New insulating material in maintenance of body temperature.

Flectalon, web of aluminised polyvinylchloride fibres, has been formulated to minimise radiant heat losses and to provide conventional insulation. Critical temperature determinations were used to assess the insulating efficacy of this and other swaddling materials in infants. The critical temperature for a baby 2 to 10 days old was 31 degrees C when naked and 23 degrees C when wrapped in a Silver Swaddler or a sheet and two blankets. The use of a quilt made with Thinsulate or Hollofil with a mass per unit area of 160 to 180 g/m2 reduced the critical temperature to 19.5 degrees C, while Flectal

The Skincubator: A Novel Incubator for Skin-to-Skin Care (SSC) of Premature Neonates, Enables SSC within Humidified Environment and May Improve Thermoregulation during SSC.

This study aimed to assess thermoregulation and humidity within the Skincubator-a novel, wearable skin-to-skin incubator designed to attach to the caregiver.Preterm neonates (PN; born between 24 and 33 weeks gestational age [GA]) received skin-to-skin care (SSC) either via the Skincubator or traditional SSC (t-SSC) with continuous axillary temperature monitoring.Twenty PN were enrolled in the study and treated in the Skincubator. One couple who consented to Skincubator care during delivery subsequently revoked their consent and the baby was excluded from further analysis. Fifty-four paired ses

Olfactory stimulation prevents apnea in premature newborns.

OBJECTIVE: Methylxanthines and doxapram are currently used to treat apnea of prematurity but are not fully effective and often present undesirable side effects. The present study examines whether exposure to an odor known to modulate the infant's respiratory rate could reduce the frequency of apneic spells. METHOD: Fourteen preterm newborns born at 24 to 28 gestational weeks presenting recurrent apnea despite caffeine and doxapram therapy were exposed to a pleasant odor diffused during 24 hours in the incubator. Efficiency of the olfactory treatment was judged by comparing frequency and sever

Maintenance of whole-body therapeutic hypothermia during patient transport and magnetic resonance imaging.

Therapeutic hypothermia has become standard treatment for neonatal hypoxic-ischemic encephalopathy (HIE), with brain MRI commonly performed after the child has been rewarmed. However, early imaging during hypothermia might provide information important in designing clinical trials that refine and personalize therapeutic hypothermia. We tested a protocol to ensure safety and maintenance of hypothermia during in-hospital transport and MRI. MRI during therapeutic hypothermia was performed in 13 newborns on the 2nd-3rd postnatal days. Mean one-way transport time was 20.0 ± 3.3 min. Mean rectal tem

Gangliosides and sialic acid effects upon newborn pathogenic bacteria adhesion: an in vitro study.

The effect of the main gangliosides (GM(1), GM(3), GD(3)) and free sialic acid (Neu5Ac) upon the adhesion of pathogenic bacteria implicated in infant diarrhoea is assessed in vitro using the Caco-2 cell line. Concentrations of the bioactive compounds found in the bioaccessible (soluble) fraction of infant formula and human milk are employed. Bacterial adhesion behaviour included enterotoxigenic Escherichia coli (ETEC), enteropathogenic E.coli (EPEC), Listeria monocytogenes, Salmonella entericaserovartyphi, Shigella sonnei, Campylobacter jejuni and Helicobacter pylori. Three different approache

Keeping babies warm: a non-inferiority trial of a conductive thermal mattress.

BACKGROUND: External thermal support is critical for preterm or ill infants due to altered thermoregulation. Incubators are the gold standard for long-term support and have been adopted successfully in many countries. Alternatives such as radiant warmers, blankets and others are often used as standard of care (SoC) in resource-limited settings when infants are otherwise not in Kangaroo Mother Care (KMC). METHODS: In this pilot study, we evaluate the feasibility of a conductive thermal mattress (CTM) using phase change materials as a low-cost warmer. We conducted a prospective multicentre open

Immediate skin-to-skin contact may have beneficial effects on the cardiorespiratory stabilisation in very preterm infants.

AIM: Our aim was to investigate what effect immediate skin-to-skin contact with a parent had on the cardiorespiratory stabilisation of very preterm infants. METHODS: This randomised clinical trial was conducted during 2018-2021 at two university hospitals with three neonatal intensive care units in Norway and Sweden. Infants born from 28+0 to 32+6 weeks of gestation were randomised to immediate skin-to-skin contact with a parent for the first six postnatal hours or standard incubator care. The outcome was a composite cardiorespiratory stability score, based on serial measures of heart and res

Noncontact optical imaging of brain hemodynamics in preterm infants: a preliminary study.

Extremely preterm infants' hemodynamic instability places them at high risk of brain injury. Currently there is no reliable bedside method to continuously monitor cerebral hemodynamics in the neonatal intensive care unit (NICU). This paper reports a feasibility study to adapt and test an innovative speckle contrast diffuse correlation tomography (scDCT) device for noncontact, high-density, 3D imaging of cerebral blood flow (CBF) in preterm infants. The scDCT scans a focused point near-infrared illumination to multiple source positions for deep tissue penetration, and controls an electron multi

Infants ≤24 weeks are not just smaller extremely preterm infants.

Among the most pressing topics in neonatal-perinatal medicine today is intensive care for infants born at ≤24 weeks' gestation. Infants born at 22-24 weeks comprise ~1 in 500 live births, with ~7500 liveborn infants annually in the U.S.-more common than Down syndrome or critical congenital heart disease-and make up 1 in 5 U.S. infant deaths. Major uncertainties exist about clinical decisions, including regarding obstetric care, delivery room procedures, incubator management, nutrition, respiratory support, and the optimal developmental environment. Partnering with families, we can develop a so

A comparison of fluorescent and nonfluorescent light sources for phototherapy.

We have compared fluorescent and nonfluorescent light sources for phototherapy for newborn infants with hyperbilirubinemia. Phototherapy was provided by a tungsten halogen lamp and conventional fluorescent light with identical radiant flux of 6 microW/sq cm. For 22 infants treated with the nonfluorescent lamp the mean duration of phototherapy was 33.77 hours and the mean reduction of bilirubin was 3.84 mg/100 ml/day. This did not differ significantly from infants treated with conventional fluorescent lights. The nonfluorescent light can be utilized for infants in incubators or on radiant warme

Diagnosis of hereditary spherocytosis in newborn infants.

The normal range of red blood cell osmotic fragility and autohemolysis was determined in venous blood of 32 healthy newborn infants. With these normal ranges as a reference, the diagnosis of hereditary spherocytosis was definitely possible in five newborn infants by demonstration of increased osmotic fragility of fresh and incubated red blood cells, moderately increased autohemolysis, and partial reduction of autohemolysis by the addition of glucose. Most previously studied newborn infants with hereditary spherocytosis have had atypical hyperbilirubinemia. Inconstant signs are anemia, reticulo

[Emergency transport of newborn infants. Organization and experiences in the Bern region].

Mechanisms of cardiorespiratory adaptation to extrauterine life are reviewed and the pathophysiology of impaired adaptation is discussed. An emergency transport service for sick newborns in the region of Berne, Switzerland, is described. It is manned by a specialized team consisting of a pediatric resident trained in anesthesiology and a pediatric intensive care nurse, and in 1974 emergency transport was provided for 135 newborns. Most had respiratory distress and there was a large proportion of prematures; a further large group was made up of newborns with infections and congenital cardiopath

Neonatal primary peritonitis in Nigeria.

Six cases of neonatal primary peritonitis admitted to Ahmadu Bello Hospital, Zaria, between January 1972 and July 1973 are reported, of whom 4 survived. All patients had evidence of umbilical sepsis, while 2 had erythema marginatum, which has not previously been reported in this condition. The importance of peritoneal aspiration and x-ray of the abdomen in establishing the dignosis is stressed, as is prevention of umbilical sepsis by increased health education.

Use of therapeutic hypothermia for term infants with hypoxic-ischemic encephalopathy.

Newborn encephalopathy represents a clinical syndrome with diverse causes, many of which may result in brain injury. Hypoxic-ischemic encephalopathy represents a subset of newborns with encephalopathy and, in contrast to other causes, may have a modifiable outcome. Laboratory research has demonstrated robust neuroprotection associated with reductions of brain temperature following hypoxia-ischemia in animals. The neuroprotective effects of hypothermia reflect antagonism of multiple cascades of events that contribute to brain injury. Clinical trials have translated laboratory observations into

Use and abuse of oxygen in the newborn.

Hypoxia must be prevented in the newborn. It causes atelectasis, acidosis and pulmonary vasoconstriction, which leads to further hypoxia and, ultimately, brain damage. On the other hand, retrolental fibroplasia and bronchopulmonary dysplasia may result from too-vigorous use of oxygen therapy. Frequent blood gas measurements are required. Administered oxygen must be humidified and heated, and the oxygen concentration must be monitored with each delivery system. It is not enough to know the oxygen flow rate; an oxygen analyzer is essential.

A study of visual function in the premature infant.

Within the first week of life visual tracking and pattern differentiation were tested in 34 out of 47 premature infants born between 28--34 weeks of gestation. Positive results were achieved from 30 weeks onwards. Pattern differentiation was tested as soon as infants could be taken out of the incubator. Preference could be observed from 31 weeks post-conceptional age. Both functions showed comparable maturation to full-term controls by 34 weeks gestation.
