OBJECTIVE: Total hip arthroplasty with a minimal-incision technique that can be performed in the widely used supine position. The accustomed and good overview of this position allows safe positioning of the implant and combines this with the advantage of a soft tissue preserving technique. All standard instruments and implants can be further applied.
INDICATIONS: Primary and secondary coxarthrosis, femoral head necrosis.
CONTRAINDICATIONS: Revision surgery, severe anatomic deformity, implantation of hip resurfacing arthroplasty.
SURGICAL TECHNIQUE: Supine position. The skin incision runs fr

The article presents the case of a 55-year-old patient with an injury of the right leg, which was due to a falling lightly at her buttocks during squatting. Patient was transferred to hospital with complaints of severe pain and limited motion in her right hip. In view of the patient's clinical status and following radiological diagnostic work-up, she was diagnosed with femoral neck fractures, the decision was made to perform a primary total hip arthroplasty (THA) using an lateral approach in the lateral decubitus position. A good clinical and radiological outcome was achieved. Patient after 12

Recurrent dislocation of a total hip prosthesis can be a challenging and often disappointing problem. This case report describes a 78-year-old woman who had recurrent posterior dislocations of a revision total hip replacement (THR) that occurred on flexion and internal rotation of the hip. The initial THR for posttraumatic osteonecrosis and extensive revision THA were performed through an anterolateral approach. Conservative treatment was not successful in preventing posterior re-dislocation, neither was open surgery in which offset was improved. Investigation under anesthesia by means of an i

Infection of a total hip prosthesis by Mycobacterium tuberculosis (MT) is an uncommon complication. We report a patient with septic loosening of a total hip prosthesis with a presumptive diagnosis of MT infection. His previous history was negative to any form of tuberculosis, and there was no evidence of a primary focus on the radiological study of the chest. He was diagnosed initially as presenting aseptic loosening and revision surgery was performed. Six months after surgery, septic loosening was diagnosed. The prosthesis was then removed. The histological study showed caseum granulomas and

We performed a clinical and radiological analysis to evaluate the significance of the stem position of cementless CLS components with respect to function and survival. We used patient material from a multi-center study of 3,732 CLS stems with a mean follow-up of 43 months (range: 12-142 months). The clinical outcome was assessed using Merle D'Aubigné Scores and stem alignment was determined as the deviation of the longitudinal stem axis from the longitudinal femur axis. We found no correlation between stem alignment and function, survival, implant migration or periprosthetic radiolucent lines.

PURPOSE OF THE STUDY: Aseptic loosening of the acetabular component is the most worrying problem after hip arthroplasty. During revision surgery we prefer to rebuild a solid bony acetabulum close to the anatomy in which the implant will be cemented. On the basis of the first 48 acetabular reconstructions using deep-frozen bony allografts, we carried out a review of our results in a pathology which will surely increase in the future.
MATERIAL: 48 hips were operated according to this technique. It has been possible to review 38 of them, with an average follow-up of 7.3 years (extremes 5 years,

At present, revision is the only standard option for cases of recurrent hip prosthesis dislocation. A device is described that contains the head of the femoral component, preventing dislocation, while only slightly restricting external rotation of the affected leg. The device can be inserted through a small incision, minimizing operation time. This device differs from others, e.g., the reversed acetabular cup, by being predrilled (with drill guide supplied), modular in design, and tailored to the dimensions of the cup site.

The present paper reports on the results of a specific scintigraphic examination method for diagnosing infections in cases of loosening of hip prostheses by means of labeled granulocytes. In 103 patients the result of the examination was compared with the result of the bacteriological and/or histological investigation following removal or replacement of the implant. Where there was an infectious loosening of the prosthesis the findings were in agreement with the results of the scintigraphic examination in 83.1% of the cases. It was possible to rule out infectious loosening of the prosthesis in

Using a titanium alloy femoral prosthesis (STH Zimmer) and an ultra high molecular weight polyethylene acetabular cup with a posterior surgical approach to the hip joint, 237 consecutive total hip arthroplasties in 215 patients were performed between December 1975 and May 1977. The preliminary results and early postoperative complications suggest that this system can be considered an alternative to total hip arthroplasty using other materials and surgical approaches.

The results of 189 Brunswik total hip replacements, performed at the Orthopaedic Hospital of the Invalid Foundation, Helsinki, Finland, and followed up for 2-3 years, are presented. According to the modified system of Merle D'Aubigne & Postel excellent results were achieved in 11.1 per cent (21/189), good in 56.6 per cent (107/189) and fair in 27.5 per cent (52/189). The failure rate was 4.8 per cent (9/189) and consisted of four deep infections, four cases of aseptic loosening and one case of allergic loosening. Local complications occurred after primary operations in 7.5 per cent (12/159) an

Background Revisions of hip prostheses are increasing, and conventional radiography (CR) is a primary tool for managing -complications. However, dual-energy CT (DECT) with virtual monoenergetic imaging is capable of reducing periprosthetic metal artifacts compared with standard CT. Purpose To compare the diagnostic performance of DECT and CR in detecting hip prosthesis loosening, using surgery as a reference for diagnosis. Materials and Methods This retrospective single-center study conducted between January 2018 and October 2020 included -consecutive patients with unilateral painful hip prost

The trend in arthroplasty of the hip joint to implement new models is partly based on theoretical considerations. In order to verify to which extent the philosophy of individual models is ultimately successful, the presentation of long-term results is required. In the years 1991 and 1992, 433 patients with primary implantation of an uncemented total hip replacement in primary coxarthrosis with a stem type AML (anatomic medullary locking) were treated surgically. 283 of them got a cementless cup type Duraloc. In 311 (71.8 %) patients the mean survival rate of the prosthesis could be determined

Recurrent hip dislocation has become an increasing problem in patients who had multiple component revision in total hip arthroplasty. These are often frail elderly patients with loss of constraining soft tissue envelope around the hip and an excessive weakness of the abductor mechanism. We present a simple technique for open reduction of a dislocated hip using a custom-made fully constrained construct with potential for preserving the well fixed femoral implant and parts of the acetabular component. The custom-made constrained construct consists of the application of an inverted cup over the f

OBJECTIVES: O. Sacks describes in his book "A leg to stand on" the inability to move his leg for along period after rupture of the tendon of m. quadriceps. He refers the history of similar cases, especially of a woman with a hip fracture. He discusses this as a result of disruption of proprioception (peripheral induced neglect). The aim of this investigation is to answer the question how frequently this symptom may be found in patients with hip fracture and/or arthrosis perioperativly after implantation of a total prosthesis.
METHODS: we examined 106 patients aged 70.35 +/- 10.01 years (36-ma

The aim of this study is to determine the differences between hip fracture and hip arthrosis groups and to assess pain related fear of injury in patients who were operated using the TPP following hip fracture or hip arthrosis. Fifty-eight patients (mean age = 63.9 ± 10.3 years) who were operated using the TPP, following hip fracture (hip fracture group; n = 25) or coxarthrosis (coxarthrosis group; n = 33) were recruited. All of the measurements were performed after a follow-up time of at least 2 years. Functional level by Harris Hip Scoring System (HHS), pain related fear by Tampa Scale for Ki

[Purpose] We report a case of an elderly patient with rheumatoid arthritis who underwent hip disarticulation because of necrotizing fasciitis and regained the ability to walk independently with a prosthetic limb. [Participant and Methods] A 61-year-old female patient underwent right hip disarticulation due to severe necrotizing fasciitis of the right lower limb. Her chief complaint was that she was not able to walk inside her house or outdoors to perform instrumental activities of daily living. We applied a Canadian-type hip disarticulation prosthesis to the stump. The patient received in-hosp
