[Role of psychiatry in prevention of accidental falling in the aged].

The high incidence of falling among the psychogeriatric patients has recently been acknowledged. Elderly patients with depression and dementia, and long-term hospitalized schizophrenic patients are particularly prone to falling. By sustaining falling, these patients suffer from not only injuries including femoral neck fracture but also consequent fall-phobia. As the results, quality of life and the prognosis of psychiatric illnesses of the fallers are affected. In order to prevent fallings, we must pay attention to the prescribed drugs, nursing issues and environmental aspects of wards. Muscle

An endoluminal method of hemorrhage control and repair of ruptured abdominal aortic aneurysms.

PURPOSE: To report our initial experience with endovascular grafting to treat ruptured abdominal aortic aneurysms (AAAs). METHODS: Three consecutive patients with severe comorbid illnesses and symptoms of aneurysm rupture and hemodynamic instability were treated with aortomonoiliac grafts. The Z-stent-based devices were implanted with the assistance of an occlusion balloon placed in the distal descending thoracic aorta. RESULTS: All patients survived the procedure with successfully excluded AAAs. Two patients had relatively short hospital stays (4 and 14 days), while the third required prolo

Skatepark injuries and the influence of skatepark design: a one year consecutive case series.

Skateboarding and in-line skating are popular activities that attract millions of participants yearly and have spawned the development of skateparks (SPs). This study examined the types and distribution of injuries within a local commercial SP. Subjects were prospectively enrolled in a university Emergency Department (ED) after an injury at a local SP. One hundred subjects were enrolled over 1 year, representing 102 episodes and 106 injuries. Musculoskeletal injuries accounted for 80% of all injuries. Fractures and dislocations were the most frequent injuries (59%); six required operative mana

Patient Perspectives on Open-Door Policies in Psychiatry: Mixed Methods Study.

BACKGROUND: Open-door policies in psychiatric wards are increasingly recommended as a means to reduce coercion and enhance patient autonomy. However, evidence that integrates patient perspectives on ward openness and related safety measures remains limited. Traditional qualitative approaches often lack the breadth to fully capture the complexity of these views. We hypothesized that patients would prefer open-door treatment and hold a critical view of locked-ward environments, emphasizing autonomy and dignity in care. OBJECTIVE: This study aims to systematically explore psychiatric patients' p

Laparoscopic surgery in a mobile army surgical hospital deployed to the former Yugoslavia.

The 212th Mobile Army Surgical Hospital was deployed from a United States military base in Germany to the former Yugoslavia to provide medical support for more than 25,000 United Nations soldiers. This medical unit was the first mobile hospital ever to deploy with a capability for laparoscopic surgery. During a 2-month trial period, seven laparoscopic procedures were performed for various abdominal emergencies. There were no complications, and all patients returned to full duty within 1 week. No equipment problems or other logistical obstacles were identified. This preliminary report suggests

Analysis of nosocomial acquired Clostridium difficile infection in an Italian research and teaching hospital.

BACKGROUND: Clostridium difficile (Cd) infection is a nosocomial plague which is correlated with several clinical and medical factors such as antibiotics intake. It is known that prevention is possible through infection control measures both clinical and epidemiological. METHODS: We examined the data from a study about Cd infection in four internal medicine wards in a teaching and research hospital in the north part of Italy in a two years period. The wards are only slightly different in size, plan, structures, nursing staff and patient's characteristics but have a different room' organizatio

Investigation of the Relationships Between Clinical and Environmental Isolates of Aspergillus fumigatus by Multiple-locus Variable Number Tandem Repeat Analysis During Major Demolition Work in a French Hospital.

BACKGROUND: Genotyping is needed to explore the link between clinical cases from colonization of invasive aspergillosis (IA) and major building construction. Attempts to correlate Aspergillus fumigatus strains from clinical infection or colonization with those found in the environment remain controversial due to the lack of a large prospective study. Our aim in this study was to compare the genetic diversity of clinical and environmental A. fumigatus isolates during a demolition period. METHODS: Fungal contamination was monitored daily for 11 months in 2015. Environmental surveillance was und

Institutional risk factors for outbreaks of nosocomial gastroenteritis: survival analysis of a cohort of hospital units in South-west England, 2002-2003.

Nosocomial outbreaks of gastroenteritis are a major burden on hospital inpatient services, costing an estimated pound115 million annually to the English National Health Service. We actively followed-up 171 inpatient units from four major acute hospitals and 11 community hospitals in South-west England for one year. Outbreaks of gastroenteritis were ascertained through an active surveillance network using standard clinical definitions. Survival analysis Cox regression models using an outbreak of gastroenteritis as the endpoint were fitted to identify institutional and operational attributes rel

A proposed improvised pleural drainage system for veterinary use.

BACKGROUND: Active pleural suction devices may be required for continuous drainage of pleural fluid or air. Such devices may not always be available or economical to stock at all veterinary hospitals. Three designs of pleural drainage systems adapted from standard suction canisters are proposed. KEY FINDINGS: The designs were constructed from readily available materials from the veterinary hospital and consist of (1) a 1-bottle system intended for use with pneumothoraces, (2) a 2-bottle system, which can be used for draining air and fluid, and (3) an abbreviated 2-bottle system. The suction p

Invasive pulmonary aspergillosis in neutropenic patients during hospital construction: before and after chemoprophylaxis and institution of HEPA filters.

Between September 1993 and December 1993, during extensive hospital construction and indoor renovation, a nosocomial outbreak of invasive pulmonary aspergillosis occurred in acute leukemia patients treated in a regular ward that has only natural ventilation. The observed infection rate was 50%. Chemoprophylaxis with intravenous continuous low-dose amphotericin B was then instituted as a preventive measure. During the next 18 months invasive pulmonary aspergillosis developed in 43% of acute leukemia patients. After that period a new hematology ward was opened with an air filtration system throu

Noise and the premature infant: physiological effects and practice implications.

Premature infants in the neonatal intensive-care unit often are exposed to continuous loud noise. This excess auditory stimulation creates negative physiological responses, such as increased heart and respiratory rates and decreased oxygen saturation. Modifying the neonatal intensive-care unit to reduce noise levels provides an environment more conducive to maturation and recovery from illness. This article reviews the literature addressing the pathophysiology and physiological changes of the premature infant in response to stress and discusses practice interventions for decreasing noise level

The "no needles" dilemma: Mothers' experiences in supporting children aged 3 to 6 years in coping with hospital fear.

PURPOSE: Hospital experiences are a major source of fear for preschool children due to developmental limitations in understanding medical procedures. Mothers play a central role in mediating these experiences while managing their own emotional burden. This study aimed to explore mothers' perspectives and meaning-making processes in supporting children aged 3 to 6 years in coping with hospital fear. DESIGN AND METHODS: This qualitative study was conducted within a social constructionist and interpretivist paradigm, adopting a pure reflexive thematic analysis design. Sixteen mothers of children
