Simulating Radial Pressure Waveforms with a Mock Circulatory Flow Loop to Characterize Hemodynamic Monitoring Systems.

PURPOSE: Mock circulatory loops (MCLs) can reproducibly generate physiologically relevant pressures and flows for cardiovascular device testing. These systems have been extensively used to characterize the performance of therapeutic cardiac devices, but historically MCLs have had limited use for assessing patient monitoring systems. Here, we adapted an MCL to include peripheral components and evaluated its utility for qualitative and quantitative benchtop testing of hemodynamic monitoring devices. METHODS: An MCL was designed to simulate three physiological hemodynamic states: normovolemia, c

Hemodynamic monitoring of intracranial collateral flow predicts tissue and functional outcome in experimental ischemic stroke.

Intracranial collaterals provide residual blood flow to penumbral tissue in acute ischemic stroke and contribute to infarct size variability in humans. In the present study, hemodynamic monitoring of the borderzone territory between the leptomeningeal branches of middle cerebral artery and anterior cerebral artery was compared to lateral middle cerebral artery territory, during common carotid artery occlusion and middle cerebral artery occlusion in rats. The functional performance of intracranial collaterals, shown by perfusion deficit in the territory of leptomeningeal branches either during

Transpulmonary thermodilution hemodynamic monitoring for pheochromocytoma surgery in a child with complex congenital heart disease.

Hemodynamic alterations related to pheochromocytoma surgery may lead to decompensation with an associated cardiac malformation. Moreover, they are incompletely described in children because of potential hazards of invasive monitoring. We report transpulmonary thermodilution hemodynamic monitoring during pheochromocytoma surgery in a child with a complex congenital heart malformation. Comprehensive analysis of hemodynamic events was obtained that guided therapeutic decisions. Cardiac function was preserved and no complication occurred.

The Hemodynamic Envelope, a novel aviation-inspired safety framework for personalized monitoring in cardiogenic shock.

Mortality in severe forms of cardiogenic shock (CS) remains above 50%, despite substantial advances in mechanical circulatory support strategies that have enabled clinicians to manage levels of patient complexity once thought unattainable. A key obstacle to effectively implementing and individualizing these sophisticated treatments lies in the continued reliance on outdated safety monitoring strategies that apply uniform cut-off values to a limited set of clinical variables. Such approaches fail to account for the dynamic and patient-specific nature of "safe" hemodynamics. To overcome this, we

Invasive hemodynamic monitoring-guided resuscitation improves survival in shock: A systematic review and meta-analysis.

BACKGROUND: Mortality in shock may reach 60%; hence, immediate, adequate resuscitation has a crucial role in improving outcomes. Detailed hemodynamic monitoring is desirable, but evidence on its outcome benefits is limited. Therefore, this study aimed to compare advanced hemodynamic monitoring (AHDM)-guided clinical decision-making and treatment with conventional ones in terms of outcomes in shock. METHODS: A systematic search was performed in three databases (PubMed, EMBASE, and Cochrane Library) until 9 November 2024. Randomised controlled trials, non-randomised and observational studies in

Less-Invasive Hemodynamic and Tissue Perfusion Monitoring in Sepsis and Septic Shock: A Narrative Review.

Sepsis and septic shock remain major causes of morbidity and mortality in critically ill patients. Hemodynamic management is a cornerstone of treatment, yet the optimal monitoring strategy to guide resuscitation is still debated. The progressive decline in the use of invasive techniques, such as pulmonary artery catheterization, has favored the development of less-invasive and non-invasive monitoring approaches. Recent technologies allow continuous assessment of cardiovascular function through arterial waveform analysis, non-invasive blood pressure monitoring, and predictive algorithms, while

Blood Pressure Optimization During Fetoscopic Repair of Open Spinal Dysraphism: Insights from Advanced Hemodynamic Monitoring.

Background/Objectives: Fetoscopic repair of open spinal dysraphism (OSD) is a rare intrauterine procedure performed in specialized fetal surgery centers. Conducted under restrictive fluid management and continuous tocolysis, it poses substantial challenges to maternal hemodynamic stability. Blood pressure optimization with vasopressor boluses is often required, yet intraoperative hemodynamic data remain limited. Methods: This prospective observational study was conducted between December 2023 and January 2025 during fetoscopic repair of OSD at Marburg University Hospital, Germany. Maternal hem

Intraoperative Cerebral Hemodynamic Monitoring during Carotid Endarterectomy via Diffuse Correlation Spectroscopy and Near-Infrared Spectroscopy.

Objective: This pilot study aims to show the feasibility of noninvasive and real-time cerebral hemodynamic monitoring during carotid endarterectomy (CEA) via diffuse correlation spectroscopy (DCS) and near-infrared spectroscopy (NIRS). Methods: Cerebral blood flow index (CBFi) was measured unilaterally in seven patients and bilaterally in seventeen patients via DCS. In fourteen patients, hemoglobin oxygenation changes were measured bilaterally and simultaneously via NIRS. Cerebral autoregulation (CAR) and cerebrovascular resistance (CVR) were estimated using CBFi and arterial blood pressure da

Cerebral hemodynamic monitoring of Parkinson's disease patients with orthostatic intolerance during head-up tilt test.

Significance: Monitoring of cerebral perfusion rather than blood pressure changes during a head-up tilt test (HUTT) is proposed to understand the pathophysiological effect of orthostatic intolerance (OI), including orthostatic hypotension (OH), in Parkinson's disease (PD) patients. Aim: We aim to characterize and distinguish the cerebral perfusion response to a HUTT for healthy controls (HCs) and PD patients with OI symptoms. Approach: Thirty-nine PD patients with OI symptoms [10 PD patients with OH (PD-OH) and 29 PD patients with normal HUTT results (PD-NOR)], along with seven HCs participate

Serial hemodynamic monitoring to guide treatment of maternal hypertension leads to reduction in severe hypertension.

OBJECTIVES: To examine whether treatment for hypertension in pregnancy that is guided by serial monitoring of maternal central hemodynamics leads to a reduction in the rate of severe hypertension, defined as blood pressure ≥ 160/110 mmHg; and to assess the distinct longitudinal hemodynamic profiles associated with beta-blocker monotherapy, vasodilator monotherapy and dual agent therapy, and their relationships with outcomes, including fetal growth restriction. METHODS: This was a prospective observational study at a dedicated antenatal hypertension clinic in a tertiary UK hospital. Fifty-two

Hemodynamic monitoring of the contralateral testis during unilateral testicular torsion describes the mechanism of damage.

Contralateral testicular perfusion during unilateral testicular torsion was evaluated using simultaneous blood flow and O2 content determinations. Two groups, each consisting of 7 rats, were studied. Sham operation or 720 degrees clockwise twisting was performed on the left testes, and blood flow, O2 content and temperatures were monitored in the right testes for 180 min. An ultrasonic perivascular Doppler flowmeter system, an electronic thermometer and an O2 electrode were used for the monitoring. The contralateral testicular blood flow and relative O2 contents were stable in the control grou

[Cesarean section, mitral valve disease and pulmonary hypertension. Implications of hemodynamic monitoring on anesthetic management].

We present the case of a 25-year-old woman with mitral valve disease and severe pulmonary hypertension scheduled for Caesarean section under general anaesthesia. General anaesthesia for such cardiac patients requires usually high doses of fentanyl prior to intubation, which favours maternal inhalation and neonatal ventilatory depression. Invasive haemodynamic monitoring allowed a rapid sequence induction, with an optimal drug titration and an early recovery.

Predictive Value of Intraoperative Transcranial Doppler Hemodynamic Monitoring for Cerebral Hyperperfusion Syndrome After Carotid Artery Stenting.

BACKGROUND: Cerebral hyperperfusion syndrome (CHS) is a potentially serious complication following carotid artery stenting (CAS). Conventional risk assessment methods may not adequately capture rapid intraoperative hemodynamic changes. Real-time transcranial Doppler (TCD) monitoring may provide valuable information for the early identification of patients at risk of CHS. AIMS: To investigate the predictive value of intraoperative real-time hemodynamic monitoring using TCD ultrasonography for CHS following CAS. METHODS: A total of 600 patients who underwent CAS were included. Factors associat

Embolization Followed by Self-Repositioning of a CardioMEMS™ Wireless Pulmonary Artery Hemodynamic Monitoring Sensor Device.

We report a case of early migration of a CardioMEMS™ heart failure device (Abbott) from the right to the left pulmonary artery, which then spontaneously repositioned itself within the left pulmonary artery without the need for intervention. A balloon Swan-Ganz catheter was then used to reposition the device, successfully lodging it into a more distal position. This case underscores the importance of close device monitoring and demonstrates an effective technique for repositioning without device retrieval.

Artificial Intelligence-Driven Hemodynamic Monitoring of Simulated Bruxism Using Functional Near-Infrared Spectroscopy: A Preliminary Study.

BACKGROUND: Sleep-related and neuromuscular conditions affect the daily lives of individuals as they impact physical and cognitive well-being. While not classified as a disorder, bruxism has emerged as a prevalent condition characterized by involuntary teeth grinding and jaw clenching, occurring either during sleep or wakefulness. Often left unnoticed, this unconscious behavior can contribute to severe dental damage, facial muscle fatigue, and temporomandibular joint disorders. These consequences require early detection and intervention to prevent long-term complications. Traditionally, polyso

Association Between Hemodynamic Monitoring Need and Prognosis of Patients with Sepsis in the Intensive Care Unit.

Sepsis, a life-threatening disease characterized by organ dysfunction, is primarily caused by an abnormally regulated host response to infection. This study aimed to evaluate the association between hemodynamic monitoring (HM) and the prognosis of patients with sepsis in the intensive care unit (ICU). This retrospective analysis used data from the MIMIC-IV database (2008-2019) at a tertiary medical center in Boston, focusing on adult patients with sepsis. Hemodynamic monitoring was conducted using PICCO. The primary outcome was in-hospital mortality, and the secondary outcomes were ICU mortali

Advanced Markers for Hemodynamic Monitoring in Cardiogenic Shock and End-Stage Heart Failure: A Mini Review.

Right heart catheterization (RHC) provides critical hemodynamic insights by measuring atrial, ventricular, and pulmonary artery pressures, as well as cardiac output (CO). Although the use of RHC has decreased, its application has been linked to improved outcomes. Advanced hemodynamic markers such as cardiac power output (CPO), aortic pulsatility index (API), pulmonary artery pulsatility index (PAPi), right atrial pressure to pulmonary capillary wedge pressure ratio (RAP/PCWP) and right ventricular stroke work index (RVSWI) have been introduced to enhance risk stratification in cardiogenic shoc
