Hand-assisted laparoscopic nephrectomy for polycystic kidney disease.

BACKGROUND AND OBJECTIVES: Historically, nephrectomy for autosomal dominant polycystic kidney disease was performed by an open technique. We performed this study to compare outcomes in hand-assisted laparoscopic nephrectomy with open nephrectomy in this population. METHODS: Charts of patients with autosomal dominant polycystic kidney disease who underwent nephrectomy by a transplant surgeon from January 1, 2000, to December 31, 2011, were reviewed. The hand-assisted laparoscopic nephrectomy group was compared with the open group. Data collected included unilateral versus bilateral nephrectomy

Hand-assisted laparoscopic nephroureterectomy with cystoscopic en-bloc excision of the distal ureter and bladder cuff and isthmusectomy in a horseshoe kidney for invasive urothelial carcinoma of the renal pelvis.

Upper tract invasive urothelial carcinoma and horseshoe kidneys are familiar to the practicing urologist but relatively rare individual entities. The complication of managing them when they coexist in the same patient can be challenging. Herein, we present the first reported case in which an upper tract invasive urothelial carcinoma in a horseshoe kidney was successfully managed with a combined hand-assisted laparoscopic nephroureterectomy and isthmusectomy with cystoscopic en-bloc excision of the distal ureter and bladder cuff. This highlights the fact that complex anatomy can be managed in a

Intraoperative hyperkaliemia complicating hand-assisted live-donor nephrectomy.

OBJECTIVES: Live donation has become increasingly valuable as a potential source of kidneys for transplant with hand-assisted laparoscopic donation is an important method of organ procurement. An important consideration is adequate preoperative and intraoperative planning, and precautions to minimize potential risks in the donor while providing a graft with good primary function. MATERIALS AND METHODS: We present a case of live donation in which a hand-assisted laparoscopic approach was used to facilitate kidney donation. The process was complicated by a period of profound intraoperative hype

Comparison of open, laparoscopic, and hand-assisted laparoscopic devascularization of the upper stomach and splenectomy for treatment of esophageal and gastric varices: a single-center experience.

INTRODUCTION: Laparoscopic devascularization of the upper stomach and splenectomy (Dev+Sp), is technically difficult in patients with portal hypertension because of enlarged collateral vessels and spleen. This report presents the efficacy and safety of hand-assisted laparoscopic (HALS) Dev+Sp. METHODS: Ten patients underwent HALS Dev+Sp as a treatment for esophagogastric varices between 2005 and 2012 (HALS group). A vessel-sealing system was used for devascularization of vessels. An autosuture device was applied to dissect the splenic hilum and the left gastric and enlarged short gastric vess

Hand-assisted laparoscopic versus total laparoscopic right colectomy: a randomized controlled trial.

AIM: Laparoscopic colectomy for colorectal cancer is associated with definite short-term benefits, and is increasingly practised worldwide. The limitations of a pure laparoscopic approach include a relative lack of tactile feedback and long procedural time. Hand-assisted laparoscopic surgery was introduced in an attempt to facilitate operation by improving the tactile sensation. To date, there is no consensus as to which approach is better. Herein we conducted a randomized controlled trial comparing hand-assisted laparoscopic colectomy (HALC) with total laparoscopic colectomy (TLC) in the mana

[Finger assisted laparoscopic prostatectomy].

OBJECTIVES: Laparoscopic prostatectomy is the treatment of choice for localized prostate cancer. The learning curve for unexperienced teams is dauntingly steep because it is associated with high incidence of complications and such long operation times that the procedure becomes almost unacceptable. Therefore, we needed to seek an alternative for this operation that has been almost unmodified since first description in 1998. METHODS: The patient is placed in lithotomy position and draped with an urological apron similar to the one used for transurethral resection. A beniqué sound is introduced

Temperature safety profile of laparoscopic devices: Harmonic ACE (ACE), Ligasure V (LV), and plasma trisector (PT).

BACKGROUND: Reports of iatrogenic thermal injuries during laparoscopic surgery using new generation vessel-sealing devices, as well as anecdotal reports of hand burn injuries during hand-assisted surgeries, have evoked questions about the temperature safety profile and the cooling properties of these instruments. METHODS: This study involved video recording of temperatures generated by different instruments (Harmonic ACE [ACE], Ligasure V [LV], and plasma trisector [PT]) applied according the manufacturers' pre-set settings (ACE setting 3; LV 3 bars, and the PT TR2 50W). The video camera used

Development and validation of a risk stratification score for ventral incisional hernia after abdominal surgery: hernia expectation rates in intra-abdominal surgery (the HERNIA Project).

BACKGROUND: Ventral incisional hernias (VIH) develop in up to 20% of patients after abdominal surgery. No widely applicable preoperative risk-assessment tool exists. We aimed to develop and validate a risk-assessment tool to predict VIH after abdominal surgery. STUDY DESIGN: A prospective study of all patients undergoing abdominal surgery was conducted at a single institution from 2008 to 2010. Variables were defined in accordance with the National Surgical Quality Improvement Project, and VIH was determined through clinical and radiographic evaluation. A multivariate Cox proportional hazard

Complex renal artery aneurysm managed with hand-assisted laparoscopic nephrectomy, ex vivo repair, and autotransplantation.

A 58-year-old woman had an incidentally found complex right renal artery aneurysm (RAA) during a clinical work-up for diverticulitis. The aneurysm measured 2.5 cm in diameter and was located at the right renal artery bifurcation. She was hospitalized and underwent hand-assisted laparoscopic nephrectomy with ex vivo repair of the RAA and autotransplantation into the right iliac fossa. The same incision was used to remove the kidney from the retroperitoneum as was used to transplant into the right lower quadrant. She tolerated the procedure well. Her postoperative course was uncomplicated. Hand-

A Hybrid Laparoscopic-Robotic Technique for Single Anastomosis Duodeno-Ileostomy With Sleeve Gastrectomy (SADI-S): A Consecutive Case Series Demonstrating Feasibility and Early Outcomes.

INTRODUCTION: Single anastomosis duodenoileostomy with sleeve gastrectomy (SADI-S) provides durable weight loss and metabolic improvement, particularly in patients with severe obesity. Despite outcomes comparable to other metabolic and bariatric surgery procedures, its adoption has been limited by the technical complexity of duodenal mobilization and hand-sewn duodeno-ileal anastomosis. METHODS: We describe a standardized hybrid laparoscopic-robotic approach to SADI-S developed at our institution. Laparoscopy is used for sleeve gastrectomy, bowel measurement, and initial duodenal mobilization
