[Cementless humeral head replacement for dislocation arthropathy of the shoulder joint].

BACKGROUND: Only a few reports in the literature describe that total shoulder replacement provides better functional results in dislocation arthropathy than hemiarthroplasty. Nevertheless, the risk of aseptic loosening in young and active patients is high after long-term follow-up. We evaluated the results of cementless humeral head resurfacing in this group. METHODS: We evaluated 29 patients with dislocation arthropathy who had been followed up for a mean of 39 months. The mean age at the time of operation was 43.7±14.7 years. Preoperatively, cuff deficiency was seen in 11 cases. RESULTS: T

Osteochondritis dissecans of the humeral head in two small-breed dogs.

Shoulder pathology consistent with osteochondritis dissecans of the humeral head was diagnosed in two small-breed dogs. In both cases, the diagnosis was made tentatively on the basis of radiography and confirmed by arthroscopy of the affected shoulder joint. Both dogs were successfully treated by surgical removal of a flap of cartilage from the affected area. Clinical and radiographic follow-up was available in both cases. The excised cartilage flaps were examined histopathologically and showed the same characteristics as cartilage flaps from dogs of large breeds prone to developing osteochond

Treatment of osteonecrosis of the humeral head with cementless surface replacement arthroplasty.

BACKGROUND: Osteonecrosis of the humeral head is a disabling condition that can lead to joint destruction with persistent pain and dysfunction. The aim of the present study was to examine cementless humeral surface replacement arthroplasty as a treatment option for posttraumatic and nontraumatic osteonecrosis. METHODS: Between 2002 and 2005, seventeen shoulders with posttraumatic (n = 8) or nontraumatic (n = 9) osteonecrosis of the humeral head were treated with cementless humeral replacement arthroplasty. The mean proportion of the humeral head that was occupied by necrotic bone was 18.6% (r

Hemiarthroplasty and total shoulder arthroplasty for avascular necrosis of the humeral head.

The purpose of this study was to review the outcome of patients with osteonecrosis of the humeral head, based on etiology and treatment with either hemiarthroplasty or total shoulder arthroplasty (TSA). Sixty-four shoulders, with an average age of 57 years, were evaluated at a mean of 4.8 years. Outcomes included L'Insalata and American Shoulder and Elbow Surgeons (ASES) scores, as well as range of motion (ROM). The overall ASES score was 67, average flexion was 127 degrees, and external rotation was 49 degrees. Outcomes did not differ based on etiology, but ROM was decreased with post-traumat

Recentering the humeral head for glenoid deficiency in total shoulder arthroplasty.

Substantial posterior glenoid wear causing static posterior subluxation of the humeral head in patients with primary osteoarthritis has been described. Persistent humeral head subluxation after total shoulder arthroplasty can result in early polyethylene wear and glenoid component loosening. In our prospective cohort study, we hypothesized that in patients with posterior glenoid wear from osteoarthritis, static posterior decentering of the humeral head could be recentered during total shoulder arthroplasty by surgical correction of glenoid alignment in the transverse plane with soft tissue bal

[Reconstruction of humeral head retroversion in the treatment of anterior shoulder instability].

Reduction of the physiologic retroversion of the humeral head can lead to shoulder luxation. After shoulder luxation it can also lead to shoulder instability. The aim of the paper was to check the level of humeral head retroversion in patients with anterior shoulder instability and to estimate the effectiveness of treatment of this instability with the derotational Weber osteotomy. The study included 38 patients, 34 males and 4 females, ranged 16-48 yr. (mean 31) treated in our Clinic. Every patient was done CT in order to estimate the level of humeral head retroversion. Patients with reduced

Osteochondritis dissecans of the humeral head. An unusual cause of shoulder pain.

Idiopathic osteochondritis dissecans of the humeral head is a rare cause of shoulder pain and immobility. If conservative management fails, surgical debridement and revascularization by drilling subchondral bone can lead to the restoration of a painless, functional joint.

Humeral Head Reconstruction of Reverse Hill-Sachs Lesions With Osteochondral Allograft.

BACKGROUND: True confirmed posterior shoulder dislocations are relatively uncommon injuries, with an estimated incidence of 1.1 per 100,000 individuals and are initially missed in up to 50% to 80% of cases. There are several treatment options for this injury presentation. In this case, we will focus on reconstruction with osteochondral allograft. INDICATIONS: If the cartilage cannot be fixed due to comminution or the cartilage is not viable due to chronicity or impact, osteochondral allograft might be a treatment option. This technique is typically considered for defects involving greater tha

Spontaneous Avascular Necrosis of the Humeral Head Following COVID-19 Vaccination.

We report a case of a 40-year-old man who presented with left shoulder pain and marked functional impairment two days after receiving the second dose of the Pfizer COVID-19 vaccine. Advanced imaging demonstrated focal avascular necrosis (AVN) of his humeral head. Initial management included simple analgesics, activity modification, and an ultrasound-guided intra-articular hydrodilatation injection. The patient subsequently had an improved range of motion but persistent severe shoulder pain. He has now been offered surgical intervention. Despite having risk factors for developing AVN previously

Inferior subluxation of humeral head after osteosynthesis for greater tuberosity fracture.

BACKGROUND: Inferior subluxation of the humeral head is frequently observed immediately after surgery for proximal humerus fractures; however, the incidence and risk factors of inferior subluxation after osteosynthesis for isolated greater tuberosity fractures remain unsolved. Additionally, the postoperative course of inferior subluxation has not been elucidated. The purpose of the present study is to identify the predictors for the occurrence of postoperative inferior subluxation by multivariate analysis and investigate the postoperative shift of inferior subluxation and its effect on surgica

Rapid collapse of the humeral head after arthroscopic rotator cuff repair.

UNLABELLED: This report presents a case of rapidly progressive collapse of the humeral head following arthroscopic rotator cuff repair. We performed humeral head replacement 8 months after the index surgery; histological examination of the collapsed area revealed necrosis, degeneration, fibrillation, and granulation in the cartilage and trabeculae. Osteonecrosis due to the use of metal anchors was considered the primary cause of the post-operative humeral head collapse. As the demand for arthroscopic cuff repair is expected to increase in the future, we recommend that surgeons become aware of

Bone grafting of humeral head cystic defects during rotator cuff repair.

Cystic bony defects of the humeral head greater tuberosity are often encountered during rotator cuff repair. These defects may be idiopathic, related to a patient's rotator cuff disease, or secondary to suture anchor placement from previous repairs. Some cysts are visible on preoperative magnetic resonance imaging, but most are discovered on footprint exploration or implant removal during revision surgery. These osseous defects reduce biological healing capacity and may decrease repair fixation strength. Bone grafting techniques are needed to address these defects. In this article, we present

Initiatory biomechanical study on humeral head migration after coracoacromial ligament cut.

OBJECT: To design and evaluate the strength of muscles in preventing humeral head migration after acromioplasty by biomechanical research on cadaver models. METHODS: Six fresh shoulder cadavers were studied. The coracoacromial ligament cut model was prepared in five shoulders for the study. Different forces were applied to the head of humerus with designed strengthened force on rotator cuff muscles. The results were calculated with SPSS software. RESULTS: The anterior and superior pulling force caused obvious migration of humeral head in coracoacromial ligament cut model; after strengthening

Shoulder arthroplasty in sickle cell patients with humeral head avascular necrosis.

The purpose of this study was to evaluate the results of shoulder arthroplasty for the treatment of avascular necrosis in patients with sickle cell disease. Medical records, radiographs, operative reports, and outcome scores of 8 adult patients with sickle cell disease were evaluated. The mean follow-up was 51 months (range, 2-10 years). Seven patients had a hemiarthroplasty, and one had a total shoulder arthroplasty. One patient had an intraoperative rotator cuff tear. Two had sickle cell crises in the immediate postoperative period. In one patient, stiffness developed that required arthrosco

Osteoarticular allograft transplantation for large humeral head defects in glenohumeral instability.

During traumatic anterior shoulder dislocation, significant derangement of normal anatomy occurs. Open and arthroscopic techniques alike have emphasized anatomic reconstruction of anterior capsulolabral structures while often neglecting associated osteoarticular pathology. Recent studies have shown that failure rates after Bankart repair are dramatically higher in the presence of large glenohumeral bone defects, yet little has been written about how to best address these lesions. Historically, these patients will undergo multiple procedures culminating with nonanatomic solutions to constrain t

[Treatment outcome after primary hemi-alloarthroplasty in dislocated humeral head fractures].

Humeral head fractures often lead to a high complication rate because of vulnerable vascularization of the humeral head, pretraumatical soft tissue lesions and osteoporotic bone. Compared with open reduction and internal fixation primary hemiarthroplasty seems to be an encouraging alternative treatment in case of dislocated and comminuted fractures. To evaluate our results following primary hemiarthroplasty we examined the outcome of 27 patients treated between November 1993 and May 1997 by primary hemiarthroplasty (15 Neer II, 12 Aequalis) in our institution within two weeks after trauma. The

[Sequestered bone infection after Kirschner wire osteosynthesis of humeral head fracture].

Percutaneous stabilization of fractures of the humeral head by K-wires is a proved therapy. This proceeding provides enough stability for early moving of the joint and has a low incidence for complications. In the case described here the special is the cause for local infection: local osteonecrosis induced by drilling-holes too close together.
