Political commitment for vulnerable populations during donor transition.

The responsibilities for the programmatic, technical and financial support of health programmes are increasingly being passed from external donors to governments. Programmes for family planning, human immunodeficiency virus, immunization, malaria and tuberculosis have already faced such donor transition, which is a difficult and often political process. Wherever programmes and services aimed at vulnerable populations are primarily supported by donors, the post-transition future is uncertain. Overreliance on donor support is often a reflection of limited domestic political commitment. Limited c

A Radiological Emergency Health and Safety Plan for City Agencies Without First Response Responsibilities.

In New York City, the citywide radiological response plan charges 4 primary agencies with various roles and objectives: police, fire, health, and environmental protection. These agencies perform essential functions such as criminal investigation, life safety, public health, and environmental assessments. In the late phase of an incident where components of remediation and recovery have been initiated, other municipal agencies that had not previously planned to respond to a radiological incident may be expected to supply various levels of support. To prepare the personnel of these other agencie

Exploring pregnant women's perspectives on pre-pregnancy physical activity in Nebraska: A qualitative study.

Physical activity (PA) is important for a woman's health particularly before pregnancy, yet many remain inactive during this time despite its benefits for themselves and their child. This qualitative descriptive study explored pregnant women's experiences with pre-pregnancy PA and the factors influencing their engagement. Using convenience sampling, data were collected from women across Nebraska between December 2023 and February 2024 via Zoom interviews. Thematic analysis, guided by the Theory of Planned Behavior (TPB), identified three key themes: (1) Behavioral attitudes - women recognized

The association between perceived 15-minute fitness circle accessibility and physical activity in China: the mediating role of informal social support.

BACKGROUND: Differences in physical activity levels partly reflect structural inequalities in access to health opportunities. According to the socio-ecological model theory, physical activity is influenced by multiple factors across individual characteristics, interpersonal relationships, community environments, and policy systems. Among these, the spatial distribution of fitness facilities, as a key component of urban planning, potential to reshape the allocation of community level and thus offers a novel pathway to alleviate health inequities. In recent years, the Chinese government has acti

Optimizing consumer choice in selection of health insurance.

Patients are confronted with an overwhelming number of choices when enrolling in health insurance plans, with marketplace enrollees often having over 100 plan options and Medicare Advantage enrollees having on average 42 plan options. This can fuel confusion, administrative burden, and suboptimal plan selection, which in turn can be costly to the patient, who may face greater cost sharing and thus reduce even necessary health care utilization. This is particularly troubling because most Americans experience low levels of health insurance literacy as well as limited numeracy, making it especial

Mental health of South Korean adolescents in the COVID-19 Era: Web-based survey focused on suicide-related factors.

BACKGROUND: The coronavirus disease 2019 (COVID-19) pandemic has negatively impacted many aspects of life. Measures for preventing the spread of COVID-19 (e.g., school lockdowns, remote and hybrid classes, group and outdoor activity restrictions, and social distancing in the classroom and meal time) could have led to adolescents to experience anxiety and depressive symptoms. Such mental health impacts could increase the risk of suicidal ideation in this population. Moreover, according to a report by the Organization for Economic Co-operation and Development, although the total number of suicid

Surprising success in Kenya's FP programme.

Mrs. Margaret Gachara, Deputy Director of the Ministry's Family Health Division, explained that the strengths and weaknesses of the program have recently been examined using a new operations research approach, known as situation analysis. "If family planning was not working in Kenya, we had to know why in order to decide what to do about it. We began the study to find out where our weaknesses lay. To our surprise, we found that we were not weak, but moderately successful," she said. The Kenya Situation Analysis Study, conducted by the Health Ministry and the Population Council, revealed that c

Reaching out: a community initiative for disadvantaged pregnant women.

A major challenge for public health services is to reach disadvantaged pregnant women with a relevant program that assists them to reduce modifiable risk factors. A community development approach was used to initiate an outreach program aimed at urban natives and other high-risk pregnant women. Community involvement in program planning, the use of native and non-native community health workers, the provision of social support and the tangible health benefits of the program contributed to its success in attracting women for service.

Care plans for women pregnant using assisted reproductive technologies: a systematic review.

BACKGROUND: Between 1 and 5% of children in industrialized countries are conceived through Assisted Reproductive Technologies (ART). As infertility and the use of ART may be associated with adverse perinatal outcomes, care plans specific to these pregnancies are needed. We conducted a systematic review to examine the existing care plans specific to women pregnant following Assisted Reproductive Technologies (ART). METHODS: MEDLINE, Embase and the Cochrane Library were searched by a senior information specialist. The population of interest included women becoming pregnant with ART (e.g., Intra

Monitoring quality in Israeli primary care: The primary care physicians' perspective.

BACKGROUND: Since 2000, Israel has had a national program for ongoing monitoring of the quality of the primary care services provided by the country's four competing non-profit health plans. Previous research has demonstrated that quality of care has improved substantially since the program's inception and that the program enjoys wide support among health plan managers. However, prior to this study there were anecdotal and journalistic reports of opposition to the program among primary care physicians engaged in direct service delivery; these raised serious questions about the extent of suppor

Measuring progress in pregnancy planning and preconception health.

As efforts to support pregnancy planning and improve preconception health are increasing at scale, appropriate systems to monitor progress are required. Despite developments in a few countries, no surveillance systems currently in operation are using a comprehensive set of indicators for monitoring preconception health. This Review describes relevant indicators, reflecting both system-level and individual-level factors, that can be drawn from routine data sources to form the basis for developing new surveillance systems. We present a new framework for national and international surveillance th
