Treating periodontal disease: from antimicrobials to immunomodulation.

Dysregulated host immune responses are characteristic of a variety of inflammatory-mediated diseases. As such, novel immune-modulating therapies have been developed for the treatment of these inflammatory conditions. One condition that has seen a paradigm shift in therapeutic strategies is periodontal disease (PD), a common oral inflammatory condition. While initiated by bacteria, a dysregulated host immune response drives the disease. In this review, we discuss key innate and adaptive immune cells that have a role in PD progression, as well as different ways to target them. We highlight thera

Current Treatment and Immunomodulation Strategies in Acute Myocarditis.

Myocarditis is an inflammatory disease of the myocardium characterized by a great heterogeneity of presentation and evolution. Treatment of myocarditis is often supportive, and the evidence for immunosuppression is scarce and debated. Conventional treatment is based on clinical presentation, ranging from conservative to advanced mechanical assist devices. In this setting, immunosuppression and immunomodulation therapies are mostly reserved for patients presenting with major clinical syndromes. In this review, we will summarize the current evidence and strategies for conventional and immunosupp

When to initiate immunomodulatory therapy in sepsis.

Immunomodulatory therapy in sepsis has long failed to deliver consistent clinical benefit, in part because the heterogeneity of host responses and the dynamic nature of disease trajectories have been underestimated. Rather than equating the timing of initiation with a fixed interval after symptom onset, a more clinically meaningful definition is a chronology-informed clinical decision point at which standard sepsis care has been implemented, serial monitoring supports a dominant or clinically important discordant immunophenotype, and the organ dysfunction trajectory suggests that continued obs

Immunomodulation as a novel treatment strategy for periodontal disease: a systematic review.

BACKGROUND: Periodontal diseases (PDs) are globally prevalent and are associated with reduced quality of life and systemic health complications. Conventional therapy, involving professional mechanical plaque removal (PMPR) and oral hygiene instruction, faces limitations involving disease recurrence and patient factors. This systematic review explores immunomodulatory therapies as alternative or adjunctive treatments. METHOD: MEDLINE (Ovid), EMBASE (Ovid), Cochrane Database of Systematic Reviews and MEDLINE (Web of Science) databases were searched on 9 June 2024. Two authors independently scre

Immunomodulatory and immune-toxicological role of nanoparticles: Potential therapeutic applications.

Nowadays, Nanoparticle-based immunotherapeutic research has invoked global interest due to their unique properties. The immune system is a shielding structure that defends living things from external threats. Before the use of any materials in drug design, it is essential to study the immunological response to avoid triggering undesirable immune responses in the body. This review tries to summarize the properties, various applications, and immunotherapeutic aspects of NP-induced immunomodulation relating to therapeutic development and toxicity in human health. The role of NPs in the immune sys

Topical immunomodulators in dermatology.

Topical immunomodulators are agents that regulate the local immune response of the skin. They are now emerging as the therapy of choice for several immune-mediated dermatoses such as atopic dermatitis, contact allergic dermatitis, alopecia areata, psoriasis, vitiligo, connective tissue disorders such as morphea and lupus erythematosus, disorders of keratinization and several benign and malignant skin tumours, because of their comparable efficacy, ease of application and greater safety than their systemic counterparts. They can be used on a domiciliary basis for longer periods without aggressiv

The role of immunomodulatory nano systems in the treatment of sepsis: past, present, and future.

Sepsis is a life-threatening condition caused by a dysregulated host response to infection and remains a leading cause of death in intensive care units. Although antimicrobials and supportive care are vital, patient outcomes are hindered by two conflicting immune states: excessive inflammation and immune paralysis, both contributing to organ failure. Immunomodulatory nanotechnology provides a means to target both aspects of this immune response. Early nanocarriers improved the pharmacokinetics of antibiotics and anti-inflammatory drugs, while modern nanoplatforms enhance this approach with bio

Biologic immunomodulators and spinal fusion: evidence, mechanisms, and perioperative considerations.

Purpose Biologic immunomodulators are increasingly used across autoimmune, inflammatory, oncologic, and metabolic diseases, resulting in a growing population of patients undergoing spinal fusion while receiving these agents. Because immune pathways targeted by biologic therapies play essential, time-dependent roles in bone graft incorporation, this review examines how pharmacologic modulation of these pathways may affect fusion outcomes and perioperative risk. Methods We synthesized mechanistic, translational, and clinical evidence examining the impact of major classes of biologic immunomodulat

Predictable biomarkers of developing lymphoma in patients with Sjögren syndrome: a nationwide population-based cohort study.

Sjögren syndrome (SS) is commonly known to be correlated with lymphoma. This study included 16,396 individuals in the SS cohort and 65,584 individuals in the non-SS cohort, all of whom were enrolled in the Taiwan National Health Insurance database between 2000 and 2010. We evaluated the risk factors of non-Hodgkin's lymphoma (NHL) in the primary SS cohort by applying a Cox multivariable proportional-hazards model. We increased the correlation of patients with SS and NHL, with an adjusted HR of 4.314 (95% CI 2.784 - 6.685). Of the 16,396 SS patients, 66 individuals had salivary gland slices wit

Immunomodulation and immunodeficiency.

This article briefly reviews the concepts of immunodeficiency and immunomodulation as they relate to selected skin diseases in the dog and cat. Immunodeficiency states are uncommon and may be associated with a subnormal or down-regulated immune system, including humoral deficiencies, such as IgA, and abnormal lymphocyte or neutrophil function. Establishing a causal relationship between a skin disease and presumed immunodeficient state has been difficult due to the rarity of such conditions, and the limited nature of the techniques used to characterise the immune system response. Severe combine

Dry eye diagnosis and management in 2004.

PURPOSE OF REVIEW: To document the changes in treatment paradigm for the management of dry eye. Restasis is 0.05% cyclosporine A and is the first immunomodulatory agent approved for treatment of dry eye. RECENT FINDINGS: Mounting evidence supports the inflammatory mechanism as explanation for the occurrence of dry eye disease. Immunomodulation has been shown to be effective in treating patients with moderate to severe dry eye disease caused by keratoconjunctivitis sicca. This addresses one of the two main forms of dry eye. There is a new report that immunomodulation using topical 0.05% cyclos

Disseminated herpes zoster mimicking rheumatoid vasculitis in a rheumatoid arthritis patient on etanercept.

Tumor necrosis factor-alpha (TNFalpha)-blocking agents are immunomodulating agents introduced for treatment of a variety of chronic inflammatory disease conditions. Adverse effects include an increased incidence of infections. Clinically, these infections often have atypical presentations that may hamper prompt diagnosis. In our report of a patient on etanercept therapy for rheumatoid arthritis, the correct diagnosis was delayed because disseminated herpes zoster was clinically mimicking vasculitis. Initially assuming rheumatoid vasculitis, immunosuppression was increased, resulting in worseni

AI-Driven Secondary Immunomodulatory Effects of Conventional Drugs on Patient-Derived Macrophages.

The secondary immunomodulatory effects of conventional therapeutics, such as antibiotics and cytostatics, are frequently overlooked despite their significant clinical implications. Building on our previous findings that drugs like paclitaxel and doxorubicin heavily influence macrophage polarization-potentially driving metastasis or inflammation-this study systematically evaluates the secondary immune-modulating actions of standard drugs and natural adjuvants. Using patient-derived bronchoalveolar lavage (BAL) fluid (ex vivo alveolar macrophages), we developed an analytical platform using synth

Impaired macrophage functions as a possible basis of immunomodification by microbial agents, tilorone and dimethyldioctadecylammonium bromide.

Four microbial and two chemically defined immunomodulating agents namely viable BCG, killed Mycobacterium butyricum, killed Lactobacillus plantarum, zymosan, tilorone, and dimethyldioctadecylammonium bromide (DDA) were studied for their effects on macrophage functions in vitro and in vivo. All agents induced a dose-dependent mortality of macrophages as determined by trypan blue exclusion. DDA and especially tilorone were rather toxic for macrophages in vitro. All agents except tilorone and DDA inhibited phagocytosis of yeast cells and uptake of acridine orange in vitro at doses which killed up

Advances in antiviral therapy.

Infections with five of the herpesviruses (herpes simplex virus 1 [HSV-1], HSV-2, varicella zoster virus, Epstein-Barr virus, and cytomegalovirus) are treated with topical or systemic antiviral therapies. There are more than 100 genotypes of human papillomaviruses (HPVs), which may manifest as warts, skin cancers, cervical cancer, anogenital cancers, and upper digestive tract cancers. Molluscum contagiosum (MC) is a common, benign viral infection of the skin. Immunomodulating agents, such as imiquimod, act on HPV and MC indirectly by inducing host immune responses, such as cytokines and cell-m

Potential treatments and treatment strategies in Creutzfeldt-Jakob disease.

Creutzfeldt-Jakob disease is a rare neurodegenerative disease that follows a rapidly progressive course, leading to death, usually only a few months from onset. Over the last 30 years, many treatments have been tried for this condition, including a wide variety of antiviral agents and immunomodulating drugs. More recently, potential treatments have been devised that interfere with the pathological processes involved in the formation of the prion protein. Most of these experiments have been performed in in vitro and tissue culture models and have yet to be tried in human prion disease. As yet,

Effects of human calcitonin on pain in the treatment of Tietze's syndrome.

Tietze's syndrome consists of pain, tenderness, and swelling of the costosternal or sternoclavicular joints. The etiology is unknown, although microtrauma or rare infections have been though to be responsible for the lesion. Conventional therapy consists of anti-inflammatory drugs, sometimes combined with chemotherapeutic agents and antibiotics. Surgery is usually deemed necessary for cases that do not respond to conventional therapy. Experience with the use of human calcitonin in five patients suffering from severe Tietze's syndrome who did not respond to conventional therapy is reported. Sym

Diagnosing and Managing Stevens-Johnson Syndrome and Toxic Epidermal Necrolysis in Adults: Review of Evidence 2017-2023.

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are life-threatening reactions associated with long-term disabling sequelae. In the acute phase, the best supportive care in expert centers is the cornerstone of treatment. The follow-up is prolonged and multidisciplinary, according to the patient's needs. In this paper, we review the evidence from 2017 to 2023 for the diagnosis and management of SJS/TEN in adults. On the basis of this review and our own experience, we present recommendations for the diagnosis of SJS/TEN in adults, management in the acute phase (best supportiv

Osteonecrosis of the jaw in a metastatic lung cancer patient with bone metastases undergoing pembrolizumab + denosumab combination therapy: Case report and literature review.

The rare adverse drug reaction MRONJ (medication-related osteonecrosis of the jaw) can be induced by treatment with antiresorptive, antiangiogenic, or immunomodulating agents. Immune checkpoint inhibitors (ICIs; e.g., pembrolizumab) are the standard care for advanced or metastatic cancer patients. Denosumab directed against receptor activator of NF-kB ligand (RANKL) is approved for preventing skeletal-related events (SREs) in cancer patients with bone metastases. The combination therapy of ICIs + denosumab has shown promising efficacy and no unexpected safety issues in metastatic melanoma and
