BACKGROUND: Differentiating ulcerative colitis-associated "backwash" ileitis (BWI) from Crohn's terminal ileitis (CTI) is a diagnostic challenge and highly affects patient's management. This study aimed to investigate magnetic resonance enterography (MRE) features including ileocecal valve patency index (ICPI) in patients with BWI and CTI and distinguish these entities based on MRE findings.
METHODS: After obtaining institutional review board approval, we reviewed 1654 MREs; 60 patients with pathologically proven BWI (n = 30) and CTI (n = 30) were enrolled. Two radiologists who were blinded t

The CT and radiologic findings in 11 patients (five with AIDS and six without AIDS) with ileocecal tuberculosis are described. On CT scans, five cases showed mild circumferential wall thickening of the terminal ileum and cecum, thickening of the ileocecal valve, and a few regional nodes. One case presented as nonspecific small-bowel obstruction. In five patients a more characteristic CT appearance was detected: preferential thickening of the ileocecal valve and medial wall of the cecum, exophytic extension engulfing the terminal ileum, and massive lymphadenopathy with low-density areas consist

Nerve-sparing radical cystectomy with ileocecal bladder substitute is highly recommended for male patients with schistosomal bladder carcinoma and for selected male patients with transitional cell carcinoma. Twenty-three patients underwent this procedure with preservation of the distal one third of prostatic capsule and inframontanal urethra, reinforcement of the ileocecal valve, and tightening of the two levator ani muscles over the cecourethral anastomosis. Urodynamic study revealed that the closed ileocecal segment is a highly compliant reservoir, and coupled with a reinforced ileocecal val

UNLABELLED: To evaluate the clinical features, surgical management, and prognosis of ileocecal duplication in children. A total of 115 patients diagnosed with ileocecal duplication at Beijing Children's Hospital between January 2010 and June 2021 were retrospectively reviewed. Ileocecal duplications were divided into ileal intraluminal (n = 41), ileal extraluminal (n = 24), ileocecal valve (n = 11), cecal intraluminal (n = 18), and cecal extraluminal (n = 3) types according to their locations. Median age at diagnosis was 9.5 (0.1-169.2) months. Intussusception was only observed preoperatively

Pediatric Crohn's disease localized exclusively to the ileum is a rare condition in our setting. Due to its overlapping clinical presentation, a high index of suspicion is required, and differential diagnoses must be considered with respect to infectious, infiltrative, and other less frequent conditions. We present the case of an 11-year-old patient with a history of anemia, pain in the right iliac fossa, weight loss, and fever. Imaging studies revealed thickening of the terminal ileum and intestinal stenosis near the ileocecal valve, with prestenotic dilation. Video colonoscopy showed that th

From 1976 to 1998 we have treated 17 neonates with short-bowel syndrome. Those 8 patients who had an intact ileocecal valve as well as the total colon preserved did significantly better than the 9 children without ileocecal valve and > 50% missing colon. In addition to the length of the intestinal remnants, motility had a major impact on the incidence of complications and final outcome. Four patients died (23.5%). All of them had an intestinal length of less than 30 cm, severe dysmotility, no ileocecal valve and an incomplete colon. The average duration of hospitalization of the children weane

Although gastroduodenal ulcers are common in solid organ transplant patients, there are few reports on multiple giant ulcers in the distal ileum and ileocecal valve caused by immunosuppressants Herein, we report on a liver transplant recipient and a renal transplant recipient with multiple large ulcers in the distal ileum and ileocecal valve who rapidly achieved ulcer healing upon withdrawal of sirolimus or tacrolimus and administration of thalidomide. In case 1, a 56-year-old man with primary hepatocellular carcinoma had received a liver transplantation. Tacrolimus combined with sirolimus and

A 63-year-old woman was admitted to the hospital with abdominal pain and nausea. Her abdomen was distended with obstructive bowel sounds on exam. There was diffuse abdominal tenderness but no palpable masses. Abdominal computed tomography (CT) scan revealed a large gallstone in the ileum. Surgical intervention was deferred given patient's known significant liver cirrhosis (Child-Pugh class B). Instead colonoscopy was performed and a large gallstone was found to be impacted at the ileocecal valve. The gallstone was fragmented using electrohydraulic lithotripsy (EHL) and then retrieved with snar

PURPOSE: A surgical technique is presented which creates an intestinal valve.
METHODS: In six dogs, the terminal ileum and the ascending colon were removed and the valve was made. The valve has been constructed in eight humans, three of them were cases of short bowel syndrome with resection of the ileocecal valve and five cases were right hemicolectomies attributable to a carcinoma of the cecum and ascending colon.
RESULTS: The dogs were observed for a period of four to six months, during which time the animals stayed healthy. In the cases of short bowel syndrome in humans, the valve reduced

A criterion for successful hydrostatic reduction of intussusception is reflux into the terminal ileum. In our practice, absence of reflux into the terminal ileum is not an indication for laparotomy if the radiographic appearances suggest edema of the ileocecal valve. The aim of this study was to validate our approach. We reviewed the case notes of patients with a diagnosis of intussusception (n = 107; age 11.74 +/- 1.48 months; mean +/- SEM) treated from 1987 to 1991. Eleven required primary laparotomy for peritonitis. Ninety-six patients who had a contrast enema were studied. Edema of the ile

We describe a patient who presented with mechanical obstruction of the small bowel secondary to impaction of the ileocecal valve from a gallstone. The stone impaction of the ileocecal valve was confirmed by colonoscopy. Although the stone was disimpacted, it could not be trapped and extracted during colonoscopy. Thereafter, the patient's small bowel obstruction was relieved with no further intervention. Even though surgery is the gold standard for the management of gallstone ileus. colonoscopy can be useful in management of patients with gallstone ileus in whom surgery is contraindicated. A re

A 58 year old women had massive hemorrhage from ulcers originating from lipomatosis of the ileocecal valve. The tumor and part of the intestine were removed by ample surgical resection and ileo-ascendostomy was performed. The patient relapsed 11 months later with hemorrhage and lipomatosis adjacent to the anastomosis established surgically 11 months before. This case report demonstrates that lipomatosis does not only occur at the ileocecal valve but as well at junctions established artificially between ileum and colon. The reasons for this latter event are unknown.

Ileocecal conduit urinary diversion was performed on 18 patients. Over-all surgical complications were few. Creation of a sutured ileal intussusception across the ileocecal valve prevented reflux in 9 of 12 patients (75 per cent) followed for 8 to 25 months. A stapling technique prevented reflux in all 5 patients followed for 8 to 12 months. These modifications may allow chronically reliable nonrefluxing urinary diversion along with the previously recognized advantages of the ileocecal segment.

The hydraulic ileal valve, which we developed in 1975, ensures continence by adapting to 5 different urinary reservoirs. An isolated ileal segment (14 cm) is folded inward on itself throughout its entire length. We performed 260 continent urostomies. An ileocecal reservoir was used in 179, detubularised ileocecal reservoir in 32, ileum 34, sigmoïd 5, rectum 1 and bladder (continent cystostomy) in 8. Of the patients 82% were continent immediately. Continence was obtained after repair of the valve in 12.5% therefore, 94.5% patients over all were continent.

Right hemicolectomy was done in a three and a half month old infant presenting irreductible acute intestinal intussusception. Analysis of the anatomical specimen allowed confirmation of the authors' histological theory on the genesis of infantile intussusception. Histological study demonstrated the key elements of this theory: thickness of the cecal musculosa clearly less than that of the right colon, thereby accounting for the lesser resistance of the former; contribution to the outer longitudinal muscle fibers of the ileum and colon to the constitution of the ileocecal valve commissures. The

In experiments on animals the function of cardia, pylorus and ileocecal valve was characterized electromyographically. Under non-pathologic conditions a reflux in the cardia and ileocecal region cannot be expected; the pylorus permits a physiological reflux between duodenum and stomach. A connection to other reflux-dependent pathological processes is discussed.

Inflammatory fibroid polyps (IFPs) are rarely found in the gastrointestinal tract. The majority of IFPs are sessile-pedunculated or pedunculated polypoid lesions, whereas a polyp presenting like a Borrmann type II lesion is extremely unusual. This report describes the case of a 74-year-old man with a history of intussusception, in whom a preoperative diagnosis of a cecal tumor of the ileocecal valve was made. A laparotomy subsequently revealed a lesion similar to a Borrmann type II tumor located 15 cm above the ileocecal valve, but not at the valve. The lesion was diagnosed as an IFP which had
