Harm reduction approaches to alcohol use: health promotion, prevention, and treatment.

Harm reduction approaches to alcohol problems have endured a controversial history in both the research literature and the popular media. Although several studies have demonstrated that controlled drinking is possible and that moderation-based treatments may be preferred over abstinence-only approaches, the public and institutional views of alcohol treatment still support zero-tolerance. After describing the problems with zero-tolerance and the benefits of moderate drinking, the research literature describing prevention and intervention approaches consistent with a harm reduction philosophy ar

Correlates of concurrent use of stimulants and opioids among people who access harm reduction services in British Columbia, Canada: Findings from the 2019 Harm Reduction Client Survey.

BACKGROUND: During a public health overdose emergency driven by fentanyl in British Columbia (BC), Canada, a parallel increase in concurrent use of methamphetamines may be contributing to the risk of overdose. Concurrent use refers to the use of stimulants and opioids one after the other, or together. Describing substance use practices and elucidating correlates of concurrent use can inform harm reduction service provision and prevent overdose events. METHODS: This cross-sectional study analyzed the 2019 Harm Reduction Client Survey administered at 22 harm reduction sites across BC and sample

Walking a tightrope: a scoping review of the use, perceptions and experiences of harm reduction strategies in self-harm management.

Harm reduction involves safer practices for self-harm, a concept well-established in drug management but less explored in the context of self-harm. This scoping study examines empirical research on harm reduction strategies in self-harm management, focusing on practitioners' and individuals' perspectives. The scoping review follows the Joanna Briggs Institute (JBI) recommendations and analyses internationally peer-reviewed empirical studies. Systematic searches in databases like CINAHL, Scopus, Sociological Abstracts, PsycINFO, and PubMed identified 377 studies, with seven meeting the inclusio

Assessing working musicians' perspectives of need and barriers regarding lay harm reduction efforts for substance-related harm in South Carolina.

BACKGROUND: Harm Reduction (HR) approaches applied to substance use aim to decrease the toxic consequences of overdose. The addition of xylazine and benzodiazepines to fentanyl, the primary opioid in the US drug supply, increases the likelihood of overdose. Likewise, adding fentanyl to other popular substances, such as cocaine and methamphetamine, has further driven the exponential increase in opioid and stimulant overdoses. Engaging lay community members in HR efforts may help to combat the overdose crisis. Substance use is common in live music settings, and working musicians may be uniquely

The subordination of harm reduction to control in Swedish drug policy formation.

INTRODUCTION: Harm reduction has gained international policy traction and increasing recognition as essential for health equity, while punitive drug control approaches have re-emerged. This study examines Sweden, known for its restrictive drug policy, to explain why harm reduction measures have gained greater acceptance alongside increasingly severe penal responses to drug offences. Furthermore, it explores the role of stakeholders in shaping these policy processes. METHODS: We analyzed three data sets 2015-2025 (media texts, key informant interviews, stakeholder comments to two commissions o

Meeting Veterans Where They Are: Harm Reduction Supply Distribution via Vending Machines.

BACKGROUND: Harm reduction vending machines (HRVMs) provide low-barrier access to supplies that reduce overdose deaths and infections. Evidence within Veterans Affairs (VA) and supportive housing settings is limited. We evaluated 8-month HRVM implementation measures at a VA Health Care System, mapped to RE-AIM (reach, effectiveness, implementation) domains. METHODS: Prospective, single-system, single-arm, descriptive implementation evaluation across 15 HRVMs in VA health care and supportive housing settings. Considering water, sanitation, and hygiene as a valuable component of harm reduction.

Harm reduction strategies effectively minimize the immediate health consequences of drug use, such as syringe exchange and overdose prevention. However, the critical role of access to adequate Water, Sanitation, and Hygiene (WASH) services in mitigating these consequences remains underemphasized. This paper argues for the consideration of WASH services as an essential framework within the ongoing evolution of harm reduction, particularly for marginalized people who use drugs (PWUD) facing housing insecurity and limited access to public infrastructure. Lack of access to WASH services forces PWU

Transforming community health advocates into peer interventionists: a binational harm reduction model.

BACKGROUND: Community Health Advocates (CHAs) are widely employed as outreach workers for people who use drugs (PWUD), yet there is limited evidence on how to design capacity-building interventions that strengthen peer-driven behavior change in binational border settings. This study examined a structured capacity-building model that trained peer CHAs in Ciudad Juárez, Mexico, and El Paso, Texas to deliver an HIV prevention and skills-building intervention to PWUD and assessed its impact on HIV risk and protective behaviors. METHODS: We employed two complementary evaluation methods: (1) a quan

Vernacular and discursive harm reduction: an ethnography of an online drug community.

BACKGROUND: In the context of online drug communities, the ethos of harm reduction comprises a set of ethical and practical principles that enable drug enthusiasts to shift stigma by normalizing certain behaviors to the detriment of others. The HR ethos is the dominant discourse in drug forums, which may give the impression that it is a natural expression of the online sociability of drug enthusiasts. However, according to the interactionist theory of deviance, there is a process behind the demarcation between deviance and normality; this would suggest that before becoming the dominant discour

A Student's Call for Medical School Wellness Curriculums to Include Harm Reduction.

Substance use among medical students is high, has had fatal consequences, and continues postgraduation, with substance use rates among physicians being higher than the general population. Unfortunately, many institutions lack specific policies on substance use, have policies centered on punitive and disclosure-prohibitive approaches, or students are unaware of such policies. Notably, though physical and mental wellness are typical components of medical school curricula and policy, student substance use is often glaringly absent. As a result, schools can be unprepared to address substance use a

The Commercial Determinants of Nonalcoholic Beer: Redemption, Revenue, or Men's Harm Reduction?

Public awareness about the connections between men's alcohol use and poor health outcomes, including increased male suicide risk, has led to reduced consumption and increased use of nonalcoholic beverages-most prominently nonalcoholic beer (NAB). Marketed as a healthy substitution option (i.e., periodically switching to an NAB rather than abstaining from alcoholic beer), the rapidly growing NAB sector might be somewhat redemptive, wherein the alcoholic beer industry (as the predominant producers of NAB) is selling harm reduction to men, albeit for profit. The commercial determinants of NAB are

Impact of Fentanyl Test Strips as Harm Reduction for Drug-Related Mortality.

This study examines the impact of legalizing fentanyl test strips (FTSs) on drug-related mortality in the United States from 2018 to 2022. Using a difference-in-differences approach with state-level data, we find that FTS legalization is associated with a significant reduction in drug-overdose deaths. Across the population, FTS legalization corresponds to a 7% decrease in overdose mortality, with an even more pronounced 13.5% reduction among Black individuals. Our analysis employs two-way fixed effects models and triple differences specifications to isolate the effect of FTS legalization from

Exploring the harm reduction paradigm: the role of Board-Certified Psychiatric Pharmacists.

Deaths related to opioid overdoses continue to climb, and there remains a need for innovative strategies to address this ongoing crisis. Harm reduction (HR), a nonjudgmental philosophy aimed at reducing consequences associated with drug use and other potentially unsafe behavior, has emerged as a compassionate and effective approach. Harm reduction further emphasizes overdose prevention and fosters a shift in perspective that recognizes substance use disorder as a disease and not a moral failing. The tenets of HR collectively advocate for the well-being of individuals who use substances and sup

Harm Reduction as a Form of 'Wrap-Around' Care: The Nursing Role.

In addiction treatment, harm reduction is a philosophy that aims to reduce the harms from ongoing alcohol and other drug use. Although abstinence may be the 'gold standard' in reducing harm from ongoing alcohol and other drug use, harm reduction recognises that abstinence may not be achievable for certain individuals. Accordingly, harm reduction is used to enable medical or mental health treatment for individuals who continue to use alcohol and other drugs, providing a form of care which meets individuals where they present to healthcare facilities. Harm reduction accepts ongoing alcohol and o

Harm Reduction in the Field: First Responders' Perceptions of Opioid Overdose Interventions.

INTRODUCTION: Recent policy changes in Washington State presented a unique opportunity to pair evidence-based interventions with first responder services to combat increasing opioid overdoses. However, little is known about how these interventions should be implemented. In partnership with the Research with Expert Advisors on Drug Use team, a group of academically trained and community-trained researchers with lived and living experience of substance use, we examined facilitators and barriers to adopting leave-behind naloxone, field-based buprenorphine initiation, and HIV and hepatitis C virus

Introducing the Harm Reduction Collaboration Framework for Policy, Systems, and Environmental Change.

A revised way of thinking is essential for promoting harm reduction strategies and reducing the negative implications of injection drug use (IDU). Despite the growth of harm reduction approaches in the United States, there is limited guidance for designing and implementing multi-sector efforts that address the external determinants that promote and facilitate IDU. Current frameworks fail to acknowledge the individual's role and influence in multi-sector change. To address the multifaceted nature of IDU, we must address the complex relationship between people who inject drugs (PWID) and their e

Opioid Use Disorder Stigma and Support for Harm Reduction in Rural Counties.

Background: The opioid crisis is a public health emergency in the United States, particularly in rural Pennsylvania. Stigma in rural communities is a treatment barrier and impacts harm reduction programming availability. Objectives: The current study utilized an observational, cross-sectional design to examine latent subgroups of stigma and differences in support for harm reduction strategies (i.e., safe injection facilities, syringe services programs, fentanyl test strips, Naloxone distribution). Participants included rural Pennsylvanians (n = 252), taken from a statewide survey of opioid use

Confidence in nicotine for tobacco harm reduction-Bridging the policy-practice gap.

Decades of research show that constituents other than nicotine are the harmful agents in tobacco products. This knowledge is incorporated into the nicotine regulatory policies of countries leading in tobacco control, such as the UK and New Zealand. Alternative nicotine products, such as nicotine replacement therapy and e-cigarettes, are now endorsed in the UK by a number of healthcare bodies in a tobacco harm reduction approach that encourages tobacco users to completely switch to a less risky nicotine-containing product. The potential role of 'clean' nicotine alternatives to reduce the harms
