromosome 6.

Split-hand and split-foot deformity inherited as an autosomal recessive trait.

A intermarried consanguineous family with split-hand and -foot deformity occurring in two sibship is presented. Both the sibship resulted from marriage between first cousins. This report, together with those of Ray (1960) and Freire-Maia (1971), further demonstrates that split-hand and -foot deformity can be inherited as an autosomal recessive trait.
