OBJECTIVE: Hoarding disorder (HD) is difficult to treat. In an effort to increase efficacy and engagement in cognitive-behavioral therapy (CBT), we developed and evaluated a novel intervention comprising group CBT combined with between-session Internet-based clinician support for people with HD.
METHOD: Twenty participants with HD received group CBT combined with an Internet-support system enabling therapist-participant communication between group sessions.
RESULTS: The treatment was associated with a significant reduction on the Saving Inventory-Revised (SI-R) and a large effect size (Cohen

This study estimated the prevalence of hoarding disorder (HD) in individuals seeking help from Eviction Intervention Services Housing Research Center (EIS), a not-for-profit community organization in New York City (NYC) that aids clients with housing problems including eviction. One hundred fifteen EIS clients were screened for HD. The prevalence of HD among those seeking help from EIS was 22% (clinician-rated) and 23% (self-rated), which is nearly 5 to 10 times greater than the rate of hoarding (2% to 5%) in the general population. Of individuals seeking help from EIS who met the criteria for

BACKGROUND: Hoarding disorder (HD) can be understood through the cognitive behavioural model in the context of vulnerability factors (for example, personality traits, co-morbidities, traumatic life events) and beliefs about possessions (for example, identity, emotional attachment, memory, utility). Less is known about the strength of these hypothesised beliefs, or how they interact within the hoarding population, with researchers suggesting that specifying beliefs would improve treatment outcomes.
AIM: The current study explored beliefs in HD, utilising Q-methodology to explore both categorie

In 2013, hoarding disorder was officially recognized as a separate Diagnostic and Statistical Manual psychiatric diagnosis after years of debate. Prior to 2013, hoarding disorder was generally considered a subset of obsessive-compulsive disorder. Though modern medicine has only recently deepened the analysis of hoarding disorder, hoarding was regularly featured as a character trait in numerous European literary works dating back over 700 years. Several prominent European writers incorporated hoarding behavior in the fictional characters they created. Each author's individual social and economi

Despite vulnerability and unsanitary conditions of animal hoarding may predispose environmental contamination and spread of vectors and pathogens, no study to date has focused on their impact on public health and zoonotic diseases. Accordingly, this study aimed to assess the seroprevalence of anti-Toxoplasma gondii antibodies and associated factors in individuals with animal hoarding disorder (AHD) and their dogs in Curitiba, Southern Brazil. Blood samples were obtained from 264 dogs (21 households) and 19 individuals with AHD (11 households). Their blood was tested by indirect fluorescent ant

BACKGROUND: Group cognitive-behavioural therapy (G-CBT) for hoarding disorder (HD) may be an intervention of choice, considering its efficacy, low costs, and impact on comorbid anxiety and depression. But although G-CBT and modifications of G-CBT have been applied, none has assessed G-CBT efficacy at follow-up. In the current open-label pilot study, we tested the efficacy of G-CBT at posttreatment and 6-month follow-up and whether the inclusion of targeted reasoning and self-identity components added to G-CBT efficacy.
METHODS: Participants (n = 16) with the HD according to the DSM-5 criteria

Childhood trauma, cold and controlling parenting experiences, and attachment difficulties are common in individuals with hoarding disorder (HD). One theory suggests that strong attachment to objects may arise as a compensatory response for early thwarted relatedness needs. However, first-line cognitive behavioural therapies (CBT) for HD rarely address these early adverse experiences during treatment. This study evaluated the feasibility, acceptability, and preliminary efficacy of an imagery rescripting intervention targeting early aversive memories in 10 individuals diagnosed with HD (Mage = 6

OBJECTIVE: The Buried in Treasures (BIT) workshop is a promising treatment for hoarding disorder (HD), though many participants struggle with home uncluttering. This randomized waitlist-controlled trial investigated the efficacy of a version of BIT, augmented with in-home uncluttering practice (BIT+).
METHOD: Adults (N = 41) with hoarding disorder were recruited from the community and randomly assigned to BIT+ or waitlist. BIT+ consisted of 16 sessions of the BIT workshop and 10 uncluttering home visits over 18 weeks. Outcome measures included the Saving Inventory-Revised (self-report) and th

The prevalence of disordered eating is reportedly high in people with hoarding behaviours, and both eating disorders (ED) and hoarding disorder (HD) show strong associations with anxiety/depression. There is also some evidence that both may be underpinned by aspects of perfectionism. In a non-clinical sample of 252 adults (187 females) we explored possible relationships between disordered eating and hoarding severity, and assessed the relationships between hoarding, eating disorders and perfectionism. After controlling for gender and mood (anxiety and depression) we found significant positive

Grit is associated with positive outcomes in nonclinical samples. However, no studies have examined grit in relation to psychopathology in patients with clinical mood or anxiety disorders. Research and clinical experience suggest that individuals who hoard struggle with characteristics associated with grit, such as task persistence, impulsivity, and self-control. The authors tested the hypothesis that hoarding symptoms are associated with less grit in a sample of individuals (N = 72) presenting for treatment to an anxiety disorders clinic. After covarying symptoms of the four mood and anxiety

BACKGROUND: It is suggested that the different psychological vulnerability factors of intolerance of uncertainty (IU), anxiety sensitivity (AS) and distress tolerance (DT) may be in important in hoarding disorder (HD). However, the extent to which these factors are specific to HD compared with other disorders remains unclear.
AIMS: The current study aimed to investigate differences in IU, AS and DT in three groups: HD (n=66), obsessive compulsive disorder (OCD; n=59) and healthy controls (HCs; n=63).
METHOD: Participants completed an online battery of standardised self-report measures to est

Hoarding disorder (HD) is a new psychiatric diagnosis in Diagnostic and Statistical Manual of Mental Disorders-Fifth edition and preliminary evidence suggests that cognitive-behavioral treatments are effective in treating this condition. However, it has been demonstrated that individuals with HD generally display poor compliance during treatment, which may lead to poor outcomes. Treatment compliance can be conceptualized as either within-session or between-session compliance, but currently there are no validated measures of within-session or between-session compliance specifically for HD. We investigated the presentation and correlates of hoarding behaviors in 204 children aged 7-13 with autism spectrum disorder (ASD) and comorbid anxiety or obsessive-compulsive disorder (OCD) symptoms. Approximately 34% of the sample presented at least moderate levels, and with 7% presenting severe to extreme levels of hoarding. Child gender predicted hoarding severity. In addition, child ASD-related social difficulties together with attention-deficit and hyperactivity disorder symptom severity positively predicted hoarding controlling for child gender and restricted and repetitive behaviors.

BACKGROUND AND OBJECTIVE: Obsessive-compulsive disorder (OCD) and hoarding disorder often co-occur but differ in onset, course, symptoms, and treatment response. This study compared diagnosis-based OCD genetic liability with hoarding-symptom liability, rather than direct expression differences between diagnosed patient groups.
METHODS: Large-scale OCD and hoarding genome-wide association study (GWAS) summary statistics were integrated with brain expression quantitative trait loci (eQTL) reference panels using a transcriptome-wide association approach. Gene-set enrichment was assessed across s

PURPOSE: Hoarding Disorder (HD) is characterized by persistent difficulty discarding possessions, leading to excessive accumulation and functional impairment. Traditional treatments, based on cognitive-behavioral therapy (CBT), emphasize motivational enhancement, cognitive restructuring, and exposure-based interventions. However, these methods face challenges, including heightened distress during in vivo exposure and avoidance tendencies. Virtual Reality (VR) has emerged as a promising adjunctive tool, offering immersive, controlled environments for gradual exposure. The current preliminary st

BACKGROUND: Individuals with hoarding disorder (HD) presented a persistent difficulty in detaching from objects and/or animals. Unhealthy conditions, frequently found in cases of animal HD (AHD), may favour environmental contamination and the spread of zoonotic pathogens. Despite that, only one study of zoonotic diseases in individuals with AHD and their companion animals has been conducted to date.
OBJECTIVES: This study aimed to assess the seroprevalence of anti-Leptospira spp. antibodies in individuals with AHD and their dogs in a major city of Southern Brazil.
METHODS: Blood samples were

The COVID-19 pandemic has necessitated the use of video-teleconferencing (VTC) for psychological treatments but VTC effectiveness studies are sparse. We examined treatment outcomes for a modified Buried in Treasures (BIT) group program for hoarding disorder (HD) delivered via VTC. Participants were 10 individuals with HD. Hoarding severity was evaluated at pre, mid, post, and six-month follow-up. Results showed significant decrease in hoarding symptoms over time (with an average decrease of 32%). The dropout rate was low (9%) and 30% of participants were fully recovered at follow-up. These res

BACKGROUND: Hoarding symptoms are associated with functional impairment, though investigation of disability among individuals with hoarding disorder has largely focused on clutter-related impairment to home management activities and difficulties using space because of clutter. This analysis assesses disability among individuals with hoarding symptoms in multiple domains of everyday functioning, including cognition, mobility, self-care, interpersonal and community-level interactions, and home management. The magnitude of the association between hoarding and disability was compared to that of me

OBJECTIVES: The aim of this study was to explore the importance of possessions as extensions of self-concept and connections to significant others for people that hoard.
DESIGN: A total of 10 participants were recruited through hoarding group treatment programmes. Participants photographed significant possessions in their home and discussed these in semi-structured interviews.
RESULTS: Analysis of transcripts using interpretative phenomenological analysis revealed three interrelated superordinate themes: (1) time travels with you; (2) possessions are me, they're a reflection of my life; and

BACKGROUND: Cognitive behavioral therapy (CBT) is a moderately efficacious treatment for hoarding disorder (HD), with most individuals remaining symptomatic after treatment. The Joining Forces Trial will evaluate whether 10 weeks of in-home decluttering can significantly augment the outcomes of group CBT.
METHODS: A randomized controlled trial of in-home decluttering augmentation of group CBT for HD. Adult participants with HD (N = 90) will receive 12 weeks of protocol-based group CBT for HD. After group CBT, participants will be randomized to either 10 weeks of in-home decluttering led by a
