From modeling to morals: imagining the future of HIV PREP in Lesotho.

Amidst growing global endorsements of new biomedical HIV prevention strategies, ARV-based pre-exposure prophylaxis (ARV PrEP) has garnered considerable attention as a potentially promising prevention strategy. Though it may offer more effective protection for certain at-risk groups than conventional prevention strategies (such as sexual partner reduction, condom use, and prevention of mother-to-child transmission), PrEP is more costly. PrEP requires more ongoing contact between individuals and providers, and a level of surveillance from the health system that is not necessary with other preven

Treatment of PTSD: a comparison of imaginal exposure with and without imagery rescripting.

We tested whether the effectiveness of imaginal exposure (IE) treatment for posttraumatic stress disorder (PTSD) was enhanced by combining IE with imagery rescripting (IE+IR). It was hypothesized that IE+IR would be more effective than IE by (1) providing more corrective information so that more trauma-related problems can be addressed, and (2) allowing patients to express emotions that they had been inhibiting, such as anger. In a controlled study 71 chronic PTSD patients were randomly assigned to IE or IE+IR. Data of 67 patients were available. Treatment consisted of 10 weekly individual the

Differences between actual and imagined usage of chopsticks: an fMRI study.

We examined neural basis underlying tool-use behavior to discuss whether or not the usage of a well-learned tool has a specific route. Regional cerebral blood flow was measured in healthy Japanese subjects using functional magnetic resonance imaging (fMRI) during object pick-up using chopsticks, object pick-up using the hand, pantomiming the use of chopsticks, imagining the use of chopsticks, and imagining the use of the hand. First, the left inferior parietal lobule (IPL) was found to selectively contribute to tasks requiring explicit retrieval of tool-related hand movements that were pantomi

Imaginal, sensory, and cognitive experience in spontaneous recovery from alcoholism.

Although alcoholism is often regarded as an intractable disorder that requires intensive treatment, studies of the natural history of alcoholism indicate that unaided, spontaneous recovery may be the most common pathway to remission from alcoholism. Negative environmental consequences of alcoholic drinking have been invoked to explain spontaneous recovery, but a more compelling reason for sudden changes in drinking behavior concerns shifts in the personal meanings surrounding alcohol use. Extensive interviews in a multimodal format were conducted with two groups of alcoholics: one group compri

Poetry as Praxis + "Illumination": Toward an Epistemically Just Health Promotion for Resistance, Healing, and (Re)Imagination.

Health promotion is facing a most challenging future in the intersections of structural racism, COVID-19 (coronavirus disease 2019), racialized police violence, and climate change. Now is a critical moment to ask how health promotion might become more responsive to and representative of people's daily realities. Also how it can become a more inclusive partner in, and collaborative conduit of, knowledge-one capable of both informing intellects and transforming hearts. It needs to feel the pulse of the "fierce urgency of now," and perhaps nothing can reveal this pulse more than the creative powe

What do recently housed young people imagine for the future of third places? Using game-based inquiry to (re)imagine affirming, youth-centered third places.

Third places-public and community settings like parks and libraries-are theorized to serve as sources of social connection, community, and play. Yet, young people who hold marginalized identities, such as those experiencing homelessness and housing instability, often endure discrimination in third place settings. This study used game-based inquiry to partner with recently housed young people who have experienced chronic housing instability (N = 21) to understand how they would (re)imagine future third places. We then analyzed transcripts from these game sessions through qualitative thematic an

Countering failures of imagination: governance, human development and public health in low- and middle-income countries.

Dramatic cuts in global aid have irrevocably changed the health funding landscape in Low- and Middle- Income Countries (LMICs), creating significant challenges for national governments. But there is a silver lining: these changes give national governments greater agency to set development goals. This moment of disruption demands bold thinking; it is an opportunity to place human development above sector-specific outcomes at the heart of national priority setting and reframe investments in public health accordingly. The social conditions that shape a person's health are the same conditions that

Enhanced activation of motor execution networks using action observation combined with imagination of lower limb movements.

The combination of first-person observation and motor imagery, i.e. first-person observation of limbs with online motor imagination, is commonly used in interactive 3D computer gaming and in some movie scenes. These scenarios are designed to induce a cognitive process in which a subject imagines himself/herself acting as the agent in the displayed movement situation. Despite the ubiquity of this type of interaction and its therapeutic potential, its relationship to passive observation and imitation during observation has not been directly studied using an interactive paradigm. In the present s

Generativity and imagination in autism spectrum disorder: evidence from individual differences in children's impossible entity drawings.

This study examined the cognitive underpinnings of spontaneous imagination in autism spectrum disorder (ASD) by way of individual differences. Children with ASD (N = 27) and matched typically developing (TD) children were administered Karmiloff-Smith's (1990) imaginative drawing task, along with measures that tapped specific executive functions (generativity, visuospatial planning, and central coherence processing style) and false belief theory of mind (ToM) understanding. The ASD group drawings displayed deficits in imaginative content and a piecemeal pictorial style. ASD participants also sh

Intersections of personal vs. collective trauma during the COVID-19 pandemic: the hijacking of the human imagination.

This paper explores how the deadly shadow of COVID-19 passing over the Earth constitutes a collective trauma that frequently opens up or 'triggers' un-remembered personal trauma, and it provides clinical examples of these intersections. The paper further explores how the human imagination, which we normally utilize to make meaning out of traumatic experience, can be hijacked by fear - leading to avoidance of suffering and to illusory formulations and alternative realities such as conspiracy theories. Alternatively, the imagination can be employed in more realistic and creative ways - leading t

Traumatic pasts and the historical imagination: Symptoms of loss, postcolonial suffering, and counter-memories among African migrants.

This work aims to rethink the relationship between anthropology and cultural psychiatry from a historical perspective, through reflections on the dynamics of forgetting and remembering in the context of migration. While migrants' symptoms often bear cultural hallmarks of suffering, they also reveal images of a traumatic history, which resurface in moments of danger, uncertainty, and crisis. I claim these symptoms are allegories of a dispossessed past, and can be interpreted as counter-memories, as "palimpsests" of an eclipsed script. Trauma symptoms keep returning to a collective past, and thu

Can imagery rescripting and imaginal extinction attenuate the return of conditioned fear?

Standard extinction (SE) is the most widely implemented intervention for reducing conditioned and generalized fear, indexed by psychophysiological skin conductance responses, and subjective self-report responses such as expectancy and valence ratings; however, the possibility of fear return still remains. Hence, novel interventions may be used to augment the efficiency of SE. Imagery rescripting (ImRS), based on the premise of unconditioned stimulus (UCS) devaluation, encompasses the devaluation of the cognitive representation of the UCS in a positive direction. Imaginal extinction (IE), based

A shared code for perceiving and imagining objects in human ventral temporal cortex.

Mental imagery allows us to remember previous experiences and imagine new ones. Animal studies have yielded rich insight into mechanisms for visual perception, but the neural mechanisms for visual imagery remain poorly understood. We determined that approximately 80% of visually responsive single neurons in the human ventral temporal cortex (VTC) use a distributed axis code to represent objects. We used that code to reconstruct objects and generate maximally effective synthetic stimuli. We then recorded responses from the same neural population while subjects imagined specific objects; about 4

Looping patchworks and imaginations in a concept: Building the patchwork of "organoid" in contemporary biotechnology.

Organoids have become a central but contested notion across developmental biology, stem cell research, tissue engineering and regenerative medicine. I argue that the term organoid functions not as a single, well-defined scientific kind but as a special kind of patchwork concept that I call looping patchwork concept: a coordination device whose stability depends on overlapping inferential patches distributed across material practices, ethical deliberations, and promissory imaginaries. Drawing on Hacking's looping kinds, Jasanoff's co-production framework, and recent philosophical work on patchw

A comparison of virtual reality and verbal imaginal exposure for childhood anxiety disorders.

Virtual reality is a promising tool for improving efficacy of exposure therapy for childhood anxiety disorders (CADs), particularly for exposures that are challenging to orchestrate in vivo. The present study compared virtual reality exposure (VRE) to verbal imaginal exposure (IE) on anxiety elicited, homework completion, and preparation for subsequent exposures. Forty-five youth with CADs completed both types of exposure in session (order randomly assigned), practiced one form of exposure as homework (randomly assigned), and returned a week later to repeat both exposures (in randomized order)

Examining Community Clinicians Use of Imaginal Exposure with Childhood Anxiety Disorders.

Although community therapists rarely use exposure when treating youth with anxiety disorders, they are more likely to endorse the use of imaginal exposure, relative to in vivo or interoceptive exposure. Such use of imaginal exposure could reflect a sub-optimal replacement for in vivo exposure or a targeted intervention to address anxiety-provoking thoughts, such as in generalized anxiety disorder. The current study used the survey responses of 178 community-based clinicians to examine these competing hypotheses. The results indicated that therapist endorsement of more clearly defined imaginal

Imagining a fentanyl future: Some consequences of synthetic opioids replacing heroin.

For nearly a century heroin has dominated the illegal opioid trade. The global supply of heroin is estimated to generate tens of billions of dollars in revenues a year and its illegal use has long been the source of many societal harms. The arrival of inexpensive and mass-produced synthetic opioids, such as fentanyl, to parts of North America and Europe may signal the beginning of the end of heroin's dominance. Data from several places suggest that fentanyl and other synthetic opioids can quickly supplant heroin. Taking the extreme case, we calculate the estimated wholesale revenues of heroin

Imagine that: cholinesterase inhibitor treatment of complex visual hallucinations of unknown aetiology.

Introduction: Our objective is to highlight the value of the neurophenomenological classification of complex visual hallucinations (VHs). This approach enabled the authors to successfully treat VHs of uncertain aetiology with cholinesterase inhibitors because the content of the hallucinations suggested dysfunction in cholinergic modulated networks. Methods: We utilise the single case report to describe the nature and content of chronic VHs experienced by a 49-year-old woman following a prolonged admission to ITU. Despite extensive investigation, no clear cause was identified for these hallucina
