Refracture after implant removal of the clavicle: a retrospective cohort analysis.

BACKGROUND: Refracture after implant removal in clavicle fractures is a relevant but insufficiently studied complication. Reported refracture rates vary due to small cohorts and heterogeneous populations. This study aims to determine the incidence of refracture following elective implant removal after confirmed fracture union and to identify potential demographic, clinical, and fracture-related risk factors. METHODS: A retrospective cohort of 575 adults who underwent implant removal after radiographically confirmed union between 2011 and 2024 was analyzed. Patients were assigned to a refractu

Secondary retrieval and complication management for failed separated instrument removal in the apical third in curved root canals: a case series.

BACKGROUND: Separated instruments in the apical third of the infected root canals can lead to persistent periapical lesions. Retrieval requires appropriate preoperative assessment and a skilled technique, especially in cases of separated instruments in the apical third of curved root canals. Otherwise, retrieval will lead to removal failure and concomitant complications. CASE PRESENTATION: Herein, we report two cases of separated instrument retrieval failure with complications due to inadequate preoperative assessment and an incorrect technical approach. In these cases, we re-evaluated the fe

Is it necessary to re-fuse a non-union of a Hallux metatarsophalangeal joint arthrodesis?

BACKGROUND: The standard treatment for a non-union of the hallux metatarsophalangeal joint fusion has been to revise the fusion. Revision fusion is technically more demanding, often involving bone grafting, more substantial fixation and prolonged period of immobilization postoperatively. We present data to suggest that removal of hardware and debridement alone is an alternative treatment option. MATERIALS AND METHODS: A case note review identified patients with a symptomatic non-union after hallux metatarsophalangeal joint (MTPJ) fusion. It is our practice to offer these patients revision fus

Outcomes Following Posterior and Posterolateral Plating of Distal Fibula Fractures.

Ankle fractures are common orthopaedic injuries often requiring open reduction and internal fixation. A variety of positions for plate placement exist for surgical fixation of distal fibula fractures, including direct lateral, posterior, or posterolateral. Direct lateral is more common, despite evidence that posterior and posterolateral placement is mechanically superior. However, concern for peroneal tendon injury from posterior plating remains. Our study investigates clinical and functional outcomes of patients who underwent operative fixation of distal fibula fractures with posterior or pos

Management of device-related wound complications in deep brain stimulation surgery.

OBJECT: Wound complications are uncommon following deep brain stimulation (DBS) surgery. However, certain key technical steps can be performed in each procedure to minimize this still troublesome risk. The authors reviewed the incidence and management of all hardware-related wound dehiscences and infections in a large patient series. METHODS: All patients undergoing new DBS hardware implantation surgery between 2002 and 2010 by a single surgeon (R.K.S.) were entered into a database after undergoing verification by cross-referencing manufacturer implantation records. All hardware-related compl

Predictors of Persistent Pain After Fixation of Distal Clavicle Fractures in an Active Military Population.

Patients who undergo open reduction and internal fixation of distal clavicle fractures have a high rate of hardware removal and persistence of symptoms, particularly when attempting to return to high-demand activities. This study evaluated the outcomes of military servicemembers after surgical treatment of distal clavicle fractures. The authors performed a retrospective analysis of active duty servicemembers who underwent open reduction and internal fixation of Neer type II distal clavicle fractures between October 17, 2007, and July 20, 2012, with a minimum of 2-year clinical follow-up. Are resorbable plating systems comparable to conventional titanium plating in paediatric maxillofacial trauma patients? A systematic review and meta-analysis.

This systematic review and meta-analysis evaluated the effectiveness of resorbable versus titanium plating systems in the management of paediatric maxillofacial fractures. A comprehensive search of PubMed, Embase, Scopus, and specialty journals up to December 2024 identified six retrospective studies published between 2010 and 2024, involving 1,584 patients aged ≤18 years. Both resorbable and titanium systems provided satisfactory fracture stability and healing. Titanium fixation was consistently associated with higher complication rates, ranging from 8%-12% whereas resorbable systems demonstr

Complication rates following open surgical removal of osteosynthesis material from the pelvis and acetabulum: a retrospective case series of 154 removals.

BACKGROUND AND PURPOSE: While complication rates for primary pelvic and acetabular fracture surgeries are well documented, limited data exists on complications following osteosynthesis implant removals. We aimed to evaluate the complication rates of pelvic implant removals with respect to the surgical approach, type of implant, and indication for removal. METHODS: This retrospective, consecutive case series included all patients undergoing pelvic implant removal between January 2013 and December 2023 using Kocher-Langenbeck, modified Stoppa (AIP), or ilioinguinal approaches for the removal. Extraction of Broken Intramedullary Nails Using Olive-Tipped Guidewire and Minifragment Screws.

In nonunion surgery, removing a broken intramedullary nail can be technically challenging and time-consuming, often requiring either creating a bone window or an open approach to retrieve the broken nail. A simple, effective, and minimally traumatic technique is presented for removing the difficult-to-access part of broken canulated intramedullary nails. First, the olive-tipped guidewire is inserted and advanced down the nail until the olive end protrudes out of the broken nail. A minifragment screw is inserted through a locking screw hole in the broken part under fluoroscopy. The guidewire is

Fractured instruments in endodontics: understanding incidence, mechanisms, and emerging retrieval techniques.

Fractured instrument retrieval remains one of the most challenging aspects of endodontics, with reported incidence rates ranging from 0.3% to 12%. This complication is particularly common with nickel‒titanium (NiTi) rotary files used in curved canals. Instrument separation hinders effective canal disinfection, compromises the quality of obturation, and may necessitate additional interventions such as bypassing, retrieval, or surgical management. Conventional retrieval techniques often lead to dentin loss, compromising root structure. The use of dental operating microscopes and their integratio

Through-Plate K-Wire Fixation for Volar Rim Fractures of the Distal Radius: A Case Report.

CASE: A 28-year-old man with a distal radius fracture and small volar marginal fragment (VMF) underwent fixation using a "through-plate K-wire" technique. Two 1.2 mm K-wires were drilled through distal plate holes into the VMF and then bent over the plate to create a fixed-angle construct. Radiographic union was achieved by 6 weeks. Following elective hardware removal at 6 months, the patient demonstrated excellent clinical outcomes (DASH 4.2) and full range of motion at 1-year follow-up without complications. CONCLUSION: Through-plate K-wire fixation is a simple, cost-effective bailout optio

Management of a Pilon Fracture Complicated by a Bent Plate in Situ with Nonoperative Correction: A Case Report.

CASE: Hardware complications in the perioperative period of a pilon fracture open reduction internal fixation or fusion procedure present a unique challenge. Revision of hardware can lead to additional complications. In this case report, we share a technique of in situ plate bending to manage damaged hardware in a patient who underwent a primary tibiotalar arthrodesis for a pilon fracture. CONCLUSION: Preoperative planning, protection of the soft tissue envelope, and use of the "F" tool are keys to the success of this treatment method. Proper technique allows for a noninvasive method for corr

Review of Calcaneal Osteotomies Fixed With a Calcaneal Slide Plate.

BACKGROUND: The calcaneal slide osteotomy is a common procedure used for the surgical correction of heel varus and valgus deformities. A variety of fixation methods exist including screws and plates. The literature shows a high rate of hardware prominence with screws leading to subsequent removal of hardware. Few studies have examined the use of plates for fixation of a calcaneal osteotomy. The purpose of this study was to assess outcomes following fixation of a calcaneal osteotomy with a calcaneal slide plate. METHODS: This is a retrospective consecutive case series of all patients who under

Does instrumentation removal cause curve progression in patients with adolescent idiopathic scoliosis?

Adolescent idiopathic scoliosis (AIS) is usually instrumented using a posterior approach. Hardware removal may be performed for specific clinical reasons. Little data is available on whether removal influences curve magnitude. The aim of the paper is to evaluate the impact of instrumentation removal on curve progression, and the safety and efficacy of the procedure. We analyzed 59 patients who underwent instrumentation removal. Curve types, reasons for removal, period between procedures, and Cobb angles: at baseline, immediately after correction, after removal and in follow-up were evaluated.

Removal of cranial springs after spring-mediated cranioplasty.

Cranial spring hardware is generally removed 3 months after placement for spring-mediated cranioplasty. Spring removal is performed as an outpatient procedure under general anesthesia in approximately 15 minutes through the incision locations of the index procedure. Herein, the authors provide a multimedia demonstration of cranial spring hardware removal after spring-mediated cranioplasty for sagittal craniosynostosis. The video can be found here: https://vimeo.com/511179695.

Extraction of cannulated percutaneous screw from scaphoid: a simplified technique.

Removal of a cannulated headless screw following scaphoid fixation, when necessary, can prove to be difficult due to loss of bony integrity and the shape of the screw. This can result in greater exposure and more extensive soft tissue dissection for removal of the hardware. We describe a simple technique for percutaneous extraction of this hardware that can be done reliably with no additional equipment.
