Clinical neuroprosthetics: Today and tomorrow.

Implantable neurostimulation devices provide a direct therapeutic link to the nervous system and can be considered brain-computer interfaces (BCI). Under this definition, BCI are not simply science fiction, they are part of existing neurosurgical practice. Clinical BCI are standard of care for historically difficult to treat neurological disorders. These systems target the central and peripheral nervous system and include Vagus Nerve Stimulation, Responsive Neurostimulation, and Deep Brain Stimulation. Recent advances in clinical BCI have focused on creating "closed-loop" systems. These system

Chronic adaptive deep brain stimulation versus conventional stimulation in Parkinson's disease: a blinded randomized feasibility trial.

Deep brain stimulation (DBS) is a widely used therapy for Parkinson's disease (PD) but lacks dynamic responsiveness to changing clinical and neural states. Feedback control might improve therapeutic effectiveness, but the optimal control strategy and additional benefits of 'adaptive' neurostimulation are unclear. Here we present the results of a blinded randomized cross-over pilot trial aimed at determining the neural correlates of specific motor signs in individuals with PD and the feasibility of using these signals to drive adaptive DBS. Four male patients with PD were recruited from a popul

[Chronic sacral nerve electrostimulation in treatment of neurogenic pelvic organ dysfunction in children].

UNLABELLED: Myelodysplasia is the most common cause of congenital pelvic abnormalities in children. The causes of acquired neurogenic pelvic dysfunctions in children include spinal cord injury, myelitis, and neurodegenerative diseases. Urination impairments in children with neurological disorders are a serious clinical problem. In most cases, the capabilities of conservative treatment of pelvic organ dysfunctions are limited. One of the most promising directions in treatment of neurogenic urination disorders is modeling of a lost urination mechanism using direct or mediated electrostimulation

Neurostimulation of the cholinergic anti-inflammatory pathway ameliorates disease in rat collagen-induced arthritis.

INTRODUCTION: The inflammatory reflex is a physiological mechanism through which the nervous system maintains immunologic homeostasis by modulating innate and adaptive immunity. We postulated that the reflex might be harnessed therapeutically to reduce pathological levels of inflammation in rheumatoid arthritis by activating its prototypical efferent arm, termed the cholinergic anti-inflammatory pathway. To explore this, we determined whether electrical neurostimulation of the cholinergic anti-inflammatory pathway reduced disease severity in the collagen-induced arthritis model. METHODS: Rats

Stimulation of dorsal root ganglia for the management of complex regional pain syndrome: a prospective case series.

BACKGROUND: Complex regional pain syndrome (CRPS) is a chronic and progressive pain condition usually involving the extremities and characterized by sensorimotor, vascular, and trophic changes. Spinal cord stimulation (SCS) is an effective intervention for this condition, but is hampered by the technical challenges associated with precisely directing stimulation to distal extremities. Dorsal root ganglia (DRG) may be more effective as a physiological target for electrical modulation due to recruitment of the primary sensory neurons that innervate the painful distal anatomical regions. METHODS

Active and Passive Bioimplants for Vocal Fold Paralysis.

Vocal fold paralysis is one of the diseases that particularly affect quality of life. While unilateral paralysis leads to glottis closure insufficiency and hoarseness, bilateral paralysis compromises respiration and limits the exercise tolerance. Bioimplants have been used to treat persistent paralysis for over 100 years. The spectrum ranges from autologous tissue transfer and resorbable or permanent injection materials to composite thyroplasty implants and active electrical implants for neurostimulation of the larynx. If bioimplants are used in accordance with the recommendations, the quality

Influence of nerve cuff channel count and implantation site on the separability of afferent ENG.

OBJECTIVE: Recording of neural signals from intact peripheral nerves in patients with spinal cord injury or stroke survivors offers the possibility for the development of closed-loop sensorimotor prostheses. Nerve cuffs have been found to provide stable recordings from peripheral nerves for prolonged periods of time. However, questions remain over the design and positioning of nerve cuffs such that the separability of neural data recorded from the peripheral nerves is improved. APPROACH: Afferent electroneurographic (ENG) signals were recorded with nerve cuffs placed on the sciatic nerve of r

Stance control knee mechanism for lower-limb support in hybrid neuroprosthesis.

A hydraulic stance control knee mechanism (SCKM) was developed to fully support the knee against flexion during stance and allow uninhibited motion during swing for individuals with paraplegia using functional neuromuscular stimulation (FNS) for gait assistance. The SCKM was optimized for maximum locking torque for body-weight support and minimum resistance when allowing for free knee motion. Ipsilateral and contralateral position and force feedback were used to control the SCKM. Through bench and nondisabled testing, the SCKM was shown to be capable of supporting up to 70 N-m, require no more

Ultra-flexible wireless endovascular stimulator for cortical simulation.

Objective. Endovascular neural stimulation (ENS) offers a minimally invasive alternative to conventional intra-cranial implants. However, present ENS devices rely on long transvascular leads, which are prone to foreign body responses, breakage, higher impedance, and noisy input. In this study, we aimed to demonstrate the feasibility of fully wireless cortical stimulation using an ultra-flexible, leadless endovascular stimulator that can be delivered with standard neurointerventional techniques.Approach. We designed an ENS implant integrating a miniaturized receiver coil, passive rectification

Improved selectivity from a wavelength addressable device for wireless stimulation of neural tissue.

Electrical neural stimulation with micro electrodes is a promising technique for restoring lost functions in the central nervous system as a result of injury or disease. One of the problems related to current neural stimulators is the tissue response due to the connecting wires and the presence of a rigid electrode inside soft neural tissue. We have developed a novel, optically activated, microscale photovoltaic neurostimulator based on a custom layered compound semiconductor heterostructure that is both wireless and has a comparatively small volume (<0.01 mm(3)). Optical activation provides a

Implantable tibial neuromodulation with an Anklet for overactive bladder: a review.

INTRODUCTION: Overactive bladder (OAB) is a prevalent chronic condition associated with substantial impairment in quality of life (QoL). Although conservative and pharmacological therapies remain first- and second-line treatment options, long-term adherence and efficacy are often limited by variable treatment response. Implantable tibial neuromodulation (iTNM) using an external wearable ankle stimulator has emerged as a minimally invasive therapeutic strategy combining the durability of implantable neuromodulation with the convenience of wireless home-based therapy. This review provides an ove

Sacral Neuromodulation for Genitourinary Problems.

Sacral neuromodulation (SNM) is a minimally invasive therapeutic option for many voiding dysfunction conditions. It is approved by the US FDA for refractory overactive bladder with and without incontinence and nonobstructive retention. Since SNM has shown a favorable response for these approved indications, other therapeutic applications have been proposed for various conditions such as painful bladder syndrome, chronic pelvic pain and neurological voiding dysfunction in both adult and pediatric age groups. SNM therapy with the most commonly used dedicated SNM device (InterStim) involves inser

Management of a Hypoglossal Nerve, Upper Airway Stimulator in the Pain Patient: Safety Considerations.

BACKGROUND: Upper airway stimulation (UAS) is an emerging neuromodulatory treatment for obstructive sleep apnea (OSA). It involves stimulating the motor fibers of the hypoglossal nerve to prevent airway collapse. Patients with OSA and pain may require radiofrequency ablation (RFA) or electrocautery during surgery. Due to the proximity of UAS leads to cervical facet joints, careful procedural considerations are necessary to prevent complications. This report summarizes best practices for performing RFA or electrocautery in patients with UAS implants, guided by manufacturer safety recommendation

Gasserian Ganglion Stimulation for Facial Pain.

Non-neuralgic trigeminal neuropathic pain can be challenging in terms of treatment as pharmacological interventions often tend to be ineffective. Within the pain-transmitting pathway, the Gasserian ganglion (GG) is a rather unique anatomical and physiological structure where the sensory (including pain) information from the entire half of the face undergoes primary processing in a very compact and clearly defined entity. Moreover, GG is positioned in a completely immobile intradural location (the Meckel's cave) and is insulated from the brain by a layer of dura. As a confluence of all three tr

Long-Term Alleviation of Parkinsonian Resting Tremor Using Wireless Optogenetic Nanonetworks.

Resting tremor is one of the major symptoms of Parkinson's disease, which causes havoc in motor functions of the body, has its genesis in communication impairments in the subthalamic nucleus of the basal ganglia. The modern sophisticated surgical treatments, including electrical deep brain stimulation do not yield satisfactory results due to their inability to provide long-term cure and minimize side effects, such as discomfort and increased infection rates. In this work, we propose a novel system based on the emerging communication technology of wireless optogenetic networks of neural dusts t
