Pharmacological Management of Sleep-Wake Disturbances in Delirium.

Delirium is a heterogeneous syndrome primarily characterized by fluctuations in attention and awareness. Sleep-wake disturbances are a common and significant feature of delirium and can manifest as circadian rhythm inversion, sleep fragmentation, and reduced rapid eye movement (REM) and slow-wave sleep. Some literature suggests that the relationship between sleep disruption and delirium is reciprocal wherein the two reinforce one another and may share an underlying etiology. As there are no FDA-approved medications for delirium or delirium-related sleep disturbances, management is primarily fo

Melatonin Receptor Agonists for the Prevention of Delirium: An Updated Systematic Review and Meta-Analysis of Randomized Controlled Trials.

BACKGROUND: Although a previous review illustrated the efficacy of melatonin receptor agonists (MRAs) in preventing delirium, some recent randomized controlled trials (RCTs) did not confirm these effects. OBJECTIVES: This study systematically reviewed the efficacy, acceptability, and tolerability of MRAs for delirium prevention. MATERIALS AND METHODS: We searched electronic databases, including Scopus, PubMed, CINAHL, and Controlled Trials Register, from their inception to February 20, 2022. The primary efficacy outcome was delirium incidence rate after MRA administration; relative risks (RR

[Preparation of brazilein from Caesalpinia sappan by high performance countercurrent chromatography].

Brazilein is among the main chemical constituents of Caesalpinia sappan. It has diverse pharmacological activities. Modern pharmacological studies have shown that the compound has antitumor, anti-inflammatory, antibacterial, antioxidant, immunomodulatory, and other pharmacological activities. Brazilein is often used as a stain in various industries. The separation of brazilein by traditional column chromatography will not only result in contamination of the chromatographic column materials, but also lead to loss of the active ingredient. Countercurrent chromatography is an advanced liquid-liqu

[Effects of ramelteon on a patient with circadian rhythm sleep disorder and mood disorder].

Ramelteon is a novel hypnotic characterized by its action as a melatonin receptor (MT1/MT2) agonist. It has been reported that ramelteon can alter the phase of the sleep period. We report a patient with circadian rhythm sleep disorder and mood disorder who improved with ramelteon. A 25-year-old man had a 5-year history of emotional instability, excessive daytime sleepiness, and difficulty awakening. He had been diagnosed with mood disorder and narcolepsy by a psychiatrist. Sertraline, milnacipran, valproate, and methylphenidate were ineffective, and so he presented to our hospital. Interview d

Sulfadoxine derivatization through multicomponent reactions to obtain new antiplasmodial compounds.

Malaria continues to have a devastating impact on disease-endemic countries. Despite significant advances have been achieved, the discovery and development of new drugs capable to avoid resistances remains a challenge. In the same way, the difficulty and complexity of drug discovery processes only aggravates this situation. Nowadays, new strategies have appeared to minimize the impact that resistance evolution entails, such as the improvement of pre-existing drugs through chemical adaptation. In this work, we describe the structural modification of the antimalarial drug sulfadoxine, to which t

Reaction of phenyl radicals with propyne.

The potential energy surface (PES) for the phenyl + propyne reaction, which might contribute to the growth of polycyclic aromatic hydrocarbons (PAHs) under a wide variety of reaction conditions, is described. The PES was characterized at the B3LYP-DFT/6-31G(d) and B3LYP-DFT/6-311+G(d,p) levels of theory. The energies of the entrance transition states, a direct hydrogen-transfer channel and two addition reactions leading to chemically activated C(9)H(9) intermediates, were also evaluated at the QCISD(T)/ 6-311G(d,p) and CCSD(T)/6-311G(d,p) levels of theory. An extensive set of unimolecular reac

[Impact of biochemisty on the duration of treatment for pulmonary embolus].

Can the biochemical evidence for "new thrombophilic factors" influence the duration of AVK treatment following the occurrence of a first pulmonary embolus? Certainly for the classic but very rare antithrombin defects as well as for the existence of circulating anticoagulant. Possibly for protein C and S defects. On the other hand, the existence of a heterozygotic "Leiden" mutation of factor V, or factor II, and an increase of factors VIII, IX, or XI, do not at present warrant a change in AVK prescription. In effect, in the case where the existence of a thrombogenic state implies a prolongation

[Hypersensitivity and resistance to antivitamins K].

During the initial phase or during treatment with oral anticoagulants, hypersensitivity or resistance may be observed, requiring significant changes in prescribed dosage. A thorough knowledge of the mode of action of vitamin K and its antagonists is useful to improve understanding of unexpected response to oral anticoagulant therapy. These abnormal responses may be due to poor compliance to treatment by the patient, insufficient or excessive vitamin K intake, interference with other drugs or abnormalities of the hepatic enzymes which are the target of oral anticoagulant drugs. The search for a

Evaluation of clinical efficacy and plasma levels of aprindine.

The clinical efficacy of Aprindine, a new antiarrhythmic agent was tested in 37 patients with ventricular and supraventricular arrhythmias. The drug was shown to be effective - disappearance or prevention of relapse of the arrhythmia - in 80% of the cases with ventricular tachycardia, and in 67.7% of the cases with premature ventricular beats. No judgement was possible for supraventricular tachycardia due to the small number of patients. Effectivness of Aprindine was found to be related more closely to the administered dose expressed in mg/kg body weight than to plasma level in steady-state. D

A First Insight into the Biological Activity Profile of Individual Compounds of Valeriana officinalis L.

Valeriana officinalis L. is a widely spread representative of the Eurasian flora. Preparations based on valerian extracts are recognized as sedatives, antidepressants, and anxiolytics by both traditional and modern medicine. Despite the extremely high interest in the biological and pharmacological properties of valerian extracts, the composition of biologically active substances and specific activities of their components are still not completely studied. Therefore, here we address the metabolic profiles of the valerian plants with a special emphasis on their major root constituents with prono

Effectiveness and safety of pharmacologic agents for postoperative delirium: a narrative review.

Postoperative delirium (POD) is a common and distressing neuropsychiatric complication following major surgery, linked to prolonged hospitalization, increased mortality, and long-term cognitive decline. Despite its widespread clinical use, evidence supporting pharmacologic treatments for established POD remains limited and inconsistent. Recent large-scale studies and clinical guidelines published between 2018 and 2025 reveal that neither traditional antipsychotics like haloperidol nor second-generation atypical antipsychotics like quetiapine, olanzapine, and risperidone significantly improve d

Optimizing the extract yield of bioactive compounds in Valeriana officinalis root: a D-optimal design.

It is estimated that 80% of all synthetic drugs are derived from medicinal plants, and nowadays, many synthetic drugs are derived from medicinal plants. Valeriana officinalis can treat many diseases of the nervous system. A crucial aspect of valerian extract is that it inhibits the proliferation of breast cancer cells. To optimize the yield of bioactive compounds in the V. officinalis root extraction, a response surface methodology-based D-optimal design was used. To fulfill this aim, the effects of various factors such as solvent type and concentration, mixing temperature, ultrasound time, an

Ramelteon for delirium prevention in hospitalized patients: An updated meta-analysis and trial sequential analysis of randomized controlled trials.

Although ramelteon has been examined as a relatively new therapeutic option for delirium prevention, current evidence to evaluate its efficacy is limited. We conducted an updated meta-analysis and examine the reliability of existing evidence regarding the effect of ramelteon on delirium prevention in hospitalized patients. Seven major electronic databases were systematically searched to identify randomized controlled trials examining the efficacy of ramelteon in delirium prevention. Data were pooled using a frequentist-restricted maximum-likelihood random-effects model. A trial sequential anal
