Erythrocyte Alloimmunization and Transfusion Strategies in Sickle Cell Disease: A Single-Center Analysis.

AIM: Alloimmunization (the production of antibodies against foreign red blood cell (RBC) antigens) is a significant complication in patients with sickle cell disease (SCD) who require chronic transfusion. This retrospective study examined the distribution of ABO and Rh phenotypes in SCD patients at Dr. Soliman Fakeeh Hospital in Jeddah (DSFH-J) and their implications for alloimmunization risk. The high immunogenicity of the K antigen in the Kell system, second only to that of the D antigen in the Rh system, makes it a frequent target. RESULTS: Among 241 patients with SCD, the most common bloo

Evaluation of the traditional and revised World Health Organization classifications of dengue cases in Brazil.

OBJECTIVE: Dengue is a worldwide public health problem with approximately 50 million cases reported annually. The World Health Organization proposed a revised classification system in 2008 to more effectively identify the patients who are at increased risk of complications from dengue. Few studies have validated this new classification system in clinical practice. We conducted a cross-sectional study of patients hospitalized for dengue in Dourados, Mato Grosso do Sul, Brazil, to evaluate the capacity of the two classification systems for detecting severe cases of dengue. A proactive approach to prevention of health care-acquired Legionnaires' disease: the Allegheny County (Pittsburgh) experience.

BACKGROUND: The Allegheny County Health Department (ACHD) in Pennsylvania distributed the first guidelines for prevention and control of health care-acquired Legionnaires' disease (LD) by 1995. The proactive approach advocated in the guidelines differed notably from that of the Centers for Disease Control and Prevention (CDC) by recommending routine environmental testing of the hospital water distribution system even when cases of health care-acquired Legionnaires' disease had never been identified. OBJECTIVES: Our purpose was to (1) evaluate the impact of the ACHD guidelines on the Legionell

Crisis management in Finnish hospital pharmacies during the COVID-19 pandemic.

BACKGROUND: Although hospital pharmacies have played a central role in managing the COVID-19 pandemic, there is a lack of crisis management theory-based empirical research on the topic. The purpose of this study was to fill this gap in the Finnish context and identify areas for development to improve future crisis preparedness. METHODS: A national cross-sectional survey was developed based on crisis management process models and sent to all hospital pharmacy heads (n = 21) during the second wave of the pandemic in October-November 2020. Descriptive statistics were calculated, and qualitative

Persistent Legionella pneumophila colonization of a hospital water supply: efficacy of control methods and a molecular epidemiological analysis.

After a nosocomial outbreak caused by Legionella pneumophila serogroup 5, the hospital water distribution system, which was found to be colonized by L. pneumophila serogroups 5 and 6, was decontaminated by the superheat and flush method and by installing an additional heat-shock unit in one of the hot water circuits. This unit exposed the recirculated water to a temperature of 80 degrees C. The efficacy of the decontamination measures was evaluated by monitoring the temperatures and legionella concentrations at different parts of the hot water distribution system. The genetic diversity of the

Urinary tract infections in the surgical patient.

Urinary tract infection (UTI) continues to be a common nosocomial infection. From a 2-year city-county hospital experience, 212 nosocomial UTI were identified in 153 patients from 3747 admissions. Mean age was 54 years; 102 were men. Foley catheterization was an associated factor in 129 patients (84%). UTI was caused by 40 different species of bacteria. In 28 infections (13%), the UTI was polymicrobial. Only nine patients had bacteremia. The bacteriology of the UTI depended on whether the patient had received systemic antibiotics previously during the hospitalization. Prior antibiotic administ
