Clubfoot care in low-income and middle-income countries: from clinical innovation to a public health program.

BACKGROUND: Clubfoot occurs in nearly 1 in every 1,000 live births worldwide, representing a significant burden of disease. In high-income countries, an evidence-based treatment protocol utilizing sequential casting was pioneered by Ponseti and has resulted in excellent outcomes among children treated for this condition. However, treatment methods and results of treatment vary greatly across low- and middle-income countries (LMICs). Our goal was to create a framework for understanding how effective programs that treat clubfoot in LMICs choose and organize their activities. METHODS: A systemat

Utilization of evidence-based therapy for acute coronary syndrome in high-income and low/middle-income countries.

Limited data exist regarding the management of patients with acute coronary syndrome (ACS) in high-income countries compared with low/middle-income countries. We aimed to compare in-hospital trends of revascularization and prescription of medications at discharge in patients with ACS from high-income (Canada and United States) and low/middle-income (India, Iran, Pakistan, and Tunisia) countries. Data from a double-blind, placebo-controlled, randomized trial investigating the effect of bupropion on smoking cessation in patients after an enzyme-positive ACS was used for our study. A total of 392

Lessons for low-income regions following the reduction in hypertension-related maternal mortality in high-income countries.

OBJECTIVES: To evaluate pre-eclampsia/eclampsia-associated maternal mortality in high-income countries to understand better the potential improvements in pre-eclampsia/eclampsia-related mortality in low-income countries. METHODS: We searched Medline, PubMed, and the Cochrane Database (1900-2010) using relevant search terms. Studies of the incidence of pre-eclampsia/eclampsia and case fatality rates in various geographic regions were included. The incidence of pre-eclampsia/eclampsia and the pre-eclampsia/eclampsia-associated case fatality rates are presented by location and year. RESULTS: Mo

How much income is enough? Measuring the income adequacy of retired persons using a survey based approach.

This paper uses survey data and a new method proposed by Dutch social scientists to estimate minimum income amounts and income equivalence scales for retired individuals and couples. The method is based on responses to survey questions about the amount of family income needed to achieve various levels of living. The minimum income amounts for retired families obtained using this method do not differ much from comparable official measures of income adequacy. The equivalence scales obtained using this method, however, do differ from official scales and indicate a need to target greater resources

Life-course trajectories and modifiable risk factors for incident walking limitation and mortality in 25 high-income, middle-income, and low-income countries (PURE): a prospective cohort study.

BACKGROUND: Life expectancy is increasing globally, but if people are to age healthily, they must do so with fewer limitations in their daily activities. However, information on either the frequency or risk factors for limitations to walking ability or other key activities across different regions of the world is limited. Our aim was to describe the incidence, trajectories, risk factors, and population-attributable fraction of new-onset walking limitations in 25 countries at all socioeconomic levels. METHODS: PURE is an ongoing, prospective cohort study. The current analysis included communit

Can artificial intelligence bridge the gaps for primary diabetes care in low-income and middle-income countries?

Artificial intelligence (AI) has the potential to improve primary diabetes care in low-income and middle-income countries (LMICs), where the rising burden of disease contrasts sharply with limited health-care resources. Emerging evidence shows the promise of AI for screening, risk prediction, monitoring, and personalised management of diabetes and its complications. However, substantial barriers remain, including infrastructure deficits, data fragmentation, equity and inclusivity challenges, limited prospective validation, and concerns about the acceptability, sustainability, and regulatory ov

Management of Speech and Hearing in Low-Income and Middle-Income Countries: Current Conditions, Problems, Future Directions.

Management of speech and hearing issues in low-income and middle-income countries (LMICs) has been challenging, considering the population size and availability of resources. There is a need to investigate the depth of these challenges and explore the efficacy of strategies to overcome them. This review aimed to delineate the speech and hearing issues in individuals with cleft lip and palate and how they have been managed in the context of LMICs. Keywords were developed based on the objectives of the study to search for literature published during the period 2010 to 2023. Articles from PubMed,

Estimated health effects from domestic use of gaseous fuels for cooking and heating in high-income, middle-income, and low-income countries: a systematic review and meta-analyses.

BACKGROUND: Exposure to household air pollution from polluting domestic fuel (solid fuel and kerosene) represents a substantial global public health burden and there is an urgent need for rapid transition to clean domestic fuels. Gas for cooking and heating might possibly affect child asthma, wheezing, and respiratory health. The aim of this review was to synthesise the evidence on the health effects of gaseous fuels to inform policies for scalable clean household energy. METHODS: In this systematic review and meta-analysis, we summarised the health effects from cooking or heating with gas co

Psychosis, human rights, and legal frameworks: global perspectives, with a focus on low-income and middle-income countries.

The rights of people with psychosis continue to be violated globally, often through practices that are legally and socially sanctioned, including coercive treatments and systemic discrimination. The Convention on the Rights of Persons with Disabilities (CRPD) has catalysed attention to rights-based mental health policy and practice, yet substantial gaps remain between international standards and lived realities. We conducted a systematic search of the literature and a structured narrative synthesis of global evidence on psychosis, human rights, and legal frameworks, focusing on low-income and

The neuropsychiatric toll of rising temperatures on women's health in low-income and middle-income countries: a scoping review.

BACKGROUND: Global warming poses a significant threat to human mental health, with disproportionate effects on vulnerable and highly exposed populations, such as women from low-income and middle-income countries (LMICs). This scoping review mapped available evidence on associations between high ambient temperature and psychiatric, neurologic and neurocognitive outcomes in women from LMICs. METHODS: A systematic literature search was conducted in the electronic databases MEDLINE, Embase, Web of Science, Global Health, CINAHL and Global Index Medicus in December 2024, and updated in September 2

Adverse childhood experiences and suicidality in low-income and middle-income countries: a systematic review and meta-analysis.

INTRODUCTION: Suicide is an important public health problem with a complex aetiology; this includes adverse childhood experiences (ACEs). Common to suicide and ACEs research is a lack of evidence from low-income and middle-income countries (LMICs). This review aims to evaluate associations between ACEs and suicidality in LMICs. METHODS: In this systematic review and meta-analysis, a search for articles investigating associations between any ACEs and suicidal outcomes in LMICs was conducted using Medline, Embase, PsycINFO, CINAHL and Web of Science on 2 December 2022; updated on 14 January 202

Colorectal Cancer Screening and Management in Low- and Middle-Income Countries and High-Income Countries: A Narrative Review.

Colorectal cancer (CRC) remains a leading global health challenge, being a highly prevalent cancer and a major cause of cancer-related deaths worldwide. The incidence of CRC varies significantly between high-income countries (HICs) and low- and middle-income countries (LMICs), with higher rates of incidence but lower mortality in HICs. Factors such as genetic predisposition, lifestyle, and dietary habits play significant roles in CRC development, with the Western diet and limited access to screening contributing to increased incidence. This review highlights disparities in CRC screening, manag

Models of care for sickle cell disease in low-income and lower-middle-income countries: a scoping review.

Sickle cell disease has a growing global burden falling primarily on low-income countries (LICs) and lower-middle-income countries (LMICs) where comprehensive care is often insufficient, particularly in rural areas. Integrated care models might be beneficial for improving access to care in areas with human resource and infrastructure constraints. As part of the Centre for Integration Science's ongoing efforts to define, systematise, and implement integrated care delivery models for non-communicable diseases (NCDs), this Review explores models of care for sickle cell disease in LICs and LMICs.
