CD4(+) T-cell immunodeficiency is more dependent on immune activation than viral load in HIV-infected children on highly active antiretroviral therapy.

OBJECTIVE: The aim of this study was to analyze the association between CD4(+) depletion and immune activation in HIV-1-infected children on highly active antiretroviral therapy (HAART). DESIGN AND SETTING: We carried out a cross-sectional study to determine the profile of several immunologic parameters in 143 children on HAART for more than 24 weeks. Children were stratified according to current immunologic status (CD4 < or =15%, 15%-25%, and > or =25%) and viral load (VL) levels (<400 copies/mL; 400-10,000 copies/mL; and >10,000 copies/mL). We also studied 23 uninfected children as healthy

Memory B cell resurgence requires repeated rituximab in myasthenia gravis.

The immunologic effects of rituximab (RTX) in myasthenia gravis (MG) remain to be explored. We aimed to clarify immunologic reactions and their association with response to RTX in MG. Regulatory T cell and B cell profiles of MG patients were monitored. Two patients presenting with generalized MG with anti-acetylcholine receptor antibodies were treated with RTX. The treatment led to sustained clinical improvement, discontinuation of intravenous immunoglobulin or plasma exchange, and reduction of prednisolone and other drugs. One patient was in remission for more than one year, whereas the other

Abnormalities of measles antibody response in human immunodeficiency virus type 1 (HIV-1) infection.

The finding that severe measles occurs in immunized as well as nonimmunized human immunodeficiency virus (HIV)-infected individuals suggests that both immunologic memory and the initial response to measles may be impaired by HIV infection. That the initial response is affected was supported by the finding that post-measles immunization titers of HIV-infected babies were significantly lower (p = 0.01) than those of normal babies. Poor immunologic memory was evidenced in HIV-infected children by lower titers than in normal children (p < 0.001) and by a continuing decline in measles antibody that

Comprehensive single-cell profiling of T and B cell subsets in mice reveals impacts on memory immune responses in FMDV infection.

The impact of Foot-and-Mouth Disease Virus (FMDV) on memory immune responses has not been thoroughly investigated due to limited availability of immunological research tools. Using single-cell RNA sequencing, we identified specific gene markers for the majority of T and B cell subsets in the spleens of mice. Our findings indicate that FMDV infection significantly reduces the proportions of memory cell populations (e.g., memory B cells, memory CD4+ T cells, and memory CD8+ T cells) relative to their respective lymphocyte subsets (total B cells, CD4+ T cells, and CD8+ T cells) in the short term,

The presence of anti-nuclear antibodies alone is associated with changes in B cell activation and T follicular helper cells similar to those in systemic autoimmune rheumatic disease.

BACKGROUND: Diagnosis of systemic autoimmune rheumatic diseases (SARD) relies on the presence of hallmark anti-nuclear antibodies (ANA), many of which can be detected years before clinical manifestations. However, ANAs are also seen in healthy individuals, most of whom will not develop SARD. Here, we examined a unique cohort of asymptomatic ANA+ individuals to determine whether they share any of the cellular immunologic features seen in SARD. METHODS: Healthy ANA- controls and ANA+ (ANA ≥1:160 by immunofluorescence) participants with no SARD criteria, with at least one criterion (undifferenti

Spatial organization of tissue-resident memory T cells.

Tissue-resident memory T (TRM) cells durably reside in peripheral tissues, poised to deliver rapid local immune protection. TRM cells are found in most organs, and they are a core component of immunological memory, complementing circulating memory T cell populations. By definition, TRM cells can persist in tissues for months to years without antigen stimulation. However, demonstrating both residency and antigen absence in reality is challenging. Here, we discuss the strengths and challenges to our canonical definitions of TRM cells. We examine how early priming and subsequent tissue imprinting

Development, acceptance, and use of immunologic correlates of protection in monitoring the effectiveness of combination vaccines.

The establishment of immunologic correlates of protection for all vaccine antigens is a worthwhile goal. It allows new vaccines to be licensed on the basis of attainment of defined immunologic benchmarks, without the need for large-scale efficacy trials for each new product. This is particularly important for the evaluation of new combination products. Efficacy trials of each new mixture would be unethical because routinely recommended vaccines would be denied children in the control group. The establishment of immunologic correlates of protection should be a defined goal of every efficacy tri

Antigen-specific B memory cell responses to Plasmodium falciparum malaria antigens and Schistosoma haematobium antigens in co-infected Malian children.

Polyparasitism is common in the developing world. We have previously demonstrated that schistosomiasis-positive (SP) Malian children have age-dependent protection from malaria compared to matched schistosomiasis-negative (SN) children. Evidence of durable immunologic memory to malaria antigens is conflicting, particularly in young children and the effect of concomitant schistomiasis upon acquisition of memory is unknown. We examined antigen-specific B memory cell (MBC) frequencies (expressed as percentage of total number of IgG-secreting cells) in 84 Malian children aged 4-14 to malaria blood-

[Immunizing and immunomodulating activity of a wild epidemic parotitis virus at a parotitis infection focus].

The immune status of 18 children previously vaccinated or not with live parotitis vaccine and living in a focus of parotitis was tested over time. Specific antibodies were found in the blood sera or nasal secretion of 88.9% children by follow-up days 10-20, and memory antigen-reactive cells were forming in 55.6%. Functional temporary T-immunodeficiency was observed at the same period in 66.7% children. Although there were no cases of clinically apparent epidemic parotitis, immunologic studies confirmed the circulation of wild type virus. Immunologic studies detected 2 cases of inapparent cours

Immunogenicity of a Booster Dose of Quadrivalent Meningococcal Conjugate Vaccine in Previously Immunized HIV-Infected Children and Youth.

BACKGROUND: The US Advisory Committee on Immunization Practices recommends a booster dose of quadrivalent meningococcal conjugate vaccine (MCV4) after initial immunization for patients at high risk for meningococcal infection. METHODS: The International Maternal Pediatric Adolescents AIDS Clinical Trials (IMPAACT) P1065 trial evaluated the use of MCV4 in human immunodeficiency virus (HIV)-infected children and youth. The final step of this trial was an open-label study of an MCV4 booster dose 3.5 years after primary MCV4 immunization. Antibody titers were evaluated at the time of the booster

Delayed memory B cell recovery in peripheral blood and lymphoid tissue in systemic lupus erythematosus after B cell depletion therapy.

OBJECTIVE: Recent data suggest that the reconstituting peripheral B cell compartment after B cell depletion therapy may be functionally immature, with a preponderance of transitional B cells and a paucity of memory B cells. This study was undertaken to determine the magnitude, duration, and cause of these defects in rituximab-treated systemic lupus erythematosus (SLE) patients. METHODS: Fifteen patients with SLE previously treated with rituximab as part of a phase I/II dose-escalation study were evaluated during a long-term followup (mean followup period 41 months). Rapid allergen-induced interleukin-17 and interferon-γ secretion by skin-resident memory CD8+ T cells.

BACKGROUND: Skin-resident memory T (TRM ) cells are associated with immunological memory in the skin. Whether immunological memory responses to allergens in the skin are solely localized to previously allergen-exposed sites or are present globally in the skin is not clear. Furthermore, the mechanisms whereby TRM cells induce rapid recall responses need further investigation. OBJECTIVES: To study whether contact allergens induce local and/or global memory, and to determine the mechanisms involved in memory responses in the skin. METHODS: To address these questions, we analysed responses to co

Resident and circulating memory T cells persist for years in melanoma patients with durable responses to immunotherapy.

While T-cell responses to cancer immunotherapy have been avidly studied, long-lived memory has been poorly characterized. In a cohort of metastatic melanoma survivors with exceptional responses to immunotherapy, we probed memory CD8+ T-cell responses across tissues, and across several years. Single-cell RNA sequencing revealed three subsets of resident memory T (TRM) cells shared between tumors and distant vitiligo-affected skin. Paired T-cell receptor sequencing further identified clonotypes in tumors that co-existed as TRM in skin and as effector memory T (TEM) cells in blood. Clonotypes tha

Novel immunotherapy response predictor based on blood Temra cell signature.

Dong et al. revealed that circulating CX3CR1+ terminally differentiated effector memory (Temra) cells are key predictors of immunotherapy response. A 20-gene Temra transcriptional signature that stratifies responders from non-responders was developed, achieving an area under the curve (AUC) of 0.834 in a prospective multi-center cohort. The study establishes a blood-based biomarker that captures the pre-existing functional state of systemic immunity, offering a translational tool to guide immunotherapy.

Exercise and tissue-resident memory T cells: from circulating numbers to spatial immune remodeling.

Tissue-resident memory T (TRM) cells have become a paradigm shift in the field of immunology and have changed our view of local immune surveillance at barrier surfaces. In contrast to circulating memory T cells, TRM cells are fixed in non-lymphoid organs like lungs, intestine and skin to act as the first line of defense against reinfection and malignant conversion. The finding contradicts the conventional emphasis on exercise immunology on the number of circulating lymphocytes and requires a new conceptual framework of the so-called quantitative to spatial immune remodeling. This review summar

Isolation and Characterization of Human Tissue Resident Memory T cells.

The majority of immune cells in the human body exist within the tissues rather than in the circulation. Nevertheless, most of our knowledge of the human immune system is biased towards the characterization and understanding of circulating immune cell populations because the latter are readily sampled, whereas cells in tissues are difficult to obtain and/or are limited to single sites of disease. Tissue-resident cells differ from circulating cells due to tissue-specific niche adaptations that influence their phenotype and function. For instance, T cells in tissues, resident memory (TRM cells),

Defective B-cell memory in patients with Down syndrome.

BACKGROUND: Patients with Down syndrome carry immunologic defects, as evidenced by the increased risks for autoimmune diseases, hematologic malignancies, and respiratory tract infections. Moreover, the low numbers of circulating B cells suggest impaired humoral immunity. OBJECTIVE: We sought to study how immunodeficiency in patients with Down syndrome results from immunologic defects in the B-cell compartment. METHODS: We studied blood B-cell subset composition, replication history, somatic hypermutation status, and class-switch recombination in 17 children with Down syndrome. Germinal cente

Immunologic dysfunction contributes to the otitis prone condition.

Acute Otitis Media (AOM) is a multifactorial disease occurring mostly in young children who are immunologically naïve to AOM pathogens. This review focuses on work from Rochester NY, USA over the past 12 years among young children who had AOM infections microbiologically-confirmed by tympanocentesis, so called "stringently-defined". Among stringently-defined otitis prone children deficiencies in fundamental immune defense mechanisms have been identified that contribute to the propensity of young children to experience recurrent AOM. Dysfunction in innate immune responses that cause an immunopa

IgG4-related disease and B-cell malignancy due to an IKZF1 gain-of-function variant.

BACKGROUND: Monoallelic loss-of-function IKZF1 (IKAROS) variants cause B-cell deficiency or combined immunodeficiency, whereas monoallelic gain-of-function (GOF) IKZF1 variants have recently been reported to cause hypergammaglobulinemia, abnormal plasma cell differentiation, autoimmune and allergic manifestations, and infections. OBJECTIVE: We studied 7 relatives with autoimmune/inflammatory and lymphoproliferative manifestations to identify the immunologic disturbances and the genetic cause of their disease.
