Difficulty discarding is the core behavioral symptom of hoarding disorder (HD). Patients with HD report greater subjective distress when discarding their own possessions as compared to others' possessions. To date, no prior studies have examined psychophysiological activation, an objective measure of anxious arousal, during discarding among individuals with HD. The current study assessed psychophysiological responses during a baseline resting period and two discarding tasks, one involving personal possessions and the other involving matched control ("experimenter-owned") items in 52 patients w

Although some studies have reported a relationship between hoarding and bipolar disorder, we are unaware of any previous description of how they may interact with each other and how they should be managed appropriately. A 48-year-old male depressed patient with hoarding symptoms and obsessive-compulsive disorder (OCD) was diagnosed with bipolar II disorder after 2 hypomanic episodes. The patient was treated unsuccessfully with different high-dose serotonin reuptake inhibitors and atypical antipsychotics, maintaining a pattern of 6 to 8 discrete, but severe, depressive episodes each year, alway

Introduction: Aberrant salience has been extensively studied in relation to various psychopathologies, such as psychosis. However, its role in hoarding is yet to be explored, despite evidence of associations between the development of hoarding and dysregulation in dopaminergic systems. Thus, the aim of this pre-registered study was to examine associations between self-reported experiences of aberrant salience and hoarding severity in a community sample. Methods: A correlational, cross-sectional design was employed. We presented an online survey to 214 adults (Mean age = 25.47, SD = 6.55), where

Individuals with hoarding disorder (HD) have difficulty parting with personal possessions, which leads to the accumulation of excessive clutter. According to a proposed biphasic neurobiological model, HD is characterized by blunted central and peripheral nervous system activity at rest and during neutral (non-discarding) decisions, and exaggerated activity during decision-making about discarding personal possessions. Here, we compared the error-related negativity (ERN) and psychophysiological responses (skin conductance, heart rate and heart rate variability, and end tidal CO2) during neutral

BACKGROUND: Hoarding disorder is characterized by a persistent inability to part with possessions due to a perceived need to keep them, regardless of their actual value. Arabic-speaking populations currently lack a validated tool specifically designed to assess hoarding symptoms.
OBJECTIVE: This study aimed to translate, adapt, and validate the Hoarding Rating Scale-Self Report (HRS-SR) into the Arabic-language.
METHODS: The study employed the gold standard approach to translation, involving forward translation by independent translators and back translation review. We conducted a cross-sect

Hoarding disorder, also known as compulsive hoarding, comes under the umbrella term of obsessive-compulsive disorder and related disorders. The constant building of clutter in the house of hoarders makes it impossible for family members to live a healthy life. It can have appalling effects on their mental health and can lead to severe depression and suicidal ideation. The shame and humiliation attached to hoarding does not allow the hoarders to seek help, causing them distress and hence continuing the vicious cycle of hoarding. By presenting the case of a patient with a spouse with a hoarding

Insight impairment contributes significantly to morbidity in psychiatric disorders. The neurologic concept of anosognosia, reflecting deficits in metacognitive awareness of illness, is increasingly understood as relevant to psychopathology, but has been little explored in psychiatric disorders other than schizophrenia. We explored anosognosia as an aspect of insight impairment in n = 71 individuals with DSM-5 hoarding disorder. We used a standardized clutter severity measure to assess whether individuals with hoarding disorder underreport home clutter levels relative to independent examiners.

Hoarding disorder (HD) is characterised by compulsive acquisition and extreme difficulty discarding possessions, resulting in clutter that substantially impacts upon functioning. Heightened object attachment is a central feature of HD according to prominent theoretical models. We review current research on the nature and function of object attachment in HD. In particular, we describe growing evidence that object attachment in HD is a compensatory response to unmet relatedness needs, and recent research underscoring the insecure nature of object attachment in hoarding. Critical gaps include the

Hoarding disorder is characterized by difficulty discarding objects and excessive clutter. The relationship between hoarding and claustrophobia, reactions to severely cluttered spaces, and clutter preferences are all areas that are yet to be investigated. The present study used a novel virtual reality (VR) platform to examine these domains. Two groups (i.e., with hoarding disorder, n = 36; without hoarding disorder, n = 40) similar in age and gender were recruited from the community. There were no differences in subjective or physiological reactivity to increasing VR clutter levels. The hoardi

BACKGROUND: Hoarding is a disorder characterized by excessive acquisition and persistent difficulty in discarding possessions. The behaviour has adverse emotional, physical, social, financial, and legal outcomes for the person with the disorder and family members, and might pose a significant public health problem. Hoarding has been included as a distinct disorder in the Diagnostic and Statistical Manual of Mental Disorders Fifth edition (DSM-5). The prevalence of hoarding disorder is approximately 2-6% globally. The current state of the evidence does not offer clear understanding of the cause

Predictions made by the "incentive hope" hypothesis account for overconsumption in unpredictable food environments. However, when applied to uncertain gambling situations, there are several areas where this theory falls short. Most notably, it has trouble explaining why, in slot machine gambling, players are motivated by extended play to spend time trying to resolve uncertainty, rather than hoarding monetary gains.

BACKGROUND: Over the past decade, functional neuroimaging studies have found abnormal brain function in several cortical systems when patients with compulsive hoarding behaviors make decisions about personal possessions. The purpose of this study was to use functional magnetic resonance imaging to test a neurobiological model of hoarding disorder (HD) that has begun to emerge from these small studies by confirming HD-related brain dysfunction in previously implicated brain regions in the largest sample of HD patients examined to date.
METHODS: We compared 79 adults diagnosed with DSM-5 HD wit

Until recently, compulsive hoarding has been treated as a type of obsessive-compulsive disorder with mixed results. Little research exists on the efficacy of behavioral interventions specifically designed to treat hoarding disorder, and most existing research is limited with regard to the numbers of participants, their ethnic and cultural diversity, and study replication; therefore, the generalizability of findings is limited. This article reviews the prevalence of hoarding disorder, cognitive behavioral therapy treatment approaches, and measurement of symptoms. A systematic review compares th

BACKGROUND: Hoarding is defined as the excessive collection and failure to discard possessions of apparently little value, leading to clutter, distress, and disability. Although patients with hoarding typically may feel ludicrous for not discarding useless, and sometimes bizarre, possessions, we are not aware of any previous description of patients displaying hoarding as a result of social anxiety.
METHOD: Single case report.
RESULTS: In this article, we describe a patient with severe social anxiety disorder who displayed hoarding as a direct consequence of social anxiety-related beliefs and

OBJECTIVES: Compulsive hoarding is a debilitating, chronic disorder, yet we know little about its onset, clinical features, or course throughout the life span. Hoarding symptoms often come to clinical attention when patients are in late life, and case reports of elderly hoarders abound. Yet no prior study has examined whether elderly compulsive hoarders have early or late onset of hoarding symptoms, whether their hoarding symptoms are idiopathic or secondary to other conditions, or whether their symptoms are similar to compulsive hoarding symptoms seen in younger and middle-aged populations. The current gold standard treatment for Hoarding Disorder (HD) is cognitive behavioral therapy (CBT). However, typical symptom reduction is 25% and only about one third of individuals achieve a clinically meaningful response. We used machine learning (ML) to identify predictors of treatment response using data from a non-inferiority trial of clinician vs peer-led group CBT for HD. Assessments were completed for 323 individuals with HD at post-treatment (N = 323) and at one-year follow-up (N = 183). Assessments included neuropsychological testing, symptom severity and psychiatric diagnoses, bel

Hoarding occurs relatively frequently in obsessive-compulsive disorder (OCD), and there is evidence that patients with hoarding symptoms have more severe OCD and are less responsive to treatment. In the present study, we investigated hoarding symptoms in 126 subjects with OCD. Nearly 30% of the subjects had hoarding symptoms; hoarding was twice as prevalent in males than females. Compared to the 90 non-hoarding subjects, the 36 hoarding individuals had an earlier age at onset of, and more severe, obsessive-compulsive symptoms. Hoarders had greater prevalences of symmetry obsessions, counting c

This study characterizes the demographic features, medical history, autopsy findings, toxicological results, and causes of death of decedents with evidence of hoarding behavior. The results were compared with three methodologically comparable studies. Inclusion criteria comprised adult decedents examined at the Institute for Legal Medicine in Frankfurt/Main,Germany whose case records mentioned hoarding or domestic squalor. In a 15-year period, 161 cases met the inclusion criteria. Of the decedents, 24.2% were female, 75.8% were male. The mean age at death was 57.8 years. Physical comorbidities
