Repair of extensor tendon injuries of the hand.

We have presented methods for treating extensor tendon injuries from the interphalangeal joint to the wrist and the musculotendinous junction in the forearm. Early and proper splinting in the treatment of extensor tendon injuries is more important than a specific method of surgical repair. We emphasize the need for prolonged splinting, up to eight weeks in distal injuries. Immobilizing the finger in full extension or hyperextension is necessary at the distal and proximal interphalangeal joints. Correct splinting is mandatory in any method of treatment. Reconstruction of the extensor mechanism

Management of Rope Friction Injuries of the Hand.

BACKGROUND: Rope friction injuries result from rapid, repetitive contact with coarse ropes or chains, causing combined mechanical and thermal damage. These injuries commonly affect young individuals during activities such as tug-of-war, climbing, air acrobatics, and rope handling. Although superficial and deep burns are most frequent, injuries may also involve vessels, nerves, tendons, and bone. This study presents our experience managing rope friction burns and reviews functional and aesthetic outcomes. METHODS: We retrospectively reviewed 23 patients with full-thickness hand defects after r

Imaging Traumatic Wrist and Hand Injuries in Children.

The pediatric wrist and hand are anatomically and functionally complex regions that undergo continuous changes throughout growth and maturation. This review opens with a discussion of normal skeletal development, emphasizing typical patterns of ossification and growth plate maturation in the distal forearm, carpus, and phalanges. Physeal anatomy is explained, followed by a review of acute physeal injuries (such as Salter-Harris fractures), and common nonphyseal fractures including buckle, incomplete, and complete fractures. Finally, repetitive stress injuries affecting the distal radius of gym

Hand injuries with mole gun: A hidden danger.

BACKGROUND: Mole guns are handmade destructive tools used in the fight against harmful rodents in agricultural areas. Acciden-tal triggering of these tools at the wrong time can result in major hand injuries that impair hand functionality and cause permanent hand disability. This study aims to draw attention to the fact that mole gun injuries cause severe loss of hand functionality and that these tools should be considered within the scope of firearms. METHODS: Our study is a retrospective, observational cohort study. The demographic characteristics of the patients, the clinical features of t

Hand and Wrist Injuries in the Pediatric Athlete.

This article examines the most common problematic hand and wrist injuries in the pediatric athlete. Hand and wrist injuries in the growing skeleton pose a different diagnostic and therapeutic challenge than in the mature skeleton. Ligaments are stronger than bone, and unossified cartilaginous sections of the skeleton are yet more susceptible to injury than bone. Although remodeling can correct for even moderate deformities if sufficient growth potential exists, remodeling cannot return the child to normal anatomy in many cases. Remodeling depends on intact periosteum, a nearby growing physis,

Sports Injuries of the Hand, Wrist, and Elbow.

Sports injuries of the upper extremity are a common problem seen by those who care for athletes and those who manage upper extremity injuries. The term "high-level athlete" may include adolescents and high school students, collegiate athletes, and of course, the professional athlete. However, the "weekend warrior" can sustain similar injuries and can have a similar desire to return to play as quickly as possible. The challenge in management of these injuries to the elbow, wrist, and hand is identifying those injuries that will benefit from early surgery compared with those that may be safely t

Ligamentous sports injuries of the hand and wrist.

Athletic injuries of the hand and wrist continue to increase as athletes get bigger, faster, and the weekend warrior continues to be active throughout his or her lifespan. The management of hand and wrist injuries in these athletes depends on the sport, position, timing during the season the injury occurs, and how many years the athlete has left to play in competition. In the past, these injuries were thought to be trivial; but if not ideally managed, complications can occur which may keep the skilled athlete or the weekend warrior out for a prolonged period of time. This article concentrates

Reverse on-top plasty in mutilating hand injuries.

For crushing injury of the hand that involves 4-digit amputations sparing the thumb, lengthening of amputated stump and covering the defect are important for hand function. From March 2007 to September 2011, the authors performed "reverse on-top plasty" in 5 patients. The fifth metacarpal bone was disarticulated, turned over 180 degrees, and embedded in reverse direction so that the metacarpal base becomes the tip of new finger. Soft tissue defects were covered using anterolateral thigh free flap or reverse radial forearm flap. The mean follow-up period was 21.2 months, and major complications

[Primary treatment of tendon injuries of the hand].

Tendon injuries of the hand are common and their treatment is surgically challenging. Precise knowledge of the anatomy of the extensor and flexor tendons in the hand is necessary to be able to perform a detailed clinical examination and to estimate adequately the extent of injury. Depending on the injury pattern, various conservative and surgical treatment options must be considered. Concerning the overall concept of the supply of tendon injuries of the hand, follow-up treatment is crucial to achieve an optimal functional outcome. It should be noted, however, that the results are influenced by

[Hand function after surgery for flexor tendon injuries].

BACKGROUND: Flexor tendon injuries are difficult to treat and the postoperative results have traditionally been poor. MATERIAL AND METHODS: The article presents postoperative results (11-22 months) after surgical treatment of injured flexor tendons at Stavanger University Hospital. 32 injuries in 30 patients met the inclusion criteria; 23 of these patients responded to letters and telephone contacts. A physician and a hand therapist followed up 20 of the patients with a clinical examination and three were just interviewed by telephone. The results were evaluated according to joint movement, g

Neglected tendon and nerve injuries of the hand.

We retrospectively reviewed 445 patients with neglected tendon and nerve injuries treated between 1983 and 2003. Of these patients, 355 patients (447 tendons) had flexor tendon injuries, 62 patients (84 tendons) had extensor tendon injuries, and 28 patients had isolated nerve injuries. (In addition to these, 48 patients had nerve injuries associated with flexor tendon injuries; making a total of 76 patients with nerve injuries.) Most of the patients presented for treatment from 2 months to 2 years after the injury. The majority (73%) of patients with flexor tendon injuries were treated by free
