Department Finance 101: Making the Money Work for Your Program.

Although hospital finance can often seem like the purview of the C-Suite, it is essential that hospitalist leaders and hospitalists understand the financial forces that impact hiring, expansion, and innovation. A strong understanding of division finances allows leaders to advocate for the patients for whom they care as well as their teams. Identifying budgetary sources (both actual and potential) will establish the key drivers of financial success. Leaders must then be able to decode and use budgetary language when reviewing key financial documents with their administrative team. Then they can

Impact of Inpatient Attending Specialty and Gastroenterology Consultation on Quality of Care of Patients Hospitalized with Decompensated Cirrhosis.

BACKGROUND: Data suggest hospitalists are less adherent to quality indicators for decompensated cirrhosis, and gastroenterology consultation may improve adherence. We sought to evaluate the impact of inpatient attending specialty and gastroenterology consultation on quality of care for decompensated cirrhosis. METHODS: This was a retrospective cohort study of patients with decompensated cirrhosis admitted to gastroenterology or hospitalist service at the University of Michigan between 2016-2020. The primary outcome was adherence to nationally recommended inpatient quality indicators for ascit

Things We Do for No Reason™: Withholding opioids among patients with acute pain and opioid use disorder.

Hospitalists care for patients with acute pain who have an opioid use disorder and require pain management. Hospitalists may withhold opioids for acute pain due to a variety of reasons. However, adequate pain management in patients with opioid use disorder, including with opioids, is the standard of care. This piece explains how to best manage acute pain in hospitalized patients with opioid use disorder with supportive evidence and discharge planning considerations.

Induction Time to Vaginal Delivery: A Comparison of Obstetric Coverage Models.

OBJECTIVE: Induction of labor (IOL) and hospitalist coverage are becoming more common. While hospitalist coverage has been associated with improved maternal outcomes and lower cesarean delivery rates, its impact on IOL remains unclear. The objective of this study was to compare the induction time to vaginal delivery across three obstetric coverage models: hospitalists, faculty generalists, and private practice generalists. STUDY DESIGN: This single-site retrospective cohort study analyzed singleton, term (≥39 weeks), vertex patients undergoing IOL at NYU Langone Hospital-Long Island from Janu

Pharmacologic Management of Malignant Bowel Obstruction: When Surgery Is Not an Option.

Malignant bowel obstruction (MBO) complicates 3%-15% of cancers and often necessitates inpatient admission. Hospitalists are increasingly involved in treating patients with MBO and coordinating their care across multiple subspecialties. Direct resolution of the obstruction via surgical or interventional means is always preferable. When such options are not possible, pharmacological treatments are the mainstay of therapy. Medications such as somatostatin analogs, steroids, H2-blockers, and other modalities can be effective in palliation and possible resolution of obstruction. Awareness of these

Clinical progress note: Secondary fracture prevention.

Osteoporosis is underdiagnosed and undertreated after fragility fractures. Hospitalists should be able to risk stratify osteoporosis severity to guide treatment decisions. Bisphosphonates remain the cornerstone of treatment. However, patients with the highest fracture risk may benefit from anabolic therapy before bisphosphonate use. Behavioral and environmental fall prevention measures also play a key role to prevent future fractures. Hospitalists are well-positioned to help address the gap in osteoporosis care-especially for patients who have already fractured and are at highest risk.

The cascade of benzodiazepine prescribing for hospitalized geriatric patients.

BACKGROUND: Hospitalists increasingly provide care for geriatric patients and little is known about the extent to which hospitalists adhere to evidence-based medication guidelines. This study aimed to characterize hospitalist adherence to BEERS guidelines for prescribing and monitoring benzodiazepines for older adults. METHODS: We conducted a retrospective chart review of admitted patients aged 70-85 years who had been prescribed benzodiazepine. Charts from 351 patients were analyzed for documentation and decision rationale, which included 638 separate notes. RESULTS: Benzodiazepines were pr

Optimizing screening for osteoporosis in patients with fragility hip fracture.

BACKGROUND: Osteoporosis, the underlying cause of most hip fractures, is underdiagnosed and undertreated. The 2008 Joint Commission report Improving and Measuring Osteoporosis Management showed only an average of 20% of patients with low-impact fracture are ever tested or treated for osteoporosis. We developed an integrated model utilizing hospitalists and orthopaedic surgeons to improve care of osteoporosis in patients with hip fracture. QUESTIONS/PURPOSES: Does our integrated model combining hospitalists and orthopaedic surgeons improve the frequency of evaluation for osteoporosis, screenin

Acute Liver Injury and Decompensated Cirrhosis.

Hospitalists often care for patients with liver disease, including those with acute liver injury and failure and patients with complications of decompensated cirrhosis. Acute liver failure is a true emergency, requiring intensive care and oftentimes transfer of the patient to a liver transplant center. Patients with decompensated cirrhosis have complications of portal hypertension, including variceal hemorrhage, ascites, spontaneous bacterial peritonitis, and hepatic encephalopathy. These complications increase the risk of mortality among patients with decompensated cirrhosis. Comanagement by

Inadequate practices for hepatic encephalopathy management in the inpatient setting.

Hepatic encephalopathy (HE) is an important complication of decompensated liver disease. Hospital admission for episodes of HE are very common, with these patients being managed by the hospitalists. These admissions are costly and burdensome to the health-care system. Diagnosis of HE at times is not straightforward, particularly in patients who are altered and unable to provide any history. Precipitants leading to episodes of HE, should be actively sought and effectively tackled along with the overall management. This mandates timely diagnostics, appropriate initiation of pharmacological treat
